Sildenafil is one of the strangest pricing stories in American pharmacy. Pharmacies buy a 100 mg tablet for about thirteen cents. The average retail price for thirty of those tablets, according to the discount card company that sees the fill data, is $1,369.17. Nothing about the drug changed in between.
That gap is the whole subject of this page. Below is what the tablet costs at each layer of the system as of September 2026, what pharmacies charge, what a discount card does, what mail order costs, and what actually happens if you try the 20 mg tablet route people ask about online. If you are looking for what the brand-name version costs, that is on our Viagra cost page. This is cost information only and not medical advice.
What Does Sildenafil Cost Without Insurance?
The acquisition column is CMS’s National Average Drug Acquisition Cost (NADAC) survey, effective August 19, 2026, which is what retail pharmacies pay wholesalers. The cash figures are published prices read live on September 2, 2026.
| Fill | Low | Average | High |
|---|---|---|---|
| Sildenafil 100 mg, 30 tablets | $6.69 (Cost Plus Drugs) | $12.44 (SingleCare card) | $1,369.17 (SingleCare average retail) |
| Sildenafil 100 mg, 6 tablets | $3.44 (Amazon Prime price) | not published | $319.02 (Amazon retail price) |
| Sildenafil 50 mg, 30 tablets | $6.36 (Cost Plus Drugs) | not published | $1,098.30 (Cost Plus comparison retail) |
| Sildenafil 20 mg, 30 tablets | $6.72 (Cost Plus Drugs) | $11.00 (Amazon Prime price) | $505.80 (Amazon retail price) |
| What pharmacies pay per tablet (CMS NADAC) | $0.05101 (20 mg) | $0.10577 to $0.12196 (50 and 25 mg) | $0.12762 (100 mg) |
| Medicare Part D spending per tablet, 2024 | $1.23 | $1.46 | $2.40 |
The sources broadly agree on the low end and disagree on the high end. Cost Plus Drugs states a comparison retail price of $1,105.50 for thirty 100 mg tablets; SingleCare’s live page the same day put the average retail price at $1,369.17 for the identical fill. Treat $1,106 to $1,369 as the honest range for “what you pay if you hand over your prescription and say nothing.”
What Drives the Price
Markup, not the medicine. At $0.12762 a tablet, the drug in a 30-tablet bottle costs a pharmacy $3.83. Cost Plus Drugs publishes its own arithmetic on that: $1.47 of drug cost, $0.22 of 15% markup and a $5.00 pharmacy labor fee for a $6.69 total, with $5.25 standard shipping added at checkout. Every other price on this page is the same $4 of medicine wearing a different amount of retail.
Which pharmacy you walk into. This is the biggest practical variable and it is almost random. SingleCare’s one-month pharmacy averages for thirty 100 mg tablets on September 2, 2026 ranged from $35.07 at CVS to $766.00 at Walmart to $1,994.10 at Kroger. Those named-chain rows move between sessions and are not stable enough to quote at a specific store, but the pattern holds across every read: two counters a mile apart will quote prices that differ by a factor of fifty.
Whether you present a card. SingleCare’s own discount price for the same thirty tablets was $12.44, and its per-chain card prices on the day ranged from $12.44 to $42.56. Notably, the card price and the cash price at a given chain are not correlated, which is why checking both is worth the two minutes.
Quantity and the fixed dispensing fee. Because the drug is nearly free and the labor fee is not, per-tablet cost falls steeply as the count rises. Amazon Pharmacy’s Prime price was $3.44 for six 100 mg tablets ($0.57 each) and $11.00 for thirty 20 mg tablets ($0.37 each). Cost Plus charges its flat $5.00 labor fee whether the bottle holds thirty tablets or ninety.
Strength barely matters, and not in the direction you would expect. The 100 mg tablet is the most expensive per unit at acquisition ($0.12762) and the 20 mg is the cheapest ($0.05101), but at the register the three ED strengths land within about forty cents of each other at Cost Plus. Asking for a lower milligram strength is not a cost-saving move here.
Cash Price vs. Insurance
For sildenafil, cash usually wins, and the reason is that drug plans treat erectile dysfunction drugs poorly. Medicare Part D has historically excluded ED indications, and many commercial plans either exclude sildenafil or apply a monthly quantity limit of four to six tablets.
