Baclofen is a high-volume generic that most people will fill for under $15 and a specialty product that can cost thousands, and the two live under the same name. Medicare Part D covered 5.2 million baclofen claims for 1.4 million people in 2024 at an average of $17.06 per claim. A 10 mg tablet reaches the pharmacy at about three cents.

Almost everything expensive about baclofen is a delivery-method question rather than a drug question. Liquids, granules and the implanted pump exist for people who cannot swallow a tablet or cannot tolerate the oral dose, and each of those routes multiplies the price. This guide keeps its center of gravity on the tablet and covers the rest so you know what you are looking at. Every figure below was researched on September 4, 2026.

What Does Baclofen Cost Without Insurance?

Acquisition costs come from the CMS National Average Drug Acquisition Cost survey, effective August 19, 2026. Cash prices come from Drugs.com, SingleCare and Cost Plus Drugs, all read September 4, 2026. Medicare figures are the 2024 data year, released in mid-2026. The middle column shows a representative example, not a computed average.

The tablet numbers cover a wider band than most generics on this site, and the sources genuinely disagree. SingleCare’s card price of $4.80 and Cost Plus Drugs’ $5.52 sit at one end, Drugs.com’s $11.39 to $27.47 range sits in the middle, and SingleCare’s own stated average retail price of $68.35 sits far above all of it. Federal claims data at $17.06 per claim suggests most people land somewhere in the low teens.

Cost Plus Drugs publishes its arithmetic, which is useful for calibration: for 30 of the 10 mg tablets it lists $0.45 of manufacturing cost, $0.07 of markup and $5.00 of pharmacy labor, plus $5.25 of standard shipping added at checkout. That means the drug itself is about 45 cents and everything else is service. A $68 counter price is not a drug cost.

What Drives the Price

Strength, and specifically the 15 mg tablet. The CMS acquisition survey prices a 15 mg tablet at $1.67990 against $0.03148 for the 10 mg and $0.05088 for the 20 mg. Only two labeler codes appear against that strength in the file, and the price has stayed above $1.60 across recent vintages, so this is a supply problem rather than a data glitch. The 5 mg is also mildly counterintuitive at $0.06844, roughly twice the cost of the 10 mg tablet.

Tablet versus liquid. Generic baclofen oral solution 5 mg/5 mL costs a pharmacy $1.10299 per mL and the 25 mg/5 mL suspension costs $3.80162 per mL. Drugs.com listed the generic 5 mg/5 mL liquid at $536.19 for a 473 mL bottle. Against roughly 94 cents of acquisition cost for 30 generic 10 mg tablets, that is not a small step up, and it exists because some patients cannot swallow pills rather than because the liquid works better.

Brands, two of which are being wound down. Brand Fleqsuvy suspension costs a pharmacy $5.98153 per mL, about $718 for a 120 mL bottle, against $3.80162 per mL for the generic suspension. Amneal announced that as of June 30, 2025 it discontinued promotion and distribution of Lyvispah granules and that the product will remain available only while existing stock lasts. FDA published on August 10, 2026 that Ozobax oral solution 5 mg/5 mL sits on the Orange Book’s Discontinued Drug Product List, after Metacel Pharmaceuticals notified the agency in October 2023 that it was being discontinued, and confirmed it was not pulled for safety or effectiveness reasons. That determination exists specifically so generic versions can keep being approved.

The intrathecal pump, which is the extreme end. In 2024 Medicare claims data, 667 providers billed HCPCS J0475 (injection, baclofen, 10 mg) for 3,727 beneficiaries, at an average submitted charge of $503.38 against $167.95 allowed. The 50 mcg trial dose (J0476) was billed at $153.62 with $44.10 allowed. Implanting the programmable pump (CPT 62362) was billed at an average of $12,982.48 in a hospital setting with $3,205.58 allowed, and each refill visit (CPT 95991) was billed at $381.68 with $97.07 allowed in an office setting. The gap between billed and allowed is roughly fourfold across every one of those codes, which is exactly the spread a self-pay patient negotiates against.

Where you fill it. The same 30 tablets ran $4.80 with a discount card, $5.52 through mail order before shipping, and a stated $68.35 at average retail with nothing applied. Baclofen is not on Walmart’s Prescription Program list at any strength, so the fixed-price retail route that works for tizanidine is not available here.

