Pregabalin now outsells its own brand name by a factor of a hundred. Medicare Part D covered 7.5 million generic pregabalin claims in 2024 against 65,724 for brand Lyrica, and the price gap explains why. The generic capsule costs a pharmacy four to six cents. The brand capsule costs about $9.68.
This guide prices both in September 2026: generic pregabalin capsules, the oral solution, extended-release tablets and Lyrica CR, and brand Lyrica itself. It also covers something most drug price guides skip, which is that pregabalin is a controlled substance, and that fact closes off several of the cash-saving routes that work on ordinary generics. Acquisition figures come from the CMS National Average Drug Acquisition Cost file with an August 19, 2026 effective date.

Low figures on the generic rows are Amazon Pharmacy Prime prices and the SingleCare card price, both read September 4, 2026. The high column on those rows is what the same pharmacy or the same market charges with no membership and no card: Amazon’s non-Prime price and SingleCare’s stated average retail. Brand rows come from the Drugs.com Lyrica and Lyrica CR price guides read the same day.
Two things stand out. The first is that the gap between the low and high column on a single row, at a single pharmacy, on the same day, is more than twenty to one. The second is that brand Lyrica CR, the once-daily extended-release version, is $517.48 for 30 tablets while generic pregabalin extended-release tablets cost a pharmacy $1.94884 each for the 165 mg strength and $2.11929 for the 330 mg.
What Drives the Price
Brand versus generic, and it is not close. Every strength of brand Lyrica costs a pharmacy essentially the same amount, between $9.67403 and $9.69847 per capsule in the August 2026 NADAC file, because brand pricing is per pill rather than per milligram. Generic pregabalin ranges from $0.04057 for a 25 mg capsule to $0.06438 for a 300 mg capsule. The molecule got cheap in 2019. The brand did not.
Membership and card status, more than pharmacy choice. Amazon Pharmacy listed the identical 30-capsule fill at $9.70 with Prime and $213.60 without on September 4, 2026. That is not two pharmacies competing, it is one seller running two prices. Discount cards produce the same effect: SingleCare’s card price for 60 capsules was $11.45 against an average retail of $428.78. Note that per-chain card prices shift between sessions and are not a rate you can hold anyone to, so treat any specific chain quote as an estimate.
Formulation. Extended-release pregabalin is genuinely more expensive than the plain capsule, and unusually the lowest strength is the priciest per tablet: generic pregabalin ER costs a pharmacy $3.64391 for an 82.5 mg tablet, $1.94884 for 165 mg and $2.11929 for 330 mg. Brand Lyrica CR is not in the federal acquisition file at all, while its generic is, which is the reverse of the usual pattern and means there is no independent acquisition benchmark for the brand version.
The liquid. Generic pregabalin oral solution costs a pharmacy $0.15466 per mL, or about $73 for a 473 mL bottle. Brand Lyrica solution was listed at $1,194.11 for the same bottle, about $2.52 per mL. Liquids are where brand-generic gaps get widest, because they are prescribed mostly to children and to people who cannot swallow capsules, and neither group shops around.
Controlled-substance status. The DEA’s alphabetical controlled substances list records pregabalin as CSA Schedule V, non-narcotic, sold as Lyrica. That single classification is why several standard cash tactics do not apply here, covered below.
Cash Price vs. Insurance
For generic pregabalin, cash almost always wins. A month runs $9.70 to $28 through the routes above, which is at or below a typical generic copay before you count a deductible. Medicare Part D’s own average was $26.54 per generic pregabalin claim in 2024 against $943.88 per brand Lyrica claim.
The Schedule V status is what makes this different from an ordinary generic, and it is worth a paragraph. Controlled substances are dispensed under tighter rules than other drugs, and pharmacies and mail-order operations decide independently whether to carry them at all. The two cheapest cash channels for ordinary generics both fail here. Cost Plus Drugs, which lists most common generics at cost plus a fixed markup, had no pregabalin product page as of September 4, 2026. Walmart’s Prescription Program drug list, effective March 24, 2025, does not include pregabalin at any tier, while it does include levetiracetam, lamotrigine and topiramate. Ninety-day supplies and automatic mail-order refills are also harder to arrange, so the per-day savings that come from buying in bulk are less available on this drug than on most.
Brand Lyrica is the one case where insurance is worth using. At $897.55 for 90 capsules, no cash strategy in this guide gets close to a brand copay. The manufacturer’s own program does not help an uninsured buyer either: the LYRICA Savings Card page states that eligible commercially insured patients may pay as little as $4 per month, and enrollment requires confirming you are not covered by a federal healthcare program. It is a copay offset for insured patients, not a cash discount.

How to Pay Less
Fill the generic, and check that your prescription allows it. A prescription marked “dispense as written” for Lyrica will be filled as Lyrica at roughly 175 times the pharmacy’s cost for the same molecule. One conversation with your prescriber is worth more than every tactic below.
Price a discount card against the pharmacy’s own cash price on the exact quantity. Neither wins reliably. Run the strength and count you actually need through a card, then ask the pharmacy for its cash price on the same fill. Our comparison of the major cards is at /save/goodrx-vs-singlecare/, and the fair price checker will tell you whether a quote is inside a normal range.
Ask whether a membership pharmacy is cheaper than a coupon. On September 4, 2026 an Amazon Pharmacy Prime price of $9.70 for 30 capsules beat the card price for the equivalent quantity. That is a membership you may already have.
Ask about the largest fill your pharmacy will legally dispense. Because pregabalin is controlled, a 90-day fill is not always available, but where it is the per-day cost drops sharply: $28.09 for 90 capsules against $9.70 for 30 works out to about 31 cents a capsule instead of 32 cents, and it saves two trips to a counter that has to log every controlled-substance fill.
Ask your prescriber to price-check the alternatives before the prescription is written, not after. Gabapentin is the usual comparator and averaged $14.78 per Medicare Part D claim in 2024 against $26.54 for pregabalin; see /costs/gabapentin-cost/ for the full breakdown. This is a cost comparison only. The two drugs are not interchangeable and which one is appropriate is a clinical decision, but a prescriber who knows the price gap up front can factor it in.
Skip the manufacturer coupon if you are uninsured. It will not work, and time spent on it is time not spent on the routes that will.
The Bottom Line
Generic pregabalin is a cheap drug wearing an expensive drug’s reputation. The medicine costs a pharmacy pennies, a month costs a prepared cash payer $10 to $30, and an unprepared one can be quoted more than $400 for the same bottle at the same counter. The two things that move the number are whether your prescription says Lyrica or pregabalin, and whether you asked for a price before you handed over the slip.
The controlled-substance wrinkle is the part most guides miss. It does not make pregabalin expensive, but it removes the flat-price cash pharmacies that make drugs like levetiracetam and topiramate so cheap, so the discount card and the membership pharmacy carry more weight here than they normally would.
