PfizerForAll is Pfizer’s consumer-facing platform: one site that runs a savings search across Pfizer products, books telehealth and in-person appointments through partner networks, schedules vaccines, and hands uninsured patients off to Pfizer’s patient assistance arm. It launched as Pfizer’s answer to the direct-to-patient trend, alongside LillyDirect from Eli Lilly and NovoCare from Novo Nordisk.
It is also structured very differently from those two, and that difference is the whole cost story. LillyDirect and NovoCare publish a dollar price for a specific product and ship it to you. PfizerForAll mostly does not. It is a router, and the price lives at whatever it routes you to. This guide separates the paths that carry a published price from the ones that do not, using figures read on September 6, 2026.
What Does PfizerForAll Cost?
| What you are paying for | Low | Average | High |
|---|---|---|---|
| Using PfizerForAll itself (savings search, doctor finder, vaccine scheduler, eligibility quizzes) | $0 | $0 | $0 |
| Online migraine consultation through UpScript | $35 | $35 | $35, plus whatever is prescribed |
| Online COVID-19 consultation through Wheel | not published on PfizerForAll | not published | not published |
| Self-pay discount on 30+ Pfizer brands, redeemed as a GoodRx coupon | varies by drug, dose, quantity and pharmacy | not published on PfizerForAll | varies |
| Paxlovid, uninsured or government-insured, through the USG Patient Assistance Program | $0 | $0 | $0, through December 31, 2026 |
| Paxlovid, commercially insured, with the Pfizer co-pay card | $0 | $0 | $0 until $1,000 of annual savings is used |
| Nurtec ODT, commercially insured, with the co-pay card | $0 for one fill, max 16 tablets | $0 for one fill | $7,000 maximum annual benefit |
| Pfizer Patient Assistance Program (60 medicines), if income is at or below 300% FPL | $0 | $0 | $0 |
| Pharmacy counter comparison: Nurtec ODT list price (Pfizer WAC) | $134.41 per tablet | $1,075.29 per 8-tablet pack | $1,075.29 per 8-tablet pack |
| Pharmacy counter comparison: Paxlovid pharmacy acquisition cost (CMS NADAC) | $49.23 per tablet | $1,477.02 per 30-tablet course | $1,477.02 per 30-tablet course |
The honest headline is that PfizerForAll publishes exactly one price of its own: $35 for the migraine telehealth consultation. Everything else on the site is either free to use or priced somewhere downstream, and pretending otherwise would put a number in your head that no Pfizer page actually stands behind.
What Drives the Price
The self-pay program is a coupon, not a price list
PfizerForAll’s self-pay page states that Pfizer is working with GoodRx to offer savings off list price on more than 30 Pfizer brands, and describes a three-step flow: find your medication, get your coupon, show it to the pharmacist at checkout. The footnote is explicit that savings vary by product, there are no membership fees, and it is not health insurance.
That design means your price is a retail pharmacy’s cash price minus a negotiated discount, so it moves with the store you pick. It is the same mechanic covered in our comparison of GoodRx vs. SingleCare and our guide to what GoodRx actually costs. The brands routed this way as of September 2026 include Eliquis, Nurtec ODT, Paxlovid, Chantix, Premarin, Protonix, Xeljanz, Diflucan, Medrol, Zavzpret and about two dozen others.
The telehealth fee is separate from the drug
The migraine path is the one place PfizerForAll commits to a number. It tells patients to book an online consultation for $35 through UpScript, and adds that you can pay with a credit, debit, prepaid, FSA or HSA card, with payment taken during the visit and no insurance required. The COVID-19 path routes to Wheel instead and only says you provide payment information ahead of booking.
That $35 is a competitive consultation fee. For context, see Teladoc costs and MDLive costs, which sit in a similar band for a self-pay virtual visit. The fee buys the visit, not the prescription.
Co-pay cards require commercial insurance
Both major co-pay cards routed through PfizerForAll are gated on having private, job-based or marketplace coverage. The Paxlovid card can take an eligible commercially insured patient to as little as $0 with a $1,000 annual savings cap. The Nurtec card gives a one-time free fill of up to 16 tablets while benefits are being verified, with a $7,000 annual maximum. If you are uninsured, neither applies to you, and the site correctly points you at assistance instead.
