CVS is the largest retail pharmacy in the country. Its FY2025 annual report says the Pharmacy and Consumer Wellness segment filled 1.81 billion prescriptions on a 30-day equivalent basis across roughly 9,000 locations, about 28.5% of every retail prescription dispensed in the United States. Most of those were paid by an insurer. If you are paying cash, you are in the minority, and you are quoted a different number than everyone in line ahead of you.
That number is unusually hard to look up. CVS does not publish a cash price list, and its own savings page points you to a pharmacist rather than a price. So this guide anchors on something CVS cannot edit: Florida’s Agency for Health Care Administration publishes a monthly file of retail prices for the 400 most-dispensed drugs, pharmacy by pharmacy and store by store. The figures below come from that file, effective August 19, 2026, cross-checked in three separate Florida counties, and sit next to the federal NADAC benchmark for what the medicine itself costs.
What Does a Prescription Cost at CVS Without Insurance?
| Generic drug | Quantity priced | CVS cash price | Drug cost at NADAC, same quantity | CVS price as a multiple |
|---|---|---|---|---|
| Losartan potassium 50 mg | 30 tablets | $146.99 | $0.97 | 151x |
| Atorvastatin 20 mg | 90 tablets | $172.99 | $2.49 | 70x |
| Lisinopril 10 mg | 30 tablets | $36.89 | $0.55 | 67x |
| Gabapentin 300 mg capsules | 90 capsules | $80.99 | $2.71 | 30x |
| Metformin 500 mg | 180 tablets | $57.59 | $2.55 | 23x |
| Amoxicillin 500 mg capsules | 21 capsules | $15.89 | $1.64 | 10x |
| Levothyroxine 50 mcg | 90 tablets | $35.19 | $3.66 | 10x |
CVS cash prices are from the MyFloridaRx file effective August 19, 2026. Acquisition costs are CMS National Average Drug Acquisition Cost per unit, also effective August 19, 2026, multiplied by the quantity in the row.
The most useful thing in that table is not any single price. It is the consistency. Across 76 CVS stores in Miami-Dade County, 33 in Orange County and every store checked in Hillsborough County, atorvastatin 20 mg came back at exactly $172.99 for 90 tablets. Losartan came back at exactly $146.99 in all 92 CVS stores checked. CVS sets one chain-wide cash price per drug, at least across every store in this file, and does not vary it by neighborhood or metro. Shopping CVS against CVS is pointless. Shopping CVS against another chain is not.
In the same file and the same counties, Walmart charged $6.28 for the identical 21-capsule amoxicillin course that CVS priced at $15.89, and the cheapest Walgreens charged $27.09 for the 30 lisinopril tablets CVS priced at $36.89. Normalized to a 30-day supply, CVS cash prices in this sample ranged from $11.73 (levothyroxine) to $146.99 (losartan).
What Drives the Price
The medicine is almost never the cost
At the federal acquisition benchmark, a lisinopril tablet costs 1.8 cents and an atorvastatin 20 mg tablet costs 2.8 cents. A month of losartan is 97 cents of actual drug. Everything above that is CVS’s markup, dispensing overhead and store economics. When you see a 151x multiple, you are not looking at an expensive medicine, you are looking at a list price nobody with insurance ever pays.
CVS’s own average tells you who the price is for
CVS’s FY2025 filing reports $115.5 billion of pharmacy revenue against 1.81 billion 30-day-equivalent prescriptions, which works out to roughly $64 of revenue per script including brand and specialty drugs. CVS separately advertises that 85% of CVS prescriptions cost less than $10 a month, and its own footnote defines that as consumer out-of-pocket cost excluding the cost of prescription insurance. Both figures describe a mostly-insured population. Neither is a promise to a cash payer.
CostVantage changed what insurers pay, not what you pay
CVS Health announced on January 6, 2025 that every commercial prescription dispensed at CVS Pharmacy is now contracted through CVS CostVantage, a reimbursement formula built on drug acquisition cost, a set markup and a service fee. It is a genuine shift toward cost-plus pricing, and CVS says it intends to extend it to Medicare and Medicaid managed prescriptions. But it is a contract between CVS and pharmacy benefit managers. No CVS document extends the formula to an uninsured customer paying at the register.
