A Brazilian butt lift is two operations sold as one. A surgeon first liposuctions fat out of the abdomen, flanks, back, or thighs, then filters that fat and grafts it into the buttocks and hips. You are paying for both halves, and the grafting is the cheap half. The liposuction is where the operating hours and most of the money go.

That structure explains the strangest thing about BBL pricing, which is that quotes for the same-sounding procedure can differ by a factor of six. As of August 2026, published practice prices run from roughly $3,500 at high-volume Miami practices to more than $22,000 where full 360-degree liposuction is bundled in. This guide separates the surgeon fee from the all-in price, shows what a cheap quote usually leaves out, and covers the costs (second rounds, garments, travel) that surface after the deposit.

What Does a BBL Cost Without Insurance?

Across the sources below, budget $5,000 to $15,000 all-in for a Brazilian butt lift in the U.S. as of August 2026, with a floor near $3,500 in the cheapest markets and premium practices well above $20,000.

The most authoritative national figure comes from the American Society of Plastic Surgeons. Its buttock enhancement cost page, re-checked on September 2 2026, puts the national average surgeon fee for buttock augmentation with fat grafting at $7,264, with buttock implants at $7,964 and a buttock lift at $7,062. Those are single national averages drawn from the society’s latest statistics, not ranges.

Read them carefully. ASPS states that the figure does not include anesthesia, operating room facilities, or other related expenses, so it is a surgeon fee rather than a price. For scale, the 2025 ASPS Procedural Statistics Report counted 25,662 buttock augmentations with fat grafting against 317,196 liposuction procedures, so this is a niche operation priced by a relatively small number of practices.

The Aesthetic Society’s number is lower and measures something different again. Its butt lift associated-costs page puts buttock augmentation at $3,522 from the 2022 Aesthetic Plastic Surgery National Databank, and that figure covers surgeon and facility fees while excluding anesthesia, medical tests, prescriptions, and surgical garments.

Practice-published prices are where the real spread shows up, and they disagree by design rather than by accident. Seattle Plastic Surgery lists a Brazilian butt lift including one area of liposuction at $4,995, with surgeon, anesthesia, and operating room fees included. Avana Plastic Surgery in Miami publishes $3,500 to $5,500. Chicago Breast and Body Aesthetics starts its standard BBL at $10,995. Austin Plastic Surgeon lists $20,000 to $22,500 for a BBL with liposuction of the abdomen, back, and flanks. Those are not four estimates of the same thing. They are four different amounts of liposuction.

What Drives the Price

The number of liposuction areas harvested

This is the single biggest lever, and it is the one most quotes hide. Seattle Plastic Surgery’s own list prices standalone liposuction at $2,995 for the first area and $1,999 for each additional area, so a BBL that bundles one area and a BBL that bundles five are thousands of dollars apart before anyone touches the buttocks.

Chicago Breast and Body’s standard BBL includes Lipo 360 of the abdomen, flanks, and lower back. Austin’s comparable package sits at $20,000 to $22,500, against $13,500 to $15,000 for that same Lipo 360 as a standalone procedure. When you compare quotes, count areas first and dollars second.

Being lean, which costs more rather than less

A common assumption is that a smaller result is a cheaper one. The published prices say the opposite. Chicago Breast and Body starts its “skinny BBL” for patients with limited donor fat at $12,995 against $10,995 for the standard version, because harvesting a usable volume from a lean patient means working more sites for smaller yields.

What the quote actually includes

Chicago publishes an unusually explicit inclusion list: surgeon fee, board-certified anesthesiologist, AAAHC-accredited facility, fat harvesting, fat processing and transfer, compression garments, and follow-up visits, with the statement that the quoted price is the final price. ASPS, at the other end, publishes a surgeon fee and says so. A $7,000 surgeon-only quote and a $10,995 all-inclusive quote can be the same deal or wildly different ones, and you cannot tell without asking.

Geography and practice volume

South Florida is the cheapest large market in the country for this procedure, and the reason is volume. A dense concentration of high-volume cosmetic surgery centers spreads facility overhead across far more cases per month than a boutique practice can. Miami’s published floor of $3,500 is real, and it is roughly a third of the accredited mid-market price elsewhere.

