A lip flip is one of the cheapest line items on a med spa menu, and it is priced that way for a structural reason: it is four to eight units of botulinum toxin, not a syringe of filler. The toxin goes into the muscle ringing the upper lip, that muscle relaxes, and the lip rolls slightly outward. Nothing is added, so there is no product volume to pay for.

That single fact explains most of the confusion around lip flip pricing. People who have priced lip filler at $600 to $900 a syringe hear “lip treatment,” brace for a four-figure quote, and then get told $70. This guide covers 2026 published prices both per unit and as flat fees, what a lip flip actually costs over a year once you account for how fast it fades, and where it sits against filler, which is a different product in a different price tier entirely.

What Does a Lip Flip Cost Without Insurance?

National figures below come from the American Society of Plastic Surgeons, from CareCredit’s Botox and lip filler cost guides (the Botox guide was reviewed in July 2026 and is built on the 2026 Synchrony Average Procedural Cost Study), and from the American Academy of Facial Esthetics guide last updated in August 2026. Practice figures are published price lists checked in September 2026.

Sources: the $11 to $35 unit range, the $325 to $710 treatment range, and the $445 average are CareCredit’s 2026 figures; $435 and $743 are the American Society of Plastic Surgeons national averages; $959 and $332 to $2,354 are CareCredit’s lip filler figures; $30 to $120 and $60 to $105 are the American Academy of Facial Esthetics; $65 is Reviv Beauty’s published flat rate, $70 is Trifecta Med Spa’s, $250 is Facile’s; $12 is House of Beauty MedSpa’s under-20-unit rate, $13 is Injector 5280’s Botox rate, $15 is Wilmington Dermatology Center’s standard 2026 rate.

The spread is narrow at the bottom and long at the top. Five of the six practice prices above sit between $48 and $120. The $250 outlier is a boutique clinic that charges a flat $300 for a full wrinkle relaxer treatment, so its lip flip is priced as a scaled-down version of that flat fee rather than as a handful of units.

Sources also disagree on dosing, which matters because dosing is the price. The American Academy of Facial Esthetics puts the common lip flip at 4 to 7 units, Injector 5280 estimates 4 to 8, House of Beauty lists 4, and Reviv Beauty describes 2 to 4 units for a standard flip and 4 to 6 for a fuller one. Take 2 to 8 units as the honest cross-source range.

What Drives the Price

Whether the practice charges per unit or a flat fee

This is the first question to ask and it changes the math completely. A per-unit practice bills you for what goes in, so a light 3-unit flip is genuinely cheaper than a 7-unit one. A flat-fee practice charges the same number either way. Reviv Beauty is explicit about this, advertising $65 “regardless of how many units are needed,” which is a good deal for a heavier dose and a mediocre one for a very light touch.

The per-unit rate at that practice

Once you are in per-unit territory, the rate is everything. CareCredit puts the national spread at $11 to $35 per unit. Real 2026 price lists sit near the bottom of that: $10 to $12 at an El Paso med spa depending on total units, $13 in Denver, $15 in Wilmington, North Carolina, and $16 at a New York City med spa. Multiply your expected units by that number and you have your quote before you walk in.

How many units your lip actually takes

Published ranges run 2 to 8 units, which is a fourfold swing in price at any given rate. A stronger orbicularis oris muscle, a bigger visible change, and adding a gummy smile correction all push the number up. Ask for a unit estimate at the consultation, not after.

Whether the lip flip rides along with a bigger appointment

A lip flip alone is a five to fifteen minute appointment for a $70 ticket, which barely covers the chair. That is why so many practices either set a flat fee that quietly includes the visit overhead, bundle the flip into a larger Botox session, or give it away as an add-on. If you are already booking forehead and frown lines, adding the lip units at the same visit is usually the cheapest path.

Which toxin is in the syringe

Botox, Dysport, Xeomin, Jeuveau, Daxxify, and Letybo are all botulinum toxin type A, but their units are not interchangeable and Allergan says so directly: BOTOX Cosmetic dosing units are not the same as, or comparable to, any other botulinum toxin product. Injector 5280 publishes conversion ratios on its own price list (three Dysport units to one Botox unit, two Daxxify units to one Botox unit), which is why its $4.25 Dysport rate is not the bargain it looks like.

