A thyroidectomy is priced as one operation and lived as two. Removing half the gland (a lobectomy) and removing all of it look nearly identical on a facility bill and diverge sharply afterward, because only one of them commits you to a daily pill and periodic blood tests for the rest of your life. A cash payer needs both halves of that math before comparing quotes.

This guide separates them using figures verified in September 2026: two surgery centers that publish bundled cash prices, the July 2026 CMS facility payment addenda, the 2026 physician fee schedule, 2024 national Medicare claims released in May 2026, CMS drug acquisition-cost data effective August 19 2026, and a multi-state all-payer analysis of same-day versus overnight thyroidectomy.

What Does a Thyroidectomy Cost Without Insurance?

Bundled cash prices for a straightforward thyroidectomy cluster between $7,155 and $8,809. Adding a neck dissection roughly doubles the top of the range.

The bundled prices come from two centers that post them. Surgery Center of Oklahoma prices every one of its non-dissection thyroid rows at $7,155, including its partial lobectomy row and its total thyroid lobectomy rows, with its lobectomy page carrying CPT 60220. The same center lists $10,760 for a thyroidectomy with neck dissection (CPT 60252), $10,460 for a complex thyroidectomy, and $14,975 for a total thyroidectomy with bilateral neck dissection. NTTC Surgery Center lists $8,809 for a total thyroidectomy under CPT 60240 in a flat-rate program covering facility, surgeon and anesthesia.

The Medicare rows are not what a self-pay patient pays. They are the cleanest measure of what the operation is actually worth, and they explain where the cash prices came from.

What Drives the Price

The building, not the gland

Under the July 2026 CMS addenda, CPT 60220 and CPT 60240 both fall into the same payment group. The ambulatory surgery center facility rate is $3,030.97 for both. The hospital outpatient rate is $6,176.47 for both. That is a 104 percent premium for walking into a hospital instead of a freestanding center to have the identical operation.

Add the 2026 physician fee and Medicare’s all-in cost for a total thyroidectomy is $3,861 at a surgery center and $7,006 at a hospital outpatient department. If your surgeon operates at both, asking which one your case is booked at is the highest-value question available to you before anything is signed.

Partial versus total, which matters less than you think up front

The facility does not charge more for taking the whole gland. Only the surgeon does. The 2026 physician fee schedule pays $640.30 for a lobectomy (60220) and $829.68 for a total thyroidectomy (60240), a difference of about $189. Surgery Center of Oklahoma charges the same $7,155 for both.

Where the difference shows up is after discharge, and it is permanent. A total thyroidectomy removes the organ that makes thyroid hormone, so replacement therapy and periodic monitoring become a lifetime line item. A lobectomy leaves half the gland, and many people never need replacement at all. That is a clinical distinction your surgeon owns, not a cost lever you can pull, but it belongs in the budget.

Whether a neck dissection is part of the plan

This is the largest step change in the whole price list. CPT 60252 adds a limited neck dissection and moves Medicare’s hospital outpatient all-in cost to $7,227, and the 2024 national average submitted surgeon charge to $4,916.15 against $3,439.61 for a plain total thyroidectomy. On the cash side the jump is bigger: $7,155 becomes $10,760 with a dissection and $14,975 with bilateral dissection at the same center. If a dissection is being discussed, get the quote re-priced rather than assuming the base number holds.

Whether you go home the same day

Medicare pays both thyroidectomy codes as outpatient procedures and covers them in freestanding surgery centers as of July 2026, so a same-day discharge is the default assumption behind every bundled price in this guide. An overnight stay changes the bill. In a multi-state all-payer analysis of 86,187 patients from 2013 to 2019, published in the journal Surgery, the median 30-day cost was $7,500 for same-day total thyroidectomy (interquartile range $5,905 to $9,894) against $8,508 for overnight admission (interquartile range $6,851 to $10,780), with a median pairwise saving of $974 for going home the same day. Our guide to hospital stay cost per day covers what happens if the stay stretches beyond one night.

