Venlafaxine is a high-volume generic that still has an expensive brand sitting on top of it. CMS recorded 5.42 million generic venlafaxine ER prescriptions in Medicare Part D during 2024 at an average of $21.85 each, against 16,997 brand Effexor XR prescriptions at an average of $1,398.74 each.
The other thing that moves the price here is form. Venlafaxine comes as immediate-release tablets, extended-release capsules and extended-release tablets, and those three price very differently even at identical doses. Every figure below was pulled on September 4, 2026 from CMS acquisition and spending data, Walmart’s published program document, Cost Plus Drugs and the drugs.com price guide.
What Does Effexor XR Cost Without Insurance?
The acquisition column is CMS’s National Average Drug Acquisition Cost survey, effective August 19, 2026, which is roughly what a retail pharmacy pays before dispensing costs. The Cost Plus column is that pharmacy’s published all-in price for a 30-count fill, read September 4, 2026, before its $5.00 standard shipping. The retail column is the drugs.com cash price for the same quantity on the same day.

Walmart’s Retail Prescription Program document, still stamped effective March 24, 2025, adds a fourth published price: venlafaxine ER 37.5, 75 and 150 mg capsules at $15 for a 30-day supply and $38 for 90 days, and immediate-release venlafaxine 37.5, 75 and 100 mg tablets at the same prices for 60 and 180 tablets. The tablet quantities are doubled because immediate-release venlafaxine is dosed more than once a day, so a 30-tablet quote for the immediate-release form is roughly half a month.
That program is worth reading carefully. The venlafaxine rows sit in the $15 and $38 tier, not the $4 and $10 tier that gives the list its name, and duloxetine does not appear anywhere in the document at any tier.
What Drives the Price
Brand versus generic
The spread here is one of the widest on any common antidepressant. CMS’s 2024 Part D file puts brand Effexor XR at $18.84 per dosage unit against $0.3469 for generic venlafaxine ER, roughly 54 to 1. FDA labeling lists Effexor XR as approved for major depressive disorder, generalized anxiety disorder, social anxiety disorder and panic disorder in adults, and the generic carries the same approvals at a fraction of the price.
Capsule versus tablet
This is the trap. Generic Effexor XR is an extended-release capsule; extended-release venlafaxine tablets are a separate product line approved on their own applications. CMS acquisition data on the same day price the 75 mg ER capsule at $0.07939 and the 75 mg ER tablet at $0.28435, and the 37.5 mg ER tablet is the worst of the group at $0.46232, nearly six times the capsule of the same dose. If you are handed a price you did not expect, the form is the first thing to check.
Immediate release versus extended release
Immediate-release venlafaxine tablets are cheaper per tablet but are taken two or three times a day, so the per-month cost lands in the same neighborhood as the ER capsule. Walmart prices both at $15 for 30 days, one at 30 units and one at 60.
Where you fill it
Because a month of the drug itself costs a pharmacy about $2.40 at the common ER capsule strengths, virtually everything you pay is dispensing overhead. Cost Plus Drugs itemizes the 75 mg capsule fill as $0.87 of drug, $0.13 of markup and a flat $5.00 pharmacy labor fee, which is why every strength lands within a dollar of the others.
The one very expensive generic
Venlafaxine besylate ER 112.5 mg, a distinct salt sold as an authorized product, sits at $6.18067 per tablet in the same CMS file and $209.03 for thirty at drugs.com. It averaged $286.53 per prescription in CMS’s 2024 Part D file against $21.85 for ordinary venlafaxine ER. It is a generic in name only on price.
Pricing Pristiq (Desvenlafaxine) Against Effexor
Desvenlafaxine is the active metabolite of venlafaxine, sold as brand Pristiq by Wyeth/Pfizer, and it is often priced alongside Effexor XR because they land in similar treatment slots. It is a separate purchase decision with a very different price structure.
Brand Pristiq runs $13.93944 to $13.97300 per tablet in CMS acquisition data effective August 19, 2026, which is about $418 to $419 for thirty at every strength from 25 to 100 mg. CMS’s 2024 Part D file recorded 7,562 Pristiq prescriptions at an average of $801.06 each.
