Escitalopram is the generic version of Lexapro, and the price difference between the two is one of the largest for any commonly prescribed drug. CMS’s Medicare Part D data for 2024 records 12.7 million escitalopram prescriptions covering 2.7 million beneficiaries at an average of $11.11 per prescription. Brand Lexapro, the same molecule, averaged $867.93 per prescription across 20,281 claims.
Both products are approved for the same uses. AbbVie’s prescribing information lists Lexapro as indicated for major depressive disorder in adults and patients 12 and older, and generalized anxiety disorder in adults and patients 7 and older. What differs is what you pay at the counter. Every figure below was pulled on September 4, 2026 from CMS acquisition data, Cost Plus Drugs’ itemized pages, drugs.com and Walmart’s published program document. This is a cost guide, not medical advice.
What Does Lexapro (Escitalopram) Cost Without Insurance?
In the table below, Low is the cheapest verified published price for that quantity, Average is the price a typical cash customer sees at a retail counter, and High is the top of the published retail range. Where a row shows the same figure twice, only one published cash price exists for that quantity.
| Form and quantity | Low | Average | High |
|---|---|---|---|
| Escitalopram 5 mg, 30 tablets | $5.79 | $8.80 | $8.80 |
| Escitalopram 10 mg, 30 tablets | $6.24 | $9.02 | $9.29 |
| Escitalopram 20 mg, 30 tablets | $6.31 | $8.90 | $13.10 |
| Escitalopram 10 mg or 20 mg, 90 tablets | $12.27 | $14.73 | $23.70 |
| Escitalopram 15 mg, 30 capsules | $159.17 | $159.17 | $162.22 |
| Escitalopram oral solution 5 mg/5 mL, 240 mL | $18.63 | $28.55 | $67.91 |
| Brand Lexapro 5, 10 or 20 mg, 30 tablets | $454.80 | $475.50 | $495.90 |
The brand row is derived from the per-tablet prices drugs.com publishes, because it lists Lexapro only in 100-count bottles: $1,516.19, $1,584.90 and $1,653.48 for a hundred 5 mg, 10 mg and 20 mg tablets. CMS acquisition data effective August 19, 2026, which carried forward unchanged into the September 2, 2026 file, puts brand Lexapro at $15.41933 per 10 mg tablet and $16.12427 per 20 mg tablet. Retail sits only a few percent above acquisition, which is why brand Lexapro is essentially non-negotiable at the counter.
The generic tells the opposite story. CMS acquisition prices are 4.2 cents for a 5 mg tablet, 3.9 cents for a 10 mg tablet and 6.3 cents for a 20 mg tablet, so thirty tablets cost the pharmacy between $1.18 and $1.88. Cost Plus Drugs itemizes its $6.24 fill for thirty 10 mg tablets as $1.08 of manufacturing, $0.16 of markup and a flat $5.00 pharmacy labor fee, with $5.00 standard shipping added at checkout.
What Drives the Price
Brand versus generic. Nothing else on this page comes close. Brand Lexapro runs about $475 for a 30-tablet month against roughly $6 to $13 for generic escitalopram. Because the brand’s retail price tracks its acquisition cost closely, shopping around does very little for it. Switching to the generic does everything.
The dosage form, not the strength. Escitalopram tablets at 5 mg, 10 mg and 20 mg are all priced within about a dollar of each other. The 15 mg capsule is a separate product with only three national drug codes in CMS’s file, priced at $5.40748 each, about $162 for thirty, and drugs.com prices thirty at $159.17. That is more than twenty times the cost of the tablets for the same daily dose range.
The oral solution. The 5 mg per 5 mL liquid exists for people who cannot take tablets. A 240 mL bottle costs $18.63 at Cost Plus Drugs and $67.91 at drugs.com, against about $28.55 at CMS acquisition prices. The 3.6x spread between those two published prices is the widest on this page for a routine product, so price the liquid at more than one pharmacy.