The drug bill that starts the week after

If the whole gland comes out, levothyroxine becomes permanent. In CMS acquisition-cost data effective August 19 2026, generic levothyroxine 100 mcg costs pharmacies $0.05073 per tablet, which works out to roughly $1.52 for 30 days and $18.52 for a year. Brand Synthroid 100 mcg costs pharmacies $1.66245 per tablet, about $49.87 for 30 days and $607 for a year, roughly 33 times the generic. Those are acquisition costs rather than shelf prices, so retail cash sits above both, and our levothyroxine cost guide covers what pharmacies actually charge.

Dose adjustment also means blood work. Expect periodic TSH testing while the dose is being tuned and then at intervals afterward, priced in our thyroid test cost guide. Neither figure is large on its own, and both compound over decades.

Cash Price vs. Insurance

The billed-versus-paid gap on the professional side is wide. In 2024 national Medicare claims released in May 2026, the average submitted charge for CPT 60240 was $3,439.61 against a Medicare allowed amount of $732.36 across 6,289 services. For a lobectomy it was $2,991.35 against $627.57 across 9,140 services, and for 60252 with neck dissection $4,916.15 against $924.41. Every one of those is the surgeon’s fee alone, with no facility fee and no anesthesia inside it.

That is what makes a hospital’s first quote negotiable. A chargemaster number for a thyroidectomy is built on a submitted charge that already runs four to five times the allowed amount, and then a facility fee that Medicare values at $6,176.47 is stacked on top. Our guide to hospital price transparency walks through pulling a specific facility’s own posted rate for a CPT code before you call.

The usual trade-off applies to paying cash. The money does not count toward a deductible or out-of-pocket maximum, so if you expect other large claims this year, price both routes before committing. Thyroid surgery is also one of the procedures most often preceded by biopsies, ultrasounds and repeat labs, and those add up on either route.

How to Pay Less

Get the CPT code before you shop. 60220, 60240 and 60252 are three different price tiers, and quotes are only comparable within a tier. Ask the surgeon’s office which one is planned and whether a neck dissection is expected.

Ask whether the case can be done at an ambulatory surgery center. Medicare’s July 2026 rates put the same operation at $3,861 all-in at a surgery center and $7,006 at a hospital outpatient department. Whether your case is a candidate is a clinical decision, but the question is worth asking out loud.

Get bundled quotes from at least two centers. The two published prices in this guide are $7,155 and $8,809, which tells you a fair all-inclusive cash price for a straightforward thyroidectomy is roughly $7,100 to $8,900 as of September 2026. Run any quote through our fair price checker before agreeing to it.

Ask for the self-pay discount before scheduling, not after the bill. Hospitals almost always have a posted uninsured discount that is applied on request rather than automatically. If a bill has already arrived, our medical bill negotiation guide has the language, and requesting an itemized hospital bill first is what makes the negotiation possible.

Check charity care before assuming you do not qualify. Nonprofit hospitals must maintain financial assistance policies with income thresholds often well above the federal poverty line. Our hospital charity care guide and the charity care checker show how to apply to a specific hospital’s program.

Fill the thyroid hormone as a generic and buy 90 days at a time. The gap between generic levothyroxine and brand Synthroid at pharmacy acquisition cost is about 33 to one. Brand-versus-generic substitution on thyroid hormone is a conversation to have with the prescriber, since some clinicians prefer to keep a patient on one manufacturer, but it is worth having once rather than paying the brand premium by default for thirty years.

The Bottom Line

Budget $7,155 to $8,809 for a lobectomy or a total thyroidectomy as a bundled cash price at a surgery center in September 2026, $10,460 to $10,760 if a neck dissection is part of the plan, and up to $14,975 for a total thyroidectomy with bilateral dissection.

The figure to hold onto is the setting spread. July 2026 Medicare rates put a total thyroidectomy at $3,861 all-in at an ambulatory surgery center and $7,006 at a hospital outpatient department for the identical code, and the facility charges exactly the same whether half the gland comes out or all of it. Where the case is booked decides most of the bill, which is the same pattern our hernia surgery and hemorrhoid surgery guides open with. The line item unique to this operation is the one that starts the week after: a daily pill and a recurring blood test, priced in our levothyroxine and thyroid test guides.
