Methotrexate is the odd drug out in the autoimmune cabinet. The biologics people take alongside it or instead of it are priced in the thousands of dollars a month. Methotrexate, the drug rheumatologists still reach for first in rheumatoid arthritis and the one dermatologists use for severe psoriasis, is priced in single dollars.
Two things make the monthly number so small. The tablet itself costs a pharmacy about sixteen cents in federal acquisition data, and the low-dose regimen is taken once a week rather than once a day, so a month is a dozen to four dozen tablets rather than thirty. The place cash payers get hurt is the delivery device: the same molecule in a pre-filled auto-injector runs two orders of magnitude more than the vial. Every figure below was researched on September 3, 2026.
What Does Methotrexate Cost Without Insurance?
Acquisition costs come from the CMS National Average Drug Acquisition Cost survey, with tablet and vial rows effective August 19 and August 26, 2026 and the Otrexup row effective August 20, 2025 (CMS has published no fresher one). Cash prices come from Cost Plus Drugs, Drugs.com, SingleCare and Labcorp OnDemand, all read September 3, 2026. Monthly figures assume one dose a week and use 4.33 weeks per month.
| Option | Low | Average | High |
|---|---|---|---|
| CMS NADAC acquisition cost, one 2.5 mg tablet | $0.15658 | $0.15658 | $0.15658 |
| Weekly tablet regimen, one month of drug at NADAC | $2.04 (7.5 mg/wk) | $4.07 (15 mg/wk) | $6.73 (25 mg/wk) |
| Cost Plus Drugs, 30 tablets (plus $5.25 shipping) | $7.69 | $7.69 | $7.69 |
| Discount card at retail, 24 tablets | $13.60 | $13.60 | $13.60 |
| Drugs.com cash price, 2.5 mg tablets | $15.62 (20) | $21.88 (36) | $46.90 (100) |
| Stated average retail cash price | $103.99 | $103.99 | $103.99 |
| Generic vial, 25 mg/mL preservative-free | $7.15 (2 mL) | $12.38 (10 mL) | $24.31 (20 mL) |
| Rasuvo auto-injectors, 4 (one month) | $575.66 | $590.07 | $590.07 |
| Otrexup auto-injectors, 4 (one month) | $761.29 | $783.50 | $783.50 |
| Lab monitoring, CBC plus metabolic panel | $78 | $78 | $78 |
Do the tablet arithmetic yourself, because it is the whole story. At $0.15658 a tablet, three tablets a week is 47 cents of drug, and ten tablets a week is $1.57. Over a month that is $2.04 to $6.73 of actual medicine. Everything you pay above that at a counter is dispensing fee, labor and markup.
Sources disagree sharply about what an unhelped retail fill costs. SingleCare states an average retail price of about $103.99 for methotrexate, while Cost Plus Drugs publishes a $86.40 retail comparison for 30 of the 2.5 mg tablets. The honest cross-source range for walking in with no discount is roughly $86 to $104, against $8 to $16 for anyone using a mail pharmacy or a free card. Per-pharmacy quotes on discount-card sites also move between visits, so treat any single chain’s number as an illustration of the spread rather than a price you can count on.
What Drives the Price
The delivery device, by a factor of about 50. Four Rasuvo pens cost $590.07 on Drugs.com and four Otrexup pens cost $783.50, both a one-month supply at one injection a week. CMS acquisition data agrees: a Rasuvo pen is about $143.91 and an Otrexup pen about $190.32. The identical weekly dose drawn from a generic vial costs $7.15 for a 2 mL vial and $24.31 for 20 mL. Nobody is paying for more methotrexate. They are paying for a spring, a needle guard and a fixed dose.
Brand versus generic within the injectables. Generic methotrexate injection is priced per milliliter in the low single digits in NADAC ($1.24 to $3.13 for the 50 mg/2 mL vials, 70 cents for the 250 mg/10 mL vial). The branded auto-injectors are priced per milliliter in the hundreds. Same drug, different package, different market.
Whether the product is still actually marketed. FDA’s Drugs@FDA database lists every strength of Otrexup, NDA 204824, as Discontinued, and otrexup.com does not serve a working page as of September 2026. Price guides have not caught up, and CMS’s most recent Otrexup acquisition cost is a year old. If you are quoted an Otrexup price, ask the pharmacy whether it can actually source it before you plan around the number.
Quantity and days supply. Fixed costs dominate a cheap generic. Drugs.com prices 20 tablets at $15.62 (78 cents each) and 100 tablets at $46.90 (47 cents each). At Cost Plus, the $5.00 pharmacy labor fee and $5.25 shipping are charged per order regardless of how many tablets are in it, which is why a 90-day script is meaningfully cheaper per month than a 30-day one.
