Sertraline is one of the most dispensed prescriptions in the country and one of the cheapest. CMS’s Medicare Part D data for 2024 records 16.1 million sertraline prescriptions covering 3.2 million beneficiaries at an average of $9.14 per prescription. Brand-name Zoloft, the same molecule, averaged $1,051.21 per prescription across just 13,450 claims.

That gap is the whole story of this page. Almost everything you pay for a month of sertraline is dispensing overhead rather than the drug itself, which means the pharmacy you choose matters more than anything else. Every figure below was pulled on September 4, 2026 from CMS acquisition data, Walmart’s published generic program document, Cost Plus Drugs’ itemized pages and drugs.com. This is a cost guide, not medical advice, and nothing here is a recommendation about which medication you should take.

What Does Sertraline (Zoloft) Cost Without Insurance?

In the table below, Low is the cheapest verified published price for that quantity, Average is the price a typical cash customer sees, and High is the top of the published retail range. The acquisition figures behind several rows come from CMS’s National Average Drug Acquisition Cost file effective August 19, 2026, which is what retail pharmacies pay to buy the drug and which carried forward unchanged into the September 2, 2026 file.

Form and quantityLowAverageHigh
Sertraline 25 mg, 30 tablets$5.66$9.00$10.94
Sertraline 50 mg, 30 tablets$4.81$5.69$10.15
Sertraline 100 mg, 30 tablets$5.04$6.04$10.19
Sertraline 50 mg or 100 mg, 90 tablets$5.95$14.96$24.00
Sertraline 150 mg, 30 capsules$22.24$41.49$175.71
Sertraline 200 mg, 30 capsules$22.25$59.65$175.71
Sertraline oral concentrate 20 mg/mL, 60 mL$22.25$23.79$28.58
Brand Zoloft 25, 50 or 100 mg, 30 tablets$447.66$460.80$466.25

Two things jump out. First, the drug itself is close to free. A pharmacy buys thirty 50 mg tablets for $1.03 and thirty 100 mg tablets for $1.51 at CMS acquisition prices. Cost Plus Drugs shows this openly, itemizing its $5.69 fill for thirty 50 mg tablets as $0.60 of manufacturing, $0.09 of markup and a flat $5.00 pharmacy labor fee, with $5.00 standard shipping added at checkout.

Second, the capsule strengths behave like a different drug. The 150 mg and 200 mg capsules cost pharmacies 40 to 60 times what a 50 mg tablet costs, and drugs.com prices thirty of either at $107.13 to $175.71 while Cost Plus charges about $22. That is one of the widest cross-source spreads on this site, and it is worth checking both before you fill.

What Drives the Price

Brand versus generic. This is the single biggest lever, and it is not close. Brand Zoloft costs about $466 for thirty tablets at drugs.com against $4.81 to $10.94 for generic sertraline. CMS acquisition data effective August 19, 2026 prices brand Zoloft at $14.92198 per 50 mg tablet and $14.95041 per 100 mg tablet, so the retail counter price is only a few percent above what the pharmacy itself paid. There is very little room to negotiate a brand fill down.

Strength, and specifically whether it is a tablet. Sertraline tablets come in 25 mg, 50 mg and 100 mg, and all three sit within a few dollars of each other. The 150 mg and 200 mg presentations are capsules made by a much smaller set of manufacturers, and they are priced accordingly. If you take 200 mg, ask whether your prescription can be written as two 100 mg tablets rather than one 200 mg capsule, which is a billing question rather than a clinical one and is worth raising with your prescriber.

Where you fill it. The same thirty 50 mg tablets range from $4.81 to $10.15 across ordinary retail pharmacies in drugs.com’s price guide, and mail-order pharmacies that publish flat prices land near the bottom of that range. Because the drug costs the pharmacy about a dollar, almost the entire price you pay is that pharmacy’s dispensing fee, and dispensing fees are not standardized.

Quantity per fill. Larger fills spread the dispensing fee further. Drugs.com prices ninety 50 mg tablets at $5.95 to $14.86, which is roughly the cost of a single 30-count fill at some pharmacies. A pharmacy that charges a flat labor fee per fill, as Cost Plus Drugs does at $5.00, rewards 90-day prescriptions in exactly the same way.