The generic is dramatically cheaper, and the salt form matters. Desvenlafaxine succinate ER, the version that maps to Pristiq, costs $0.39504 to $0.41164 per tablet in CMS data, about $12 for thirty, and Cost Plus Drugs sells it at $11.11 (100 mg), $13.21 (50 mg) and $16.66 (25 mg) for thirty. A separate desvenlafaxine ER base product sits nearly ten times higher at $3.80556 to $3.85110 per tablet, and CMS’s Part D file shows the same split: $63.39 per prescription for the succinate against $209.93 for the base.
So the practical ranking for a cash payer, using published prices on September 4, 2026, is generic venlafaxine ER capsules at about $6 to $15 a month, generic desvenlafaxine succinate at about $11 to $17, brand Pristiq around $418 and brand Effexor XR around $595 to $649.
Cash Price vs. Insurance
For the generics, cash usually wins. A common commercial generic copay of $10 to $15 is at or above the full published cash price for venlafaxine ER capsules, so running the claim can cost you more than not running it. On a high-deductible plan the case is even clearer, because you would be paying the plan’s negotiated rate in full anyway.
For the brands the logic flips. Nothing about paying cash makes a $595 monthly fill workable, so brand Effexor XR and brand Pristiq are drugs where coverage or a manufacturer program is the only realistic path.
Viatris publishes an Effexor XR Savings Card that states patients may pay as little as $4 for each 30-day fill of brand Effexor XR, with savings of up to $150 per fill and up to $1,800 a year, and its eligibility list explicitly includes people who are uninsured. Read the terms before you count on it: as of September 4, 2026 the page states that the offer and program expire on 12/31/2024, so confirm by phone that the card is still being honored. The card is also not valid for prescriptions reimbursed by Medicaid, Medicare, TRICARE or Veterans Affairs. Viatris runs a separate patient assistance program that supplies certain medicines free to patients with demonstrated financial need, but Effexor XR is not on its published medicine list.
Before you agree to any quote, check it against a benchmark. Our fair price checker is built for exactly that.
How to Pay Less
Ask whether the prescription can be filled as the extended-release capsule. The capsule is the cheapest form of extended-release venlafaxine at every source on this page, and the 37.5 mg ER tablet costs nearly six times as much per dose in CMS acquisition data. Which form is appropriate is a question for your prescriber, but the price difference is worth surfacing.
Compare Walmart’s program against a cash mail-order pharmacy. Walmart lists venlafaxine ER capsules at $15 for 30 days and $38 for 90 days with no membership. Cost Plus Drugs is lower at $6.00 to $6.90 for thirty but adds $5.00 shipping per order, so it wins clearly when you are filling more than one prescription at a time.
Fill 90 days at once. A flat dispensing fee dominates the total on a drug this cheap. Drugs.com prices ninety 75 mg ER capsules at $24.46 against $13.35 for thirty, so the second and third months cost about five dollars each.
Ask for the cash price before any card or plan is run. Pharmacies do not always volunteer it, and on generic venlafaxine the cash price is frequently the lowest number available.
Check a discount card against that cash price. Discount cards can undercut a counter price on generics but cannot be combined with insurance. Our comparison of GoodRx and SingleCare explains how they differ and where each usually lands.
If the appointment is the real expense, price that separately. Refills need a prescriber, and a visit costs far more than this drug. Free and sliding-scale clinics charge on income, and our guides to psychiatrist costs and therapy costs without insurance cover the self-pay ranges. If a bill has already grown past what you can pay, our medical bill negotiation guide walks through the conversation.
The Bottom Line
Generic venlafaxine is a cheap drug with an expensive brand and one expensive form. As of September 4, 2026 the realistic cash range for a 30-day supply of the extended-release capsule is about $6 to $15, from Cost Plus Drugs at the low end through Walmart’s $15 program tier to retail at $12.62 to $13.35.
The two ways people overpay are filling the brand, at roughly $595 to $649 for thirty capsules in CMS acquisition data, and filling the extended-release tablet instead of the capsule, which can run $23.79 to $72.27 for the same thirty doses at retail. Both are worth a two-minute conversation at the pharmacy counter.