Quantity and format move the number more than location does
Because CVS prices per fill rather than per day, the 90-day quantity is usually the better unit. Atorvastatin at $172.99 for 90 tablets is $57.66 a month, well below the $146.99 CVS charges for a single month of losartan. Ask what the 90-day price is before you accept the 30-day one, and ask whether a different strength taken as a half tablet lands in a cheaper bracket.
Cash Price vs. Insurance
CVS’s posted cash price is a usual-and-customary figure, and the company is currently defending a set of lawsuits alleging that its retail pharmacies overcharged by not submitting the correct usual and customary price during claims adjudication, including state false-claims cases unsealed in May 2025. Those cases are disclosed in CVS’s own 10-K and are unresolved. The practical lesson for you is simpler: treat the shelf price as an opening number, not a fixed one.
If you have insurance with a high deductible, compare the copay to the discount-card price before the pharmacist runs your card. You cannot combine the two. Running a prescription through insurance means the payment counts toward your deductible; running it on a discount card means it does not, but it may cost a fraction as much. On a $146.99 losartan fill against a plan that has not met its deductible, the card almost always wins. Our fair price checker will tell you whether a quoted number is in a normal range before you decide.
One CVS-specific quirk: GoodRx publishes a separate price for CVS Pharmacy and Target (CVS) stores from the price it shows for all other participating pharmacies, and the Gold membership card has a distinct CVS view. If you show the all-other-pharmacies version at a CVS counter, the number you were expecting will not be the number you get.
How to Pay Less at CVS
Price the same drug at three chains before you fill. This is the single highest-value move, and the Florida data shows why: the same amoxicillin course was $6.28 at Walmart and $15.89 at CVS on the same day. See Walmart pharmacy price guide for the four-tier program list and Costco pharmacy prices for the mail-order side of the same comparison.
Bring a discount card and say so before the prescription is rung up. CVS accepts third-party cards, and on the drugs in the table above the gap between the posted cash price and a card price is usually most of the bill. Our comparison at GoodRx vs. SingleCare prices two of the big free cards head to head, and what GoodRx itself costs covers what the paid GoodRx Gold tier costs and whether it is worth it.
Check TrumpRx.gov before you fill a brand-name drug. TrumpRx is a federal self-pay pricing program that launched February 5, 2026 and expanded to generics on May 18, 2026. CVS states it accepts eligible TrumpRx discount cards at all of its roughly 9,000 locations and that the program now covers 600-plus generic and select brand-name medications. CVS also notes that TrumpRx pricing cannot be combined with insurance and does not count toward a deductible.
Compare against mail-order cash pharmacies for maintenance drugs. If you take a chronic medication and can wait for shipping, Cost Plus Drugs prices and Amazon Pharmacy costs frequently beat every retail counter. That matters most on the drugs with the ugliest multiples above, like atorvastatin and gabapentin.
Opt in to ExtraCare pharmacy rewards, but do not confuse it with a discount. Joining ExtraCare is free, and opting in to rewards at the pharmacy earns credits for filling prescriptions, getting vaccinations and adding family members, with $2 in ExtraBucks issued for every 4 credits, capped at $50 a calendar year. Those are store credits for front-of-store merchandise, not a reduction in your prescription price, and prescriptions are explicitly excluded from ExtraCare’s 2% back. Pharmacy rewards are also unavailable in Arkansas, New Jersey and New York. ExtraCare Plus, the paid tier, is $5 a month or $48 a year and its benefits are shipping, 20% off CVS-brand products and a $10 monthly bonus reward, none of which touch drug pricing.
Ask about therapeutic alternatives and a 90-day fill. These are the two savings routes CVS itself promotes on its pharmacy page, and both are free to ask about. If cost is the reason you would skip a medication, say that out loud at the counter. If none of it gets the number low enough, free and sliding-scale clinics covers clinics that dispense at sliding-scale or 340B pricing.
The Bottom Line
CVS is convenient, near-universal and, at the cash register, expensive. It runs no flat-price generic list, its cash prices are set chain-wide and do not vary store to store, and on common generics they land 10 to 150 times what the drug costs to acquire. The two things that actually change your bill are a discount card, which CVS accepts, and a competing pharmacy, which is often half the price for the identical bottle.
The prices in this guide are a snapshot of Florida’s August 19, 2026 file. CVS can change a chain-wide cash price at any time and does not announce it, so treat these as the shape of the market rather than a quote. Ask the pharmacist for the cash price, the card price and the 90-day price, in that order, before you hand over a payment method.