Free medicine is means-tested
Pfizer RxPathways is the piece that matters most for cash payers, and it has hard rules. The Pfizer Patient Assistance Program requires that you be uninsured or government-insured (Medicare, Medicaid, TRICARE or VA), that you have an FDA-approved diagnosis for the product, that you be a US resident, and that your annual pre-tax household income not exceed 300% of the federal poverty level.
RxPathways publishes the current thresholds for the 48 contiguous states: $47,880 for one person, $64,920 for two, $81,960 for three and $99,000 for four. Commercially insured patients are explicitly not eligible for the free-medicine program regardless of how much their plan actually pays. Applicants must submit proof of income such as a W-2, pay stub or prior-year tax return, and Medicare Part D patients must first enroll in the Medicare Prescription Payment Plan.
Cash Price vs. Insurance
For an uninsured patient, the PfizerForAll route with real money behind it is assistance, not the coupon. Sixty Pfizer medicines are available free through the patient assistance program, and Pfizer reports that more than 26,000 patients received over 160,000 free Pfizer prescriptions in 2025. Free beats any percentage off a list price.
Paxlovid is the clearest example. Pfizer’s PAXCESS page states that eligible government-insured and uninsured patients can access Paxlovid free through the US Government Patient Assistance Program through December 31, 2026, with pickup at a participating pharmacy or overnight mail order. Against that, CMS acquisition data put a pharmacy’s own cost for the standard 30-tablet course at about $1,477 as of August 19, 2026, which is why the cash counter price is in four figures. Our full breakdown is in Paxlovid costs.
Nurtec shows the same gap from the other direction. Pfizer’s own statutory price disclosure lists a wholesale acquisition cost of $1,075.29 for a blister pack of eight 75 mg tablets as of March 10, 2026, which is $134.41 per tablet. A self-pay coupon can move that meaningfully but not to zero, while patient assistance can. See Nurtec costs for the current cash spread.
What PfizerForAll Does Not Cover
It does not dispense or ship medication. There is no Pfizer-operated pharmacy behind it, so if you want prescriptions delivered you still need a separate mail order pharmacy.
It does not cover non-Pfizer drugs. The savings search only returns Pfizer products, so a generic you take alongside a Pfizer brand is priced entirely outside the platform.
It does not publish vaccine prices. The vaccine section schedules appointments and runs an eligibility check by age and risk factors, but the money question is answered at the pharmacy or clinic. See vaccine costs without insurance.
And it does not set the price of the drug. Pfizer’s list prices are set upstream and are the subject of separate federal action covered in our guides to Medicare drug price negotiation and most-favored-nation drug pricing.
How to Pay Less
Check assistance before you check the coupon. If your income is at or below 300% of the federal poverty level and you are uninsured or on Medicare, Medicaid, TRICARE or VA coverage, apply to the Pfizer Patient Assistance Program first. Our overview of prescription assistance programs covers what the application asks for and how long it takes.
Ask whether a generic exists. Several drugs on the PfizerForAll list have generic equivalents that cost a few dollars a month at a cash pharmacy. A coupon on a brand rarely beats a generic, and our fair price tool will tell you what the generic should run.
Price the coupon at more than one pharmacy. Because the self-pay discount is applied to each store’s own cash price, the same coupon produces different totals at different counters. Never assume the first pharmacy you check is the cheapest.
Compare the other manufacturer platforms if you take their drugs. LillyDirect and NovoCare Pharmacy publish flat cash prices and ship, which is a different and often better deal structure than a coupon.
Do not let an existing balance sit. If you already have a pharmacy or clinic bill you cannot pay, that debt is negotiable. Our guide to setting up a medical bill payment plan has the language to use.
The Bottom Line
PfizerForAll costs nothing to use and is genuinely useful as a routing tool, especially for finding out in a few minutes whether you qualify for free Pfizer medicine. Treat the $35 migraine consultation as the platform’s only published price, treat the self-pay savings as a coupon whose value you will not know until you check a specific drug at a specific pharmacy, and treat the 300% federal poverty level threshold as the number that actually decides what you pay.
