Most warehouse-club questions are about whether the membership pays for itself. The pharmacy is the one department where that question does not apply, because federal and state pharmacy rules mean a club cannot lock the pharmacy counter behind a paid membership. Costco says so itself, in plain language, on two separate pages of its own website.
What the membership actually buys at the pharmacy counter is a discount program, not access. That distinction drives everything below: what a non-member pays, what a member pays, and why Costco is a genuinely good deal on some prescriptions and a bad one on others. All figures here were read from Costco’s own pages on September 4, 2026.
What Does a Prescription Cost at Costco Pharmacy?
Costco publishes real cash prices in its online drug directory. The page carries an important caveat in Costco’s own words: these are the most recent cash prices, they do not account for insurance or copays, and they “apply only to those prescriptions purchased and mailed from Costco.com.” For a price at the counter in your local warehouse, Costco tells you to call that pharmacy.
The right-hand column is the pharmacy’s acquisition cost, from CMS’s National Average Drug Acquisition Cost survey, effective August 19 to August 26, 2026. It is what the pharmacy pays the wholesaler, not a retail price, and it is the fastest way to see how much of any cash price is dispensing overhead rather than medicine.
| Generic (Costco.com mail order, Sept 4 2026) | 30 tablets | 100 tablets | Acquisition cost, 30 tablets (NADAC) |
|---|---|---|---|
| Amlodipine 5 mg | $45.80 | $46.81 | $0.30 |
| Atorvastatin 10 mg | $36.39 | $42.41 | $0.70 |
| Gabapentin 300 mg (capsules) | $33.97 | $37.74 | $0.90 |
| Levothyroxine 50 mcg | $34.26 | $39.01 | $1.22 |
| Lisinopril 10 mg | $33.35 | $35.28 | $0.55 |
| Metformin 500 mg | $33.39 | $35.40 | $0.43 |
| Metformin 850 mg | $15.44 | $17.98 | $0.72 |
| Sertraline 50 mg | $23.80 | $30.87 | $1.03 |
| Synthroid 50 mcg (brand) | $163.31 | $413.52 | brand, not generic |
Two things jump out. First, a 100-count bottle almost never costs three times a 30-count. Lisinopril 10 mg goes from $33.35 for 30 tablets to $35.28 for 100, a difference of $1.93 for 70 extra tablets. Second, the medicine is a rounding error. Costco charges $33.35 for a bottle of lisinopril whose contents cost a pharmacy about 55 cents.
What Drives the Price
The dispensing fee, not the drug. Nearly the entire cash price of a cheap generic at any retail pharmacy is professional and overhead cost. That is why Costco’s quantity curve is so flat, and it is why the single highest-leverage move at Costco is asking for the largest quantity your prescriber will write.
Member versus non-member. Costco’s CMPP Getting Started page puts it directly: “By law, you do not need to be a Costco member to purchase prescriptions at our pharmacy. However, only members can take advantage of the special pricing offered through the Costco Member Prescription Program (CMPP) and may receive better pricing on certain generic drugs.” A Gold Star membership is $65 a year and an Executive membership is $130 a year plus sales tax, per Costco’s own customer service database.
Mail order versus the warehouse counter. The prices in the table are mail-order prices from costco.com. Costco states outright that warehouse pricing can differ and directs you to call the local pharmacy. Do not assume the online figure is what you will be quoted in person.
Brand versus generic. The Synthroid row above is the clearest illustration in the whole table. Brand levothyroxine at $163.31 for 30 tablets against the Accord generic at $34.26 is a 4.8x gap for the same molecule and strength. Our levothyroxine cost guide walks through why thyroid patients sometimes get told to stay on brand.
Which product Costco happens to stock. Costco warns that prices “may vary due to differences in generic product selection or the bulk package size stocked.” That is why amlodipine 5 mg ($45.80) prices higher than amlodipine 10 mg ($32.98) on the same day, which makes no sense on drug cost and everything on packaging.