CMS’s Part D Spending by Drug file for data year 2024 shows plans paying $1.46 per dosage unit and $46.39 per prescription on the largest generic sildenafil line, across 837,854 claims. That is more per tablet than a cash mail-order pharmacy charges, which tells you how little leverage the insurance route has on a drug this cheap.
The practical rule: ask the pharmacist for the cash price before you hand over an insurance card. On a $6 to $15 drug, running the claim mostly buys you a quantity limit. The usual caveat applies, which is that a cash fill does not count toward your deductible or out-of-pocket maximum, so if you are insured and headed for an expensive medical year anyway, the calculation can flip.
Our fair price checker shows the range for this drug so you can tell a normal quote from an outlier. Anything above about $50 for a 30-tablet fill is a markup question, not a drug cost question.
How to Pay Less
Price a cash mail-order pharmacy against a discount card, every time. These are two different pricing systems and the winner changes by drug and by quantity. On September 2, 2026, Cost Plus Drugs charged $6.69 for thirty 100 mg tablets plus $5.25 shipping, while SingleCare’s card price for the same thirty was $12.44 with no shipping. Our guide to GoodRx vs. SingleCare explains how to run both checks quickly.
Ask for the largest quantity your prescriber will write. The dispensing fee is fixed and the drug is nearly free, so a 90-count fill costs barely more than a 30-count fill at a transparent pharmacy. This is also the fix for insurance quantity limits, since a cash fill is not bound by them.
Ask what the pharmacy’s cash price is, out loud, before the claim is run. Pharmacies are not required to volunteer the cheaper of two prices. On a drug where chain quotes for the identical fill ranged from $35 to $1,994 in a single day’s data, the question is worth asking at more than one counter.
Check whether your existing memberships already cover it. Amazon’s Prime prescription benefit dropped the 20 mg sildenafil price from a $505.80 retail listing to $11.00 for a 30-day supply, and the 100 mg six-tablet supply from $319.02 to $3.44. Warehouse club pharmacies are frequently the low quote in the same way and generally do not require a membership to use the pharmacy counter.
If you do not have a prescriber, price the visit, not just the drug. A sliding-scale clinic visit at a federally qualified health center is means-tested and often costs less than a telehealth subscription’s first month. See free clinics and sliding-scale care. If you already have an unpaid pharmacy or clinic balance, the scripts in how to negotiate medical bills work on those bills the same way they work on hospital ones.
The 20 mg Revatio Tablet Question
This comes up constantly, so here are the numbers. Sildenafil 20 mg is the strength approved for pulmonary arterial hypertension, sold under the brand name Revatio, and it is dramatically cheaper per milligram at wholesale. CMS acquisition data prices it at $0.05101 a tablet against $0.12762 for the 100 mg ED tablet.
Per milligram, that looks like a bargain. Per dose, it is not. Five 20 mg tablets cost about $0.26 at acquisition versus $0.13 for a single 100 mg tablet, so the arithmetic runs backward. At the register it is a wash: Cost Plus Drugs charged $6.72 for thirty 20 mg tablets and $6.69 for thirty 100 mg tablets on the same day, and Amazon Pharmacy’s Prime price for thirty 20 mg tablets was $11.00.
The pricing conclusion is simple: in 2026 there is no cash-price advantage to the 20 mg route, because generic sildenafil at every ED strength has already collapsed to roughly the price of a sandwich. What strength you take, and whether tablets should ever be split, is a decision for your prescriber and pharmacist, not something a cost page should weigh in on.
Bottom Line
Generic sildenafil should cost you between $6 and $15 for a 30-tablet supply in September 2026, and it will cost you $1,100 to $1,400 if you fill it at the wrong counter without asking. Pharmacies pay about thirteen cents a tablet, so the entire question is markup. Check a cash mail-order pharmacy and a discount card side by side before every refill, ask for the largest quantity your prescriber will write, and compare a telehealth subscription like Hims against the cost of a single sliding-scale visit plus a $6.69 bottle before you commit to a monthly plan. If sildenafil is not the fit, Cialis cost covers the tadalafil pricing on the same basis.
