NovoCare Pharmacy is Novo Nordisk selling semaglutide directly to cash-paying patients. Your prescriber sends the prescription to NovoCare Pharmacy rather than to a retail counter, a licensed dispensing pharmacy fills it under a cash model, and you get free home delivery or pickup at a participating retail location. Novo built the channel in 2025 to compete with compounded semaglutide and to reach people whose plans do not cover the drug.
The prices are published and flat, which makes them easy to compare. What is not obvious from the marketing is that there are two entirely separate NovoCare offers with different eligibility rules and very different numbers, and that most of the current pricing carries a stated end date. This guide covers the cash price for every Wegovy and Ozempic presentation as of September 2026, how it compares to list price and to what pharmacies actually pay, who qualifies, and what happens after the promotional window closes.
What Does NovoCare Pharmacy Cost Without Insurance?
Figures below come from Novo Nordisk’s own NovoCare Pharmacy pages, its printable price guides and its NDC-level terms page, all checked September 6, 2026. A month is one box of four Wegovy pens, one box containing one Ozempic pen, or one bottle of 30 tablets.
| Wegovy or Ozempic, self-pay per month | Low | Typical | High |
|---|---|---|---|
| Wegovy pen (0.25, 0.5, 1, 1.7, 2.4 mg) | $349 | $349 | $349 |
| Wegovy HD pen 7.2 mg | $399 | $399 | $399 |
| Wegovy pill (1.5 / 4 / 9 / 25 mg) | $149 | $199 | $299 |
| Ozempic pen (0.25, 0.5, 1 mg) | $349 | $349 | $349 |
| Ozempic pen 2 mg | $499 | $499 | $499 |
| Ozempic pill (1.5 / 4 / 9 mg) | $149 | $199 | $299 |
| Introductory offer, first 2 fills only (0.25 and 0.5 mg pens, through Dec 31, 2026) | $199 | $199 | $199 |
| List price (WAC), for comparison | $1,027.51 (Ozempic) | n/a | $1,349.02 (Wegovy) |
The $199 introductory price is narrow. Novo’s terms limit it to a maximum of the first two 28-day fills, only at 0.25 mg and 0.5 mg, and only for patients new to the savings offer and to NovoCare Pharmacy. Any third fill moves to the standard price. Novo has published the multi-month math too: $349 a month is $698 for two months and $1,047 for three, and Ozempic 2 mg is $499, $998 and $1,497.
The pills are the cheaper entry point across the board. Oral semaglutide starts at $149 a month, roughly 40 percent below the injectable’s standard $349. If your prescriber is open to either form, that gap is worth raising. See our guides to oral Wegovy cost and Rybelsus cost.
What Drives the Price
Which dose you are on. Wegovy holds at $349 across five of its six injectable doses and jumps to $399 only for the 7.2 mg HD pen. Ozempic holds at $349 through 1 mg and jumps to $499 at 2 mg, a 43 percent increase for the same molecule in the same device. Titration decisions have a real price consequence on Ozempic that they do not have on Wegovy.
Whether you are new to the program. The $199 price is a customer acquisition offer, not a standing rate. Two fills in, everyone lands on the standard ladder. Budget for $349, not $199.
Pen versus pill. A month of the injectable is a box of four pens. A month of the tablet is a bottle of 30. Novo prices them on separate ladders and the tablet ladder is lower at every rung.
The December 31, 2026 date. Novo’s own footnotes state the $199 introductory offer runs for two monthly fills through December 31, 2026, then steps up to $349. Its Wegovy savings-offer page carries a blunter line in plain text: “Pricing to be updated after December 31, 2026.” Treat every number on this page as good through year end and no further. Separately, Novo announced on February 24, 2026 that effective January 1, 2027 it will cut the wholesale acquisition cost of Wegovy, Ozempic and Rybelsus to $675 a month, a reduction of roughly 50 percent for Wegovy and 35 percent for Ozempic. That is a list price change, not a self-pay change, but it signals that the whole pricing structure is in motion. Our guide to most favored nation drug pricing covers the policy behind the shift.