That is worth saying plainly rather than tiptoeing around, because it is a genuine cost-shopping consideration. In 2018 a multi-society task force including ASPS issued a safety advisory estimating BBL mortality at as high as 1 in 3,000, which it described as far greater than any other cosmetic surgery. Standards have tightened considerably since. Florida now requires by statute that gluteal fat grafting use ultrasound guidance during cannula placement, that the office maintain a one-physician-to-one-patient ratio from anesthesia through extubation, that a surgeon not run a second case simultaneously, and that fat extraction and injection not be delegated to staff. Those rules cap how many cases a day a practice can bill, which is part of why prices in the cheapest markets have a floor at all.

Fat survival and the second round

Not all transferred fat stays. Chicago Breast and Body puts long-term survival at 60% to 80%, which means some volume loss is the expected outcome rather than a complication. Whether you want a second round is a separate question, but the price of one is knowable in advance: Austin Plastic Surgeon publishes a returning-patient second round at $11,000 to $13,500 against $20,000 to $22,500 for the first. Very few practices publish that number, which is exactly why you should ask for it in writing.

Implants instead of fat

Buttock implants are the more expensive path, not the budget one. ASPS puts the average surgeon fee for buttock implants at $7,964 against $7,264 for fat grafting, and Austin’s all-in published prices are $14,000 to $15,000 for silicone gluteal implants alone and $24,000 to $25,500 for implants combined with Lipo 360 and fat transfer.

Cash Price vs. Insurance

There is no insurance angle here at all. The Aesthetic Society states directly that because a butt lift is elective cosmetic surgery, insurance does not cover these costs, and every major payer treats buttock augmentation the same way. It is also not an eligible HSA or FSA expense.

The upside of zero coverage is that there is no chargemaster, no network rate, and no explanation of benefits arriving two months later with a number nobody mentioned. Everything is a cash price, quoted in advance, by a business that wants your deposit. That makes it negotiable in a way a hospital bill rarely is, and it means a written quote is an actual commitment rather than an estimate.

The downside is on the financing side. If you put a BBL on a medical credit card and fall behind, none of the protections that attach to hospital billing apply, because this is ordinary consumer debt. Our guide to medical bills in collections covers where those lines fall.

How to Pay Less

Get two all-in written quotes, not two surgeon fees. Ask each practice for one total that names the surgeon fee, anesthesia, facility, liposuction areas included, fat processing, garments, pre-op labs, prescriptions, follow-up visits, and the revision policy. Run both through our fair price checker before you decide.

Count the liposuction areas before you compare dollars. At $1,999 per additional area on Seattle’s published list, a quote that bundles Lipo 360 and one that bundles a single area are not comparable numbers. If you were considering standalone body contouring anyway, our liposuction cost guide has the per-area math.

Shop the market, not just the practice. Miami’s published floor of $3,500 sits roughly $7,000 below accredited mid-market quotes for a comparable operation. Bookimed’s clinic listings put starting package prices at about $3,500 in Turkey and $4,050 in Mexico against $10,000 in the U.S. The savings are real, and so is the fact that revision surgery, complication care, and the flight home are not priced into any of those packages.

Ask what round two costs before you book round one. With 60% to 80% fat survival as the published expectation, a second procedure is a normal possibility rather than a worst case. A practice that will quote it up front (Austin’s $11,000 to $13,500) is giving you information most will not.

Bundle procedures into one operating day. You pay for the facility and the anesthesia once instead of twice, which is the whole economic logic behind combination packages. If you were already weighing more than one procedure, our mommy makeover cost guide and our fat transfer to breast cost guide cover how those bundles are priced.

Negotiate, and decline the deferred-interest card. Practices that push medical credit cards have margin to give up when you say no and offer to pay cash. The same posture that works on any provider bill works here: our guide to negotiating medical bills has the language, and our negotiation script has the call structure.

The Bottom Line

Budget $5,000 to $15,000 all-in for a Brazilian butt lift as of August 2026. Under $5,000 you are almost certainly in a high-volume market with minimal liposuction included. Over $20,000 you are buying full 360-degree body contouring with the fat transfer attached to it.

The useful question is not what a BBL costs. It is how many liposuction areas are in the quote and what is not in the number. ASPS publishes a surgeon fee, The Aesthetic Society publishes surgeon plus facility, and individual practices publish everything from a single-area package to a full-torso operation. Get all of it in writing on one page before you compare anything.
