Phalloplasty is the most expensive procedure in gender-affirming care, and the reason is structural rather than mysterious. It is not one operation. It is a course of two, three or occasionally four operations spread across twelve to eighteen months, and every one of them generates its own surgeon bill, its own facility bill and its own anesthesia bill.
That is why a single quoted number almost never describes what you will actually pay. This guide separates the per-stage arithmetic from the full-course total, prices metoidioplasty alongside it as the cheaper alternative, and is honest about the fact that in much of the country phalloplasty is not sold for cash at all.
What Does Phalloplasty Cost?
Figures verified September 4, 2026. Claims-based totals are the most reliable full-course evidence available, because they capture every stage the same patient went through.
| Line item / source | Low | Typical | High |
|---|---|---|---|
| Total cost per person, all stages (OptumLabs commercial claims) | n/a | $137,893 | n/a |
| Total cost per procedure, single stage (OptumLabs) | n/a | $65,318 | n/a |
| Total cost per episode (JAMA Surgery, MarketScan, 2019 dollars) | $74,679 | $148,540 | $240,844 |
| Metoidioplasty, published cash-pay range (Gender Confirmation Center) | $19,000 | $30,500 | $42,000 |
| Metoidioplasty, simple release, one practice (verified 7/8/2026) | $11,960 | $11,960 | $11,960 |
| Metoidioplasty, ring technique, same practice | $26,280 | $26,280 | $26,280 |
| Medicare hospital outpatient facility rate, CPT 55980 (July 2026 OPPS) | $3,601.33 | $3,601.33 | $3,601.33 |
| Medicare surgery center facility rate, CPT 55980 (July 2026 ASC) | $1,723.02 | $1,723.02 | $1,723.02 |
| Medicare hospital outpatient facility rate, penile prosthesis (CPT 54405) | $21,175.04 | $21,175.04 | $21,175.04 |
| Three-stage package abroad (Mexico Transgender Center, Kim phalloplasty) | $31,999 | $31,999 | $31,999 |
The single most useful number on that table is the pair from OptumLabs: $65,318 per procedure but $137,893 per person. Same dataset, same patients. The 2.1x multiplier between them is the staging, quantified.
Why Staging Is the Whole Cost Story
A typical course looks something like this: a first stage that creates the phallus from a donor flap (most often radial forearm or anterolateral thigh), a second stage that builds the urethra and the scrotum, and a third stage that places testicular and erectile implants. Some people stop after the first or second. Some need a revision stage for a urethral fistula or stricture.
Medicare’s own fee files show how differently those stages price. Using the July 2026 relative value file and its $33.4009 conversion factor, the surgeon side of each component is modest:
- Free microvascular flap (CPT 15757): about $1,946
- Urethroplasty (CPT 53430): about $873
- Complicated scrotoplasty (CPT 55180): about $624
- Testicular prosthesis (CPT 54660): about $334
- Multi-component inflatable penile prosthesis (CPT 54405): about $729
The facility side is where the money is. In the July 2026 hospital outpatient file, CPT 54405 pays the facility $21,175.04, the urethroplasty codes pay $5,477.93, scrotoplasty pays $3,601.33 to $5,477.93 depending on complexity, and the base masculinizing genital code 55980 pays $3,601.33. The free-flap codes carry status indicator C, meaning Medicare does not pay them in an outpatient setting at all: that stage is an inpatient admission, with an inpatient bill attached. All of these rates reset on October 1, 2026.
Stack three of those facility bills, add three surgeon fees, three anesthesia bills and one or more nights at hospital rates that run per day, and the six-figure per-person total stops looking surprising.
There is no national Medicare price for the procedure itself. CPT 55980 carries status code C in the 2026 relative value file: zero assigned relative value units, a YYY global period, and pricing left to each Medicare contractor case by case. There is, in other words, no published benchmark surgeon rate for a cash payer to negotiate against, which is unusual even by American healthcare standards.
Metoidioplasty: The Priced Alternative
Metoidioplasty uses existing hormonally enlarged tissue rather than a donor flap. It is usually one operation instead of three, and it sells in the open cash market in a way phalloplasty does not.
