Duloxetine is one of the most-dispensed prescriptions in the United States, and it is also one of the cheapest. CMS recorded 13.09 million generic duloxetine prescriptions in Medicare Part D alone during 2024, covering 2.66 million beneficiaries, at an average of $23.58 per prescription. Brand Cymbalta accounted for 23,811 of those prescriptions at an average of $667.27 each.

That gap is the whole story of what you will pay. Every figure below was pulled on September 4, 2026 from CMS acquisition and spending data, FDA product records, Cost Plus Drugs’ published cash prices and the drugs.com price guide. This page covers what duloxetine costs and where to buy it, not whether it is right for you.

What Does Cymbalta Cost Without Insurance?

The acquisition column is CMS’s National Average Drug Acquisition Cost survey, effective August 19, 2026, which is roughly what a retail pharmacy pays to buy the drug before any dispensing cost. The Cost Plus column is that pharmacy’s published all-in price for a 30-count fill, read September 4, 2026, before its $5.00 standard shipping. The retail column is the drugs.com cash price range for the same quantity on the same day.

Two things stand out. The first is that a month of generic duloxetine costs a pharmacy less than four dollars at the common strengths, so almost everything you pay above that is dispensing overhead. Cost Plus Drugs itemizes this openly: the 60 mg fill breaks down as drug cost plus a 15% markup plus a flat $5.00 pharmacy labor fee, which is why every strength lands within about sixty cents of the others.

The second is the 40 mg outlier. It is roughly ten times the per-capsule acquisition cost of the 60 mg despite containing less drug, and all three sources agree on the direction and rough size of that gap.

Brand Cymbalta is now a legacy product. Drugs@FDA lists all three strengths under Eli Lilly’s NDA 021427 as Discontinued, and the FDA National Drug Code Directory shows a marketing end date of October 2, 2026 for all three Cymbalta product codes. The most recent CMS acquisition file that carries brand Cymbalta rows at all is dated July 23, 2025, more than a year behind every generic row on this page, which is stated here because it makes that one row less current than the others.

What Drives the Price

Whether the prescription says brand or generic

This is the single biggest lever, and it is larger here than on most drugs. In CMS’s 2024 Part D file, brand Cymbalta averaged $8.98 per capsule against $0.3506 for generic duloxetine, a spread of about 26 to 1. At retail the same shape holds: $8.28 per capsule for brand at drugs.com against $0.39 to $0.50 for the 60 mg generic.

The strength on the label

Generic pricing does not scale with dose. The 20, 30 and 60 mg capsules all sit within a few cents of each other because dozens of manufacturers make them, while 40 mg has very few and prices like a niche product. The higher 80, 90 and 120 mg capsules are a different story again: drugs.com prices thirty of any of them at $156.48, and none of those strengths appears in CMS acquisition data at all.

Where you fill it

The delivered price is mostly dispensing cost, so the pharmacy you choose matters more than the drug does. The published spread on the same 60 mg fill runs from $6.48 at a mail-order cash pharmacy to $15.02 at the high end of the retail range, and independent pharmacies outside that survey can be higher still.

Quantity per fill

Because a flat labor fee dominates the total, a 90-day fill is much cheaper per capsule than three 30-day fills. Drugs.com prices ninety 60 mg capsules at $19.79 to $29.46, against $11.75 to $15.02 for thirty, so the third month is close to free.

Why you are taking it

This one does not change the price, but it changes who is at the counter. FDA labeling approves duloxetine for major depressive disorder, generalized anxiety disorder in adults and children 7 and older, diabetic peripheral neuropathic pain, fibromyalgia in adults and children 13 and older, and chronic musculoskeletal pain. A large share of duloxetine buyers are pain patients rather than psychiatric patients, which is unusual for an antidepressant and is why searches for it cluster around both.

Cash Price vs. Insurance

For duloxetine, cash usually wins outright. A typical commercial generic copay of $10 to $15 is at or above the full cash price at a discount pharmacy, and once you have met a deductible-free copay there is nothing left to save. If you have a high-deductible plan, paying cash is almost always cheaper than running the claim, because you would be paying the full negotiated rate anyway.

The exception is brand Cymbalta. At $496.69 for sixty capsules there is no cash strategy that makes it affordable, and with FDA records showing the brand ending its marketing run on October 2, 2026, the practical answer is that most people filling a Cymbalta prescription are already receiving the generic.

One thing to watch: pharmacies do not always volunteer the cash price when you present a card or an insurance ID. Ask what the drug costs without running any claim at all, then compare. Our fair price checker is a quick way to sanity check a quote before you agree to it.

How to Pay Less

Ask for the cash price before anything is run through a plan. For a drug whose acquisition cost is under four dollars a month, the cash price is often the lowest number in the room, and pharmacists can quote it if you ask directly.

Compare a mail-order cash pharmacy against your local counter. Cost Plus Drugs publishes $5.90 to $6.48 for thirty capsules of the 20, 30 and 60 mg strengths plus $5.00 shipping, which is competitive with the low end of the retail range and well below the high end. Because shipping is per order rather than per drug, it works best when you are filling more than one prescription at a time.

Move to a 90-day fill. The flat dispensing fee is the largest single component of a duloxetine fill, so tripling the quantity roughly halves the per-capsule cost at both cash and retail pharmacies.

Ask about the strength if you were written for 40 mg. It is the one generic duloxetine strength that prices like a brand, at $23.77 for thirty at Cost Plus and $70.95 at retail. Whether a different strength is appropriate is a clinical question for your prescriber, but the cost difference is worth raising.

Use a discount card, but check it against the cash price. Pharmacy discount cards can beat a counter price on generics, though they are not usable alongside insurance. Our comparison of GoodRx and SingleCare explains how the two work and where each tends to land.

If the prescription is the smaller problem, look at the visit. Refills require a prescriber, and an office visit costs far more than the drug. Federally qualified health centers and free and sliding-scale clinics charge on income, and our guide to what a psychiatrist costs covers the self-pay range for that appointment. If a past bill has already gone to collections, this guide covers your options.

The Bottom Line

Generic duloxetine is a cheap drug that is easy to overpay for. The realistic cash range as of September 4, 2026 is $5.90 to $15.02 for a 30-day supply at the common strengths, with Cost Plus Drugs at the low end and ordinary retail at the high end, and the number moves more with which pharmacy you use than with which strength you take.

Brand Cymbalta at $496.69 for sixty capsules belongs to a product that FDA records show is ending its marketing run in October 2026. If your prescription still says Cymbalta, asking for the generic is the single highest-value thing you can do.
