Keppra and its generic, levetiracetam, are one of the widest brand-to-generic price gaps in American pharmacy. The same 500 mg tablet costs a pharmacy under eight cents in its generic form and nearly ten dollars with the brand name on the bottle. If you are paying cash, which version your prescription is written for matters more than which pharmacy you walk into.
This guide covers what both cost in September 2026: immediate-release tablets, extended-release tablets (Keppra XR and generic levetiracetam ER), the oral solution, the dissolvable Spritam tablets, and the intravenous form used in hospitals. All acquisition figures come from the CMS National Average Drug Acquisition Cost file with an August 19, 2026 effective date, the most recent vintage published as of September 2026.

Low figures come from SingleCare card pricing and Cost Plus Drugs prices read September 4, 2026. Average and high figures come from the Drugs.com levetiracetam and Keppra price guides read the same day, and the $158.66 high on the first row is SingleCare’s stated average retail price for that fill.
The headline is the first row. A month of the most common generic dose costs less than a restaurant lunch if you use a discount card or a mail-order pharmacy, and about $159 if you hand the pharmacy your prescription and pay whatever the register says. That spread is not a discount on medicine. It is a spread on dispensing.
What Drives the Price
Brand versus generic. This is the single biggest lever. In the August 19, 2026 NADAC file a pharmacy pays $9.91551 for one brand Keppra 500 mg tablet and $0.07391 for the generic. The 1,000 mg brand tablet is $19.87692 against $0.15657 generic. Medicare Part D data confirms the gap at the claim level: in 2024 the program paid an average of $1,902.53 per Keppra claim and $30.89 per generic levetiracetam claim.
Which formulation you were prescribed. Plain tablets are the cheap version. Generic extended-release costs a pharmacy $0.16880 per 500 mg tablet, more than double the immediate-release tablet. The dissolvable version sold as Spritam is in a different universe entirely, at $10.24120 per 500 mg tablet in NADAC, and Drugs.com listed six of the generic dissolvable 250 mg tablets at $102.30. If your bill is unexpectedly high, check what form the label actually says before you blame the pharmacy.
Package size on the oral solution. The liquid is priced by how it is packaged, not by how much drug is in it. A 473 mL bottle of the generic solution runs $13.21 at Cost Plus Drugs or $18.74 to $24.26 at retail, while the same drug supplied as 20 to 50 individual unit-dose cups runs $88.24 to $214.23. That is the same medicine in more plastic.
Strength, not just dose. Because levetiracetam is priced per tablet rather than per milligram, two people on the same daily dose can pay different amounts. A 1,000 mg tablet costs a pharmacy $0.15657 while two 500 mg tablets cost $0.14782. The difference is trivial here, but the principle matters on every generic: ask whether a higher strength taken once is cheaper than a lower strength taken twice.
Whether the drug is given in a hospital. The IV form is not sold at a pharmacy counter. Medicare’s payment limit for HCPCS J1953 is $0.032 per 10 mg for the July 1 to September 30, 2026 quarter, which works out to roughly $1.60 for a 500 mg dose and $3.20 for 1,000 mg. When an emergency department bill arrives with a four-figure number on it, the levetiracetam is a rounding error inside it. The facility fee is the bill.
Cash Price vs. Insurance
For generic levetiracetam, paying cash usually beats using insurance, and it is not close. A typical commercial copay for a generic tier sits at $10 to $15 for a 30-day fill. Walmart’s Prescription Program, whose drug list was effective March 24, 2025 and which we read on September 4, 2026, covers levetiracetam 500 mg in its $9 tier: $9 for a 30-day supply of 60 tablets, $24 for a 90-day supply of 180. That is a flat cash price with no card, no coupon and no deductible attached to it.
Discount cards go lower still. SingleCare’s published card price for 60 of the 500 mg tablets was $6.46 on September 4, 2026 against a stated average retail of $158.66. Note that discount-card prices at individual pharmacy chains move constantly and are not a published rate you can hold anyone to, so treat a specific chain quote as an estimate and confirm at the counter. Our comparison of the major cards is at /save/goodrx-vs-singlecare/.
Brand Keppra is the exception. At $124.77 to $328.73 for 30 tablets, a brand copay through insurance will almost always beat cash. If your prescriber wants the brand specifically, that is the one scenario in this guide where running it through a plan is worth the paperwork.
One caution on cash purchases: money you spend outside your insurance generally does not count toward your deductible or out-of-pocket maximum. On a drug this cheap that rarely matters, but it matters a great deal if you are also managing a hospital bill in the same year.
How to Pay Less
Ask for the generic in writing. Most states allow automatic substitution, but a prescription marked “dispense as written” for Keppra will be filled as Keppra. One line on the prescription is worth more than every coupon in this guide combined.
Use a mail-order cash pharmacy. Cost Plus Drugs listed generic levetiracetam at $5.97 for 30 of the 250 mg tablets, $6.59 for 30 of the 500 mg, $7.21 for 30 of the 750 mg and $9.21 for 30 of the 1,000 mg on September 4, 2026, plus $5.00 shipping per order. Its own retail comparison for the 500 mg tablet was $1.48 each, or $44.40 for the same 30.
Check the flat-price programs before you check the coupons. Walmart’s $9 per 30 days and $24 per 90 days on levetiracetam 500 mg needs no enrollment and no card. Buying the 90-day quantity is the cheapest per-day route in this guide for anyone on a stable dose.
Compare a discount card against the cash price, every time. They are not the same and neither one always wins. Run the exact strength and quantity through a card, then ask the pharmacy what its own cash price is. If both look high, our fair price checker will tell you whether the quote is in a normal range.
Ask about the bottle, not the cups, on the liquid. If you or your child takes the oral solution, the 473 mL bottle is roughly a tenth of the cost of the equivalent volume in unit-dose cups. This is a packaging conversation with the pharmacist, not a clinical one.
If the barrier is the visit rather than the drug, start with a sliding-scale clinic. Anti-seizure medication requires an active prescriber, and a lapse in follow-up appointments is a common reason people go without. Federally qualified health centers charge on a sliding scale based on income; see /save/free-clinics-sliding-scale/. If a hospital or neurology bill is already in collections, /save/how-to-negotiate-medical-bills/ covers what to do next.
The Bottom Line
Generic levetiracetam is one of the cheapest chronic medications in the country, and almost every high price attached to it comes from something other than the medicine: the brand name, an extended-release or dissolvable formulation, unit-dose packaging, or a pharmacy’s own cash markup. A cash payer on ordinary 500 mg tablets should expect to pay somewhere between $6 and $17 for a month, and should treat any quote near $159 as a signal to move the prescription rather than to pay it.
If you are pricing other anti-seizure drugs, our guides to lamotrigine, topiramate and gabapentin use the same federal acquisition data, and pregabalin and Lyrica covers the drug most often priced against this one.