The Gender Confirmation Center posts $19,000 to $42,000 for metoidioplasty and labels the range “for cash pay rates only.” A surgeon pricing directory updated September 3, 2026 lists a Pennsylvania practice at $11,960 for a simple release and $26,280 for a ring metoidioplasty, both verified July 8, 2026, and independently corroborates the $19,000 to $42,000 range at a San Francisco practice. Abroad, a Guadalajara clinic publishes $5,999 for metoidioplasty without implants, $7,999 with testicular implants and $8,999 with implants and vaginectomy.
The spread inside metoidioplasty is driven by what gets bundled in. A simple release is the phallus alone. A full metoidioplasty adds urethral lengthening for standing urination, and often vaginectomy, scrotoplasty and implants, each of which is a separate CPT code with its own facility charge. Ask which of those are inside a quote before comparing two numbers.
Many people also have a hysterectomy in the same period, which OptumLabs put at a mean total of $16,050 per procedure.
Cash Price vs. Insurance
Here the honest answer is uncomfortable. One of the few US practices that publishes cash-pay rates for gender surgery states directly on its fee page that it does not offer a cost estimate for phalloplasty, because the hospitals it partners with only accept patients who can pay through insurance coverage. Note that the same practice’s blog carries a phalloplasty price range that its own fee page contradicts. The fee page is the operative document, so treat the blog number as stale.
That is the market. Phalloplasty is routed through insurance nearly everywhere in the United States, and a self-pay patient’s realistic options are to obtain coverage, travel, or fund the gap. A Guadalajara clinic that publishes a full price list posts a three-stage Kim phalloplasty at $31,999, which is roughly a quarter of the US per-person claims average, but medical tourism carries its own costs and risks, including the fact that revision and complication care will be far from home.
Coverage criteria are published and worth reading before you spend money on anything else. Aetna’s bulletin requires a signed letter from a qualified mental health professional, documented and sustained gender dysphoria, capacity to consent, and six months of continuous hormone therapy (twelve for people under 18) for genital reconstructive surgery. It also lists a limited number of electrolysis or laser hair-removal sessions as medically necessary when they are preparing a skin graft for genital surgery, which matters because forearm donor sites usually need clearing.
Even with coverage the bill is not zero. Out-of-pocket costs averaged $3,982 per person in OptumLabs claims and a median of $2,120 in the JAMA Surgery data, and half of phalloplasty patients in the latter traveled out of state, which was associated with 49% higher out-of-pocket costs across both procedures studied.
How to Pay Less
Price the stages separately, in writing. A quote covering “phalloplasty” may cover stage one only. Ask for a per-stage schedule naming the CPT codes, and ask what a revision stage would cost if a fistula or stricture develops.
Ask whether any stage can move to an ambulatory surgery center. Medicare pays a surgery center $1,723.02 for the facility side of CPT 55980 against $3,601.33 at a hospital, and $18,018.51 against $21,175.04 for the penile prosthesis code. Cash quotes broadly track those ratios.
Apply for a surgery grant, and apply early. Point of Pride’s Annual Trans Surgery Fund has awarded more than $3,173,000 to 170 recipients, with published 2026-cycle awards from $16,450 to $83,800. Applications open November 1 and close November 30, and surgery must take place on or after March 1 of the following year, so the calendar matters.
Check hospital financial assistance before paying an inpatient bill. Nonprofit hospitals are required to have a written policy and many discount well above the federal poverty line. See hospital charity care and run the charity care check tool.
Pull the hospital’s own price file before you accept a quote. Every US hospital must post a machine-readable file of its gross charges, cash prices and negotiated rates. Our guide to hospital price transparency walks through finding and reading it.
Negotiate anything that lands as a surprise balance. Our guide to negotiating medical bills has the phone scripts, and medical bills in collections covers what to do if a stage’s bill has already gone past due while you were recovering from the next one.
The Bottom Line
Budget for the course, not the operation. A realistic all-in figure for a full multi-stage phalloplasty in the United States is $65,000 to $150,000, and the per-person claims average of $137,893 is the closest thing to a national number that exists. If cost is the deciding constraint, metoidioplasty is the procedure with a real cash market attached to it, at roughly $11,960 to $42,000 depending on what is bundled.