Where you fill it. NovoCare Pharmacy is one of several routes to the same manufacturer price. Novo’s November 2025 announcement said the self-pay prices are also honored at more than 70,000 pharmacies through registration on wegovy.com or ozempic.com, at over 9,000 CVS locations for NovoCare Pharmacy pickup, and through participating partners including Costco, GoodRx, WeightWatchers, Ro, LifeMD and eMed. The price is Novo’s either way. Convenience, not cost, is what differs. Our mail order pharmacy cost guide compares delivery channels generally.
Cash Price vs. Insurance
There are two NovoCare offers and they do not stack.
The savings card is for people with commercial insurance that covers the drug. It brings the copay down to as little as $25 a month, subject to a maximum savings of $100 a month, and Novo excludes anyone enrolled in Medicaid, Medicare, VA, DoD, TRICARE or a similar federal or state program. Marketplace plans, FEHB and state employee plans are not treated as government programs for this purpose.
The NovoCare Pharmacy cash price is the $149 to $499 ladder above, and its eligibility is much broader than most people assume. Novo’s terms say patients must either be uninsured or have insurance, commercial or government-funded, but decide to self-pay. A Medicare beneficiary can use it. The condition is that you agree not to seek reimbursement or submit a claim to any insurer, primary, secondary, commercial, employer, government or supplemental, and you must have a prescription for an FDA-approved indication. NovoCare Pharmacy operates through licensed pharmacies dispensing under a cash model, and insurance is simply not accepted at the counter.
That trade matters most for Medicare. Nothing you pay in the cash channel counts toward the 2026 Part D out-of-pocket cap or your plan deductible, so a Part D member with actual coverage for Ozempic may do better running the claim. Our Medicare Part D guide explains how the cap accrues.
Against list price the cash channel is a large discount. Novo’s own pricing disclosure puts the wholesale acquisition cost at $1,349.02 a month for every Wegovy presentation and $1,027.51 for every Ozempic presentation. CMS National Average Drug Acquisition Cost data effective August 19, 2026 shows pharmacies acquiring Wegovy at roughly $653 per mL for the low-dose pens, about $1,307 for a four-pen month, and Ozempic at about $332 per mL, roughly $996 per three-mL pen. A $349 cash price is around a quarter of Wegovy’s list and about a third of Ozempic’s.
How to Pay Less
Start on the pill if it is clinically appropriate for you. At $149 versus $349 for the standard injectable month, that is $2,400 a year. It is a conversation to have with your prescriber, not a substitution to make on your own.
Claim the two introductory fills before they expire. If you are new to the offer, the $199 price on the first two 0.25 mg and 0.5 mg fills saves $300 against the standard price, and Novo has flagged December 31, 2026 as the deadline.
Compare the competing manufacturer channel on the same day. Tirzepatide and semaglutide are priced independently and the gap moves. Lilly’s Zepbound runs $299 to $449 a month self-pay as of September 2026. Check our LillyDirect cost guide and the full cheapest GLP-1 comparison before committing to a channel.
Do not pay a discount-card price above the manufacturer price. Card prices on brand-name GLP-1s often just surface the same manufacturer self-pay number through a different front door, and sometimes land higher. Our GoodRx vs SingleCare comparison explains how those quotes are built and why they move between sessions.
Check whether you qualify for free medicine first. Novo Nordisk runs a Patient Assistance Program with household income limits that can supply medicine at no cost, which beats any cash price. Eligibility is narrow, it excludes people with commercial insurance, and Novo has been tightening which products Medicare members can get through it. Start with our roundup of prescription assistance programs.
Confirm the number at checkout. These prices have moved repeatedly since November 2025, when Novo cut the standard self-pay price from $499 to $349 and launched the $199 introductory offer. Run your dose through our fair price checker and verify against the live NovoCare page before you pay.
The Bottom Line
For a cash payer, NovoCare Pharmacy is a straightforward discount: $349 a month for a drug with a $1,349 list price, or $149 a month for the low-dose pill. The eligibility rules are looser than the marketing implies, since anyone willing to pay outside their insurance can use it, including Medicare beneficiaries.
The catch is timing. The $199 offer covers two fills, and Novo has told patients pricing will be updated after December 31, 2026. If you are budgeting for a year of therapy, use $349 for Wegovy and Ozempic pens, $499 for Ozempic 2 mg, and treat anything lower as temporary. For the retail-counter comparison, see our guides to Wegovy cost without insurance and Ozempic cost without insurance.