Cash Price vs. Insurance
The Costco Member Prescription Program is a discount program, not insurance, and Costco is explicit that it is an either-or choice. Per Costco’s buying guide, CMPP “can only be used as the primary prescription discount,” it cannot be combined with commercial, Medicare Part D or state plans, and other discount cards and coupons cannot be stacked on top of it. Claims submitted to publicly funded programs are not eligible for CMPP.
That makes the counter conversation simple. Ask for both numbers: your insurance copay and the CMPP or cash price. If the cash price is lower, pay cash. Costco’s own buying guide tells members to do exactly this comparison, noting that prescription prices vary from one pharmacy to the next.
One thing worth knowing if you are insured but have a high deductible: a cash purchase generally does not count toward your deductible or out-of-pocket maximum, because no claim is filed. On a $20 generic that rarely matters. On anything expensive, it can.
The other useful CMPP detail is portability. Costco says the program price applies at Costco pharmacies and at thousands of participating neighborhood pharmacies including Albertsons, Kroger, Safeway, Walgreens, Wegmans and Vons, using Costco BIN 018422 and PCN VENTEG. So a member who lives 40 minutes from a warehouse can still use the price at a grocery-store pharmacy down the street.
How to Pay Less
Fill the biggest quantity you can. This is the most reliable saving at Costco specifically, because the price barely moves with count. Ask your prescriber to write a 90-day or 100-count prescription rather than a 30-day one, then compare the per-month math yourself.
Check the CMPP price before you fill, not after. Costco’s member price lookup lives at costco.com/cmpp and covers brand and generic drugs, pet medications, vaccines and some medical supplies. Costco advertises savings “up to 80% or more” depending on the medication, which means the spread between the public cash price and the member price is large on some drugs and nonexistent on others.
Price the same drug at a cost-plus pharmacy. For plain maintenance generics, Costco is often beaten badly. Cost Plus Drugs listed atorvastatin 10 mg at $5.25 for 30 tablets on September 4, 2026, built from $0.22 of manufacturing cost, a $0.03 markup and $5.00 of pharmacy labor, plus $5.25 shipping at checkout. Costco’s $36.39 for the same fill is roughly seven times that. See our Cost Plus Drugs price guide and the Walmart pharmacy price guide for the two cheapest competing routes.
Run a discount card against it. Cards are free and take 30 seconds to check, and on a given drug at a given store they sometimes beat every club price. Our comparison of GoodRx vs. SingleCare covers how the cards differ and where each tends to win, and the GoodRx cost breakdown explains how the free version is paid for.
Do not buy a membership just for the pharmacy without doing the math. At $65 a year, the membership has to save you more than $5.42 a month on prescriptions to break even. On one cheap generic it will not. On a brand-name drug or a family’s worth of fills, it easily can.
If the cash price is still out of reach, ask before you walk away. Community health centers and free clinics dispense many maintenance generics at sliding-scale or no cost through the federal 340B program. Our guide to free and sliding-scale clinics explains how to find one, and if a hospital pharmacy bill is what is hurting, hospital charity care sometimes covers outpatient pharmacy charges too.
The Bottom Line
Costco Pharmacy is worth a phone call from anyone paying cash, member or not, because the counter is open to you either way and the published prices are real numbers rather than a lead form. Its structural advantage is quantity: when 100 tablets cost two dollars more than 30, the smart play is always the bigger bottle.
Its structural weakness is the cheap generic. A drug that costs a pharmacy 55 cents a month is not going to be a bargain at $33 when Walmart will hand it to you for $4 or $9 and a mail-order cost-plus pharmacy will do it for about $10 delivered. Check what your specific drug costs across all three before you commit, and use our fair price checker to sanity-check any quote you are given.
Prices in this guide were read from Costco’s own pages on September 4, 2026. Costco states that CMPP pricing is subject to change without notice and may vary by location, and its limited-time GLP-1 offers (Wegovy tablets from $149 a month and first-fill Ozempic and Wegovy injection pricing at $199 a month) are dated to run through December 31, 2026. Re-check before you rely on any single figure.
