Symbicort is AstraZeneca’s budesonide and formoterol combination inhaler, a pressurized aerosol prescribed as maintenance therapy for asthma and COPD. The standard 10.2 g canister holds 120 inhalations, which is a 30-day supply at two inhalations twice daily, and it comes in 80/4.5 and 160/4.5 mcg strengths plus a smaller canister.
Symbicort now has three generic competitors, which sounds like it should collapse the price. It has not, at least not the way it did for Advair. This guide prices brand and generic side by side using figures verified on September 2, 2026, plus AstraZeneca’s own posted savings terms, which are unusually favorable to uninsured cash payers.
What Does Symbicort Cost Without Insurance?
The table covers a 30-day 10.2 g inhaler. The middle column is the CMS National Average Drug Acquisition Cost effective August 19, 2026, multiplied out to a full canister, which is what pharmacies pay to stock the product rather than what you pay at the register.
| Product (10.2 g inhaler, 30 days) | Low cash price | Pharmacy acquisition cost (CMS NADAC) | Average retail cash |
|---|---|---|---|
| Brand Symbicort 160/4.5 | $156.05 (SingleCare card) | $259.42 | $483.66 |
| Brand Symbicort 80/4.5 | $156.05 (SingleCare card) | $196.64 | $483.66 |
| Generic budesonide-formoterol 160/4.5 | $156.05 (SingleCare card) | $178.18 | $526.14 |
| Generic budesonide-formoterol 160/4.5, mail order | $204.61 (Cost Plus) | $178.18 | $403.00 (Cost Plus comparison) |
| Generic budesonide-formoterol 80/4.5, mail order | $139.54 (Cost Plus) | $161.34 | $355.00 (Cost Plus comparison) |
| Breyna 160/4.5 (Viatris branded generic) | not separately published | $232.43 | not separately published |
| Brand Symbicort 160/4.5, 6 g canister | $163.41 (Drugs.com) | $152.60 | not published |
| Any AstraZeneca inhaler with the savings card | $35 | not applicable | $35 |
The sources disagree sharply on retail, and it is worth saying so plainly. SingleCare’s brand page puts average retail at $483.66 and its generic page at $526.14, both read September 2, 2026 and both stamped “prices updated 09/03/2026.” Drugs.com, read the same day, puts the brand 10.2 g inhaler at $236.75 and the generic at $194.76 to $289.87. Cost Plus, which publishes its own retail comparison, uses $403.00. The honest cross-source range for an uncovered fill is roughly $195 to $526, and where you land is a function of which pharmacy you walk into.
Every SingleCare page for this molecule, brand and generic, showed the same lowest card price of $156.05 at CVS on September 2, 2026, with Walgreens at $193.68, Publix at $196.18, Food Lion at $213.84 and Walmart at $217.00. Per-pharmacy card prices move between sessions, so treat those chain names as an illustration of the spread rather than a promise.
The AstraZeneca $35 card covers uninsured patients
AstraZeneca capped out-of-pocket costs at no more than $35 per month across its inhaled respiratory portfolio starting June 1, 2024, per its own affordability page. Four inhalers are covered: Airsupra, Bevespi Aerosphere, Breztri Aerosphere and Symbicort.
The terms of use are explicit that this is not only a copay card. Eligible commercially insured patients “or patients who are uninsured who present this savings card at participating pharmacies will pay no more than $35 for each 30-day supply (1 inhaler), subject to a maximum savings limit of wholesale acquisition cost plus usual and customary pharmacy charges.” The patient support site carries a separate download flow that asks you to confirm you do not have commercial insurance, which is the uninsured path.
Three exclusions matter. Anyone in Medicare Part D, Medicaid, Medigap, VA, DoD or TRICARE is out, and the terms say you cannot get around that by asking the pharmacy to run you as a cash payer. The card is void for Massachusetts residents when an AB-rated generic equivalent is available, which now applies to Symbicort. And it is void for California residents when an FDA-approved therapeutic equivalent exists. If you live in either state, price the generic instead.