The oral solution. A liquid methotrexate product exists for people who cannot swallow tablets, and it is priced like a specialty drug rather than a generic: CMS puts it at $21.76 per milliliter in August 2026 data. If tablets work for you, the tablets are the cheap route by a wide margin.
The Lab Monitoring Nobody Budgets For
Methotrexate is monitored with routine blood work, typically a complete blood count and a liver and kidney panel. How often is a decision for your prescriber, not for a cost guide, but the bill is real and recurring, and it is usually larger than the drug bill.
As of September 2026, Labcorp OnDemand lists a complete blood count at $29 and a comprehensive metabolic panel at $49, and Quest’s consumer site lists the same two tests at the same two prices. That is about $78 a round if you buy them yourself and bring the results to your appointment. Four rounds a year is roughly $312, on a medication that may cost you $40 a year.
Buying your own labs is usually cheaper than being billed by a hospital outpatient lab, and our blood work cost guide walks through the difference and the draw-site logistics. If a hospital lab has already billed you at a markup, our guide to negotiating medical bills covers what to say.
Cash Price vs. Insurance
Generic methotrexate tablets are one of the clearest cases on this site for skipping the pharmacy benefit entirely. If your copay is $10 or $15, a $7.69 mail order beats it outright, and you avoid a prior authorization conversation on a drug nobody wants to argue about.
The standard tradeoff still applies. Money you spend outside a plan does not count toward a deductible or an out-of-pocket maximum, and you cannot stack a discount card with insurance on the same fill. On a drug costing less than a coffee per week, that lost deductible credit is not worth chasing. On the biologics that often sit next to methotrexate in the same treatment plan the math flips completely, which is why our Humira cost guide, Skyrizi cost guide and Rinvoq cost guide spend most of their length on manufacturer assistance rather than on cash prices.
The auto-injectors are where insurance status actually decides the outcome. Manufacturer co-pay assistance reduces an insurance copay, which means it does very little for someone paying the full $590 or $784 in cash. If cost is the deciding factor and your prescriber is willing, the vial-and-syringe route delivers the same weekly dose for the price of a sandwich.
If a pharmacy quotes you three figures for generic methotrexate tablets, run it through our fair price checker before you pay. That quote reflects a pharmacy contract, not the market.
How to Pay Less
-
Price a transparent mail pharmacy first. Cost Plus Drugs charges $7.69 for 30 of the 2.5 mg tablets, and publishes the breakdown: $2.34 of manufacturing cost, $0.35 of 15% markup and $5.00 of pharmacy labor, plus $5.25 shipping per order. At a 7.5 mg weekly dose, 30 tablets is roughly ten weeks of medicine.
-
Run a free discount card if you fill locally. SingleCare’s methotrexate page, prices updated September 3, 2026, showed a $13.60 card price for 24 of the 2.5 mg tablets against a stated $103.99 average retail. Our comparison of GoodRx vs. SingleCare explains how to check both on the same prescription and why an online quote can move between visits.
-
Ask about the vial before you accept a pen. This is the single largest saving available on this drug: roughly $580 to $775 a month between an auto-injector and a generic vial for the same weekly dose. Whether injectable methotrexate is right for you at all is a clinical question, but if you and your prescriber have already chosen the injectable route, the packaging is a cost question.
-
Ask for a 90-day prescription. Every route here charges its fixed costs per fill. Drugs.com’s per-tablet price falls from 78 cents at 20 tablets to 47 cents at 100, and Cost Plus charges its labor fee and shipping once per order.
-
Do not assume a $4 generic program covers it. Walmart’s published Prescription Program list, effective March 24, 2025, contains no methotrexate entry at any strength. Check the current list at the counter before transferring anything.
-
Budget for the visits and the labs, not the pills. A free or sliding-scale clinic can often handle the prescribing visit and refills at little or no cost, and buying your own CBC and metabolic panel is usually cheaper than a hospital outpatient draw.
Bottom Line
Budget $3 to $25 a month for the weekly methotrexate tablet regimen without insurance, and treat anything above about $30 as a signal to shop rather than a fact about the drug. Add roughly $78 every time your prescriber orders monitoring blood work, which is the part of this treatment that actually costs money over a year.
The expensive version of this medicine is the auto-injector, at $590 to $784 for four weekly doses, against $7 to $24 for the same doses drawn from a generic vial. For the drugs commonly prescribed alongside or after methotrexate, see our Otezla cost guide, and for the other cheap generic that anchors a different inflammatory condition, our colchicine and allopurinol cost guide tells a similar story about how much of a pharmacy price is not medicine.