The form. The 20 mg/mL oral concentrate exists for people who cannot swallow tablets. A 60 mL bottle runs $22.25 at Cost Plus Drugs and $28.58 at drugs.com, against $23.79 at CMS acquisition prices. Brand Zoloft concentrate is $285.36 for the same 60 mL.

Cash Price vs. Insurance

For generic sertraline, paying cash is very often cheaper than running it through a plan. A typical commercial generic copay of $10 to $15 is already at or above the full cash price at a mail-order pharmacy, and a high-deductible plan will make you pay the whole negotiated amount anyway until the deductible is met. If your copay is more than about $10, ask the pharmacist what the cash price is before you hand over the card.

The exception is the brand. At roughly $466 a month for Zoloft, insurance genuinely matters, and so does the fact that plans almost universally require the generic first. If you have been told you need brand Zoloft specifically, that is a prior-authorization conversation with your prescriber rather than a shopping problem.

One caution on discount cards. Card prices are quoted per pharmacy and can change between visits, so treat any single quoted card price as an estimate rather than a fixed rate, and compare the card price against the pharmacy’s own cash price before deciding. Our guide to comparing discount cards walks through how the two big programs differ and when a card actually beats paying cash.

Note also that a manufacturer savings card is not a cash-payer tool in the way people expect. Viatris limits the ZOLOFT Savings Offer to $150 per fill and $1,800 per calendar year, excludes anyone whose prescription is paid by Medicare, Medicaid, TRICARE or VA benefits, and is not valid for Massachusetts residents. Against a $466 fill, the best case still leaves roughly $316 to pay.

How to Pay Less

Fill the generic, and confirm the pharmacy actually substituted it. Generic sertraline costs about one percent of brand Zoloft. If your receipt says Zoloft, ask the pharmacy to re-run it as sertraline unless your prescriber has written that substitution is not permitted.

Use Walmart’s generic program if your strength qualifies. Walmart’s Retail Prescription Program document, effective March 24, 2025, lists sertraline 25 mg and 100 mg at $9 for a 30-day supply and $24 for a 90-day supply. Note the strength limitation carefully: the 50 mg tablet does not appear on the list at any tier, so the program helps only some prescriptions.

Price the mail-order pharmacies that publish flat rates. Cost Plus Drugs charges $5.66, $5.69 and $6.04 for thirty 25 mg, 50 mg and 100 mg tablets as of September 4, 2026, plus $5.00 shipping. Because the shipping and the $5.00 labor fee are per order rather than per tablet, a 90-day prescription cuts the per-month cost substantially.

Ask for a 90-day prescription. Every pricing structure on this page, flat-fee or not, rewards fewer fills. Drugs.com prices ninety 50 mg tablets at $5.95 to $14.86, which is a month’s price at some counters.

Compare a card price against the cash price, not against the list price. Discount cards advertise savings off an inflated benchmark. On a drug that already costs $6, the card may save nothing. Our fair price checker gives you a reference range to argue from.

If cost is the barrier to getting the prescription at all, start with a community health center. Federally qualified health centers and free clinics run sliding-scale fees for both the visit and, often, the medication. Our guide to free and sliding-scale clinics explains how to find one and what to bring. A psychiatrist visit is usually the larger expense on this path, not the prescription.

The Bottom Line

Sertraline is one of the least expensive prescriptions in American pharmacies, and the difference between a good price and a bad one is a few dollars rather than a few hundred, as long as you are filling the generic tablet. Budget roughly $5 to $11 a month, less if you fill 90 days at a time.

The three places to be careful are the 150 mg and 200 mg capsules, where the spread runs from $22 to $176 for the same thirty capsules; the 50 mg tablet’s absence from Walmart’s program list; and brand Zoloft, which at about $466 a month is the only version of this drug that can create a real bill. If you want to compare across the class, see our antidepressant cost guide, the companion page on Lexapro and escitalopram, or trazodone, which is priced even lower.