Lip Flip vs. Lip Filler: A Different Product and a Different Price Tier

This is the comparison that matters, and the two are not close. A lip flip is a neuromodulator that changes how your existing lip sits. Lip filler is hyaluronic acid gel that adds volume. Different product, different mechanism, different price tier.

The national numbers show the gap plainly. CareCredit puts one lip filler treatment at a $959 average with a $332 to $2,354 range, and ASPS puts lip augmentation with dermal fillers at a $743 average. Against that, a lip flip at $48 to $250 is a rounding error. Reviv Beauty’s own comparison, on its own price page, sets its $65 lip flip against $545 to $1,200 for its lip filler.

Duration flips part of that advantage back. The same practice puts its lip flip at two to three months and its filler at nine to twelve. Run both to an annual number and the gap narrows: four lip flips a year at $70 is $280, while one filler treatment at the $959 national average is $959. Filler is still the more expensive habit, but it is roughly three times more expensive per year rather than fourteen times more expensive per visit.

If volume is actually what you want, a flip will not deliver it at any price, and paying $250 four times a year for the wrong treatment is worse value than one $700 syringe. Our lip filler cost guide covers per-syringe pricing and the cost to dissolve filler if you change your mind, and the Juvederm cost guide breaks out the individual products. For everything above the lip, our Botox cost guide has the per-unit and per-area numbers.

Cash Price vs. Insurance

There is no insurance angle here at all. A lip flip is elective and cosmetic, no plan covers it, and there is no negotiated rate hiding behind the posted price. Nothing you spend counts toward a deductible or an out-of-pocket maximum.

There is also no FDA-approved indication to point at. Allergan lists the approved BOTOX Cosmetic areas as moderate to severe frown lines, crow’s feet, forehead lines, and the vertical bands connecting the neck and jaw. The lip is not on that list, so a lip flip is an off-label use. Off-label prescribing is legal and routine in aesthetics, but it has one practical cost effect: no manufacturer dosing guidance exists to anchor how many units you should be charged for.

The upside of a market with no insurer in it is that every price is a published cash price quoted before treatment. That makes it easy to comparison shop and easy to walk away, which is more leverage than most healthcare purchases give you.

How to Pay Less

Get the unit estimate before you get the price. Ask two questions at the consult: what do you charge per unit, and how many units do you expect for my lip. A practice that will quote one but not the other is not quoting you a price. Run whatever number you get through our fair price checker before booking.

Add it to an appointment you were already having. The flip itself takes under a minute of injecting. If you are booking forehead, frown lines, or crow’s feet anyway, the marginal cost of four more units at $13 to $15 is $52 to $60 rather than a standalone flat fee. Ask for it to be priced as additional units, not as a separate service.

Join the manufacturer loyalty program first. Allergan’s Allē is free and awards 200 points for each BOTOX Cosmetic treatment, plus access to member savings events like BOTOX Cosmetic Day and seasonal double-point offers. On a $70 purchase the points are small, but they accumulate across every treatment in the portfolio, and enrolling costs nothing.

Ask where the volume break is. Practices that price per unit frequently have a threshold. Wilmington Dermatology Center drops from $15 to $13.50 per unit at 60 units, and House of Beauty MedSpa drops from $12 to $10 at 20 units. If your lip flip is riding along with a larger session, crossing that line lowers the rate on every unit, including the lip ones.

Compare the flat fee against the unit math, not against another flat fee. A $65 flat rate beats 7 units at $15 ($105) and loses to 3 units at $13 ($39). You cannot know which side you are on without a dosing estimate, which is why the first tactic on this list comes first.

Skip the deferred-interest financing. A treatment in this price range should never be financed. If you already carry a balance on a medical credit card for cosmetic work, note that it behaves like ordinary consumer debt rather than medical debt, and our guides to negotiating medical bills and medical bills in collections explain where that line falls and what protections do and do not follow you.

The Bottom Line

Budget $60 to $120 for a lip flip as of September 2026, and treat anything under $50 or over $150 as a pricing model rather than a market rate. The number is small because the purchase is small: a few units of a drug that is already sitting in the practice’s refrigerator, injected in under a minute.

The figure that should actually drive your decision is the annual one. At two to three months per treatment, a lip flip is a $210 to $480 a year habit. That is still well under the $959 national average for a single lip filler treatment, but it is not the $70 impulse buy the sticker suggests. Decide whether you want the flip or the volume first, then price it, because paying repeatedly for the wrong one is the most expensive mistake available here.