What Drives the Price
How weak the generic discount is. This is the defining fact about Symbicort’s price. In CMS data effective August 19, 2026, brand Symbicort 160/4.5 costs pharmacies $25.43 per gram and the unbranded generic costs $17.47, a 31% discount. For Advair, the equivalent generic discount is about 59%. Fewer effective competitors on a harder-to-copy aerosol device means less price pressure.
Which generic. They are not priced alike. CMS puts Breyna, the Viatris branded generic on the market since July 2023, at $22.79 per gram, which is closer to brand Symbicort than to the unbranded generic at $17.47. The cheapest acquisition costs in the file belong to AstraZeneca’s own authorized generic and Teva’s new ANDA product, which launched on May 27, 2026 per FDA records.
Canister size. Drugs.com prices the 6 g brand canister at $163.41 and the 10.2 g at $236.75, so the smaller canister costs more per gram. If you are paying cash and can manage the larger up-front payment, the 10.2 g is the better per-dose deal.
Strength. The 80/4.5 runs consistently cheaper than the 160/4.5. Cost Plus shows $139.54 against $204.61, and CMS acquisition data shows $15.82 against $17.47 per gram.
Whether a card is applied. SingleCare’s own spread on the identical inhaler is $156.05 to $526.14. That $370 monthly gap exists purely because most people do not present a free discount card.
Cash Price vs. Insurance
For an uninsured patient, the AstraZeneca card at $35 beats everything else here by a wide margin, and it is worth roughly $450 a month against average retail. There is no scenario in which an eligible uninsured patient should be paying $483 for brand Symbicort.
If the card is closed to you, the fallback ladder is the generic at $139.54 to $204.61 from Cost Plus, then a free discount card at a retail counter at roughly $156, then average retail at $484 to $526. Mail order takes days, so the discount card is the practical answer if you need the inhaler this week.
For an insured patient, the calculation is the usual one. A modest preferred-tier copay is hard to beat. A deductible, a coinsurance percentage or step therapy usually makes the $35 card the better route, with two standard caveats: manufacturer assistance often does not count toward your deductible, and if your plan runs an accumulator or maximizer program, AstraZeneca’s terms say you are not eligible for the card at all. If a pharmacy quotes you a number that does not match anything on this page, our fair price checker will tell you quickly whether it is defensible.
How to Pay Less
-
Download the AstraZeneca savings card before you go to the pharmacy. The uninsured path exists and is documented in AstraZeneca’s own terms at no more than $35 per 30-day inhaler. This is the highest-value step on the page by an order of magnitude.
-
If you are in Massachusetts or California, skip straight to the generic. The card is void in those states when an equivalent generic is available, and Symbicort has one. Cost Plus lists generic budesonide-formoterol at $139.54 for the 80/4.5 and $204.61 for the 160/4.5.
-
Ask the pharmacist for the unbranded generic specifically, not Breyna. CMS acquisition data shows Breyna sitting roughly $54 above the unbranded generic per 10.2 g inhaler. Pharmacies stock what they stock, but it costs nothing to ask which version they carry.
-
Run both discount cards on the exact NDC. The retail spread here is larger than the brand-to-generic spread. Our comparison of GoodRx vs. SingleCare covers how to check both in a couple of minutes, which is what turns a $484 quote into a $156 one.
-
Apply to AZ&Me if you are on Medicare or your income is low. AstraZeneca describes the program as designed for eligible patients who meet the Federal Poverty Level and are without insurance, or who have Medicare. It is the main route for the Medicare enrollees the savings card locks out.
-
Price the prescribing visit and any hospital bills too. A free or sliding-scale clinic can handle the visits that generate and renew the prescription, and if an ER visit or pulmonology bill has already arrived, our medical bill negotiation guide covers what to do before paying it.
Bottom Line
Budget $140 to $526 for a 30-day Symbicort inhaler in September 2026 if you are paying cash without a manufacturer card, and $35 if you qualify for AstraZeneca’s savings card, which explicitly covers uninsured patients. The generic saves real money but nothing like the 60% you get on Advair, so do not assume switching solves the problem on its own.
The practical takeaway is that Symbicort is a coupon drug, not a generic-shopping drug.