Cash Price vs. Insurance

For the oral tablet, paying cash usually beats using your benefit. A $4.80 card price or a $5.52 mail-order price undercuts most $10 and $15 generic copays, and neither touches your pharmacy benefit. Our tizanidine cost guide and cyclobenzaprine cost guide reach the same conclusion for the other two common muscle relaxants, and our gabapentin cost guide covers the drug most often prescribed alongside them for the same conditions.

Money spent outside your plan does not count toward your deductible or out-of-pocket maximum, and you cannot combine a discount card with insurance on the same fill. On a $5 prescription that tradeoff is trivial. It stops being trivial the moment you move to a liquid, granules or the pump.

That is where insurance changes the picture completely. A cash payer facing branded Fleqsuvy or Ozobax DS has no realistic path to the brand at these prices, and no manufacturer coupon is listed for generic baclofen at all. If you cannot swallow tablets, the questions worth asking are whether the generic suspension or solution is acceptable, and whether a compounding pharmacy can produce a liquid from tablets more cheaply than the branded product.

Intrathecal baclofen is a hospital purchase, not a pharmacy purchase, so it is governed by hospital pricing rules rather than drug pricing. Every hospital is required to publish a machine-readable file of its cash and negotiated rates, and our guide to hospital price transparency explains how to pull the codes above out of it before you agree to anything. Run any quote you get through our fair price checker as a sanity test.

How to Pay Less

  1. Run a free discount card before you fill. SingleCare’s baclofen page, prices updated September 4, 2026, showed $4.80 for 30 of the 10 mg tablets against a stated average retail price of $68.35. That is a 93 percent gap on a card that costs nothing. Our comparison of GoodRx vs. SingleCare explains how to price the same prescription both ways and why an online quote can move between visits.

  2. Price mail order for a 90-day fill. Cost Plus Drugs charged $5.52 for 30 of the 10 mg tablets on September 4, 2026, with $5.25 of standard shipping added at checkout. Because the pharmacy labor fee and the shipping charge are flat, larger fills get proportionally cheaper, so a 90-day supply is where mail order earns its keep. Baclofen is not a federally scheduled controlled substance, so 90-day quantities are available if your prescriber writes them.

  3. Ask about the strength before you fill an unusual one. The 15 mg tablet costs a pharmacy 53 times what the 10 mg does. If a prescription arrives written for 15 mg, ask the pharmacy to quote it before dispensing, and ask the prescriber whether a common strength gets to the same daily dose. This is a prescriber decision, so raise it at the appointment.

  4. Do not assume the branded liquid is your only liquid. Generic baclofen oral solution and suspension both exist and both cost a pharmacy substantially less than Fleqsuvy. With Lyvispah discontinued as of June 30, 2025 and Ozobax 5 mg/5 mL on FDA’s discontinued list, a prescription written for a specific brand may simply not be fillable, which is a good moment to ask what the generic alternative costs.

  5. Get the hospital side priced before the procedure, not after. If a pump is on the table, ask for the estimate in writing with the CPT codes on it, then compare it against the published Medicare allowed amounts above and against the hospital’s own transparency file. If a bill has already landed for more than you expected, our guide to negotiating medical bills covers the sequence that actually works.

  6. Get the visit priced too. Baclofen is often prescribed at a neurology, rehab or primary care visit, and that appointment is frequently the larger line item. A free or sliding-scale clinic can handle routine refill visits for far less, and our physical therapy cost guide covers the therapy that usually runs alongside this prescription.

Bottom Line

Budget about $5 to $15 for 30 baclofen tablets if you use a card or mail order, and treat a quote near $68 as a reason to shop rather than a fact about the drug. The 10 mg is the cheap strength, the 20 mg costs a little more, and the 15 mg costs 53 times as much at wholesale because almost nobody makes it.

Everything above about $30 on this drug is a delivery-method decision. Generic liquid runs into the hundreds per bottle, brand Fleqsuvy costs a pharmacy roughly $718 for 120 mL, Lyvispah and the original Ozobax solution are both discontinued products living on remaining stock, and intrathecal baclofen through an implanted pump is billed at $503.38 per 10 mg dose on top of a pump that hospitals bill near $13,000 to insert. If any of those is on your prescription, the first question is whether the plain tablet will do.