The pharmacy’s dispensing fee. With the drug itself costing a pharmacy between $1.18 and $1.88 for a month, essentially your entire bill is the fee for filling it. Those fees are not standardized and are not published, which is why the same thirty 20 mg tablets range from $8.90 to $13.10 across ordinary retail in drugs.com’s price guide.
Fill size. Drugs.com prices ninety 10 mg tablets at $12.27, less than the cost of two 30-count fills at most counters, and Cost Plus Drugs charges its $5.00 labor fee and $5.00 shipping per order rather than per tablet. A 90-day prescription is the single easiest saving on this drug.
Cash Price vs. Insurance
For generic escitalopram, the cash price is frequently lower than a copay. A common commercial generic copay of $10 to $15 already meets or exceeds the full cash price at a mail-order pharmacy, and under a high-deductible plan you pay the negotiated amount in full until the deductible is met anyway. Ask the pharmacist for the cash price before you present a card, and compare.
For brand Lexapro at roughly $475 a month, coverage genuinely matters, and virtually every plan will require you to try the generic first. There is no cash-market workaround for the brand, because the retail price is close to what the pharmacy paid.
Discount cards are worth checking but not worth assuming. Card prices are quoted per pharmacy and can differ between visits, so treat a quoted card price as an estimate rather than a fixed rate. Our guide to comparing discount cards explains where the two big programs differ and when a card actually beats the cash price. On a $6 generic, it often does not.
One structural note worth knowing: escitalopram does not appear anywhere on Walmart’s generic program list, while its close relative citalopram does, at $9 for 30 days and $24 for 90 days. If your out-of-pocket cost is the deciding factor, that is a fact worth mentioning to your prescriber, who will weigh it against clinical considerations that are outside the scope of this page.
How to Pay Less
Fill the generic and check the receipt. Generic escitalopram costs roughly one to two percent of brand Lexapro. If your label says Lexapro, ask the pharmacy to re-run it as escitalopram unless your prescriber has marked the prescription as dispense-as-written.
Price the flat-rate mail-order pharmacies. Cost Plus Drugs charges $5.79, $6.24 and $6.31 for thirty 5 mg, 10 mg and 20 mg tablets as of September 4, 2026, plus $5.00 shipping. Because the shipping and the $5.00 pharmacy labor fee are charged per order, a larger fill lowers the effective monthly cost.
Ask for 90 days at a time. Drugs.com prices ninety 10 mg tablets at $12.27, roughly what a single month costs at some retail counters. This is the highest-leverage change most people can make on this prescription.
Avoid the 15 mg capsule if a tablet combination works. At about $160 for thirty capsules against about $6 for thirty tablets, this is the one strength on the escitalopram menu that can produce a real bill. Whether a different strength is appropriate is a clinical question for your prescriber, but the cost difference is worth raising.
Check the cash price against the card price rather than against the list price. Discount programs advertise savings measured from an inflated benchmark. Our fair price checker gives you an independent reference range so you can tell whether a quoted price is reasonable.
If the appointment is the real barrier, start with a community health center. Federally qualified health centers and free clinics run sliding-scale fees for the visit and often for medications too. Our guide to free and sliding-scale clinics covers how to find one. On this class of drug, the prescriber visit is nearly always the bigger line item, and so is therapy if you are budgeting for both.
The Bottom Line
Generic escitalopram belongs in the same budget tier as a couple of coffees: roughly $6 to $13 for a 30-day supply, and about $12 to $24 if you fill 90 days at a time. The brand is the outlier at roughly $475 a month, and it is priced so close to what pharmacies pay that shopping around will not rescue it.
Watch two things specifically: the 15 mg capsule, which costs more than twenty times what the tablets cost, and the oral solution, whose published price varies by more than three times between pharmacies. For a comparison across the class, see our antidepressant cost guide or the companion page on sertraline and Zoloft.
