Buccal fat removal, sometimes called buccal lipectomy or bichectomy, takes out the buccal fat pad that sits in the hollow of the cheek. The incision is made inside the mouth, so there is no visible scar, and the whole thing takes about thirty minutes. Goals Aesthetics, which publishes prices across a dozen U.S. clinics, describes it as usually performed under local anesthesia with or without sedation in an accredited facility.

That short, low-tech profile is why it sits near the bottom of the facial surgery price list. It is also why the practice-to-practice spread is so wide: when a procedure is this quick, the price is driven almost entirely by choices about anesthesia, facility, and whether you are buying it on its own or bolting it onto something bigger. This guide walks through what those choices cost as of August 2026.

What Does Buccal Fat Removal Cost Without Insurance?

Across the published prices below, budget $3,000 to $6,500 all-in at a typical U.S. practice, with the top of the market running considerably higher.

The most authoritative national number comes from the American Society of Plastic Surgeons. Its buccal fat removal cost page, re-checked on September 2 2026, puts the national average surgeon fee at $3,142. That is a single national average rather than a range, and it is a surgeon fee only. ASPS states plainly that the figure does not include anesthesia, operating room facilities, or other related expenses, which is why nearly every published practice price below sits above it.

Sources disagree at the top end, and the disagreement is real rather than a rounding issue. CareCredit puts the general range at $2,000 to $5,000 but notes cases as high as $20,000. Published practice prices cluster much tighter: $3,800 to $6,300 in Dallas, $4,200 in Richmond as a standalone procedure, $4,995 to $5,995 in Seattle, and $5,000 as a starting price across Goals Aesthetics’ twelve clinics. Treat $3,000 to $6,500 as the working range and anything above $10,000 as a premium-brand or bundled quote rather than the market rate.

One structural fact explains a lot of the noise. Buccal fat removal is small enough that ASPS does not break it out as its own line anywhere in the 2025 Plastic Surgery Statistics Report, which does count 118,978 eyelid surgeries and 77,933 facelifts for 2025. A procedure too small to be tracked nationally is a procedure with few practices competing openly on price, and price discovery is correspondingly poor.

What Drives the Price

Anesthesia, which is the cheapest lever you control

Seattle Plastic Surgery’s published list is the clearest example on the internet: buccal fat pad removal is $4,995 under local anesthesia and $5,995 under general, same surgeon, same operation. That is a flat $1,000 for the anesthesia tier. Niamtu Cosmetic Facial Surgery prices anesthesia separately at $400 per hour, so the arithmetic works out similarly.

Not everyone is a candidate for the lighter option, and that is a conversation for the surgeon rather than a price sheet. But it is worth asking for both quotes.

Where the operation happens

CareCredit notes the procedure can be done in a hospital, a licensed ambulatory surgery center, or the surgeon’s own office. That choice carries a real line item: Niamtu’s published surgery center fee runs $500 to $2,500. An in-office procedure under local anesthesia skips most of it, which is a large part of why the cheapest published quotes are the office-based ones.

Whether it is a sole procedure or an add-on

This is the biggest single discount available. Niamtu lists buccal fat pad reduction at $4,200 specifically “as sole procedure,” and states that some fees may be discounted when performed with a primary procedure. Dr. Cocco’s Dallas price list carries the same kind of note about reduced fees for combined surgery.

The savings compound. You pay the facility fee once, the anesthesia is already running, and CareCredit notes that patients having a facelift at the same time may not need a separate incision inside the mouth. Against a deep plane face and neck lift at $25,000 in Richmond or a facelift and neck lift at $18,500 to $20,500 in Dallas, the marginal cost of adding cheek reduction is small.

Surgeon and market, not complexity

Because the operation itself barely varies, the price mostly tracks the practice’s positioning. ASPS names the surgeon’s experience, the type of procedure, and geographic office location as the drivers of its fee, and the published lists bear that out: the same thirty-minute operation is $4,200 in suburban Richmond and starts at $5,000 at a national chain with Manhattan and Los Angeles locations.

The permanence factor

Buccal fat does not grow back, so unlike injectables there is no recurring cost. The flip side is that reversing an over-thinned cheek means paying for facial fat grafting, a second operation with its own surgeon, anesthesia, and facility bills, and ASPS publishes no national average cost for it. That is a real financial consideration, not a scare tactic: the procedure has no repeat cost, but it also has no free undo.

Cash Price vs. Insurance

There is no coverage angle. Aetna’s clinical policy bulletin on cosmetic surgery states that its plans exclude coverage of cosmetic surgery and procedures that are not medically necessary, and lists adjacent facial contouring work including cheek implants and deoxycholic acid injection (Kybella) among the procedures it treats as cosmetic. Other major payers take the same position, and a purely cosmetic cheek reduction is not an eligible HSA or FSA expense either.

That is not all bad news for a self-pay shopper. There is no chargemaster, no network rate, and no explanation of benefits landing six weeks later. Everything is a cash price quoted up front by a business that wants your deposit, which makes it negotiable in a way a hospital bill almost never is.

The risk sits on the financing side. A cosmetic procedure put on a medical credit card is ordinary consumer debt, and none of the protections that attach to hospital billing apply if you fall behind. Our guide to medical bills in collections covers where those lines fall.

How to Pay Less

Get one all-in written number. Ask each practice for a single total naming the surgeon fee, anesthesia, facility fee, pre-op labs, prescriptions, and follow-up visits. A $3,142 surgeon fee and a $4,995 all-in price can be the same deal. Run both through our fair price checker before deciding.

Ask what the local anesthesia version costs. Seattle’s published $1,000 gap between its two anesthesia tiers is the single easiest saving on this procedure, and most practices will not volunteer the cheaper option unless you ask.

Ask where it will be done. In-office under local avoids a surgery center fee that runs $500 to $2,500 on Niamtu’s published list. If a practice quotes an ambulatory surgery center, ask whether an office-based version exists.

Bundle it if you were already planning facial surgery. The add-on discount is published policy at multiple practices, and you pay the facility and anesthesia once. If a lift is already on your list, our face lift cost guide and neck lift cost guide cover how those quotes are structured.

Price the non-surgical comparisons honestly. Neurotoxin for a wide jaw, filler for jawline definition, and submental liposuction get compared to buccal fat removal constantly, and none of them do the same thing. On price, Goals starts Botox at $800 and fillers at $1,200 per treatment, while Niamtu charges $14 per Botox unit and $600 per filler syringe. Our Botox cost and dermal filler cost guides have the per-visit math, and the key difference is that injectables recur indefinitely. If under-chin fat is the actual concern, our liposuction cost guide covers submental work.

Negotiate, and decline the deferred-interest card. Practices that push medical credit cards have margin to give up when you offer cash instead. Our guide to negotiating medical bills has the language and our negotiation script has the call structure.

The Bottom Line

Budget $3,000 to $6,500 all-in for buccal fat removal as of August 2026. Under $3,000 you are probably looking at a surgeon fee with anesthesia and facility charges still to come. Above $8,000, ask what else is in the number, because the operation itself is thirty minutes long.

The two questions worth asking every practice are what the local anesthesia version costs and what the price drops to if you combine it with something else. Between them, those two answers account for most of the spread on this procedure.
