Tamoxifen is a five-to-ten-year prescription, and that is the thing that makes its price worth thinking about. A single month costs less than lunch. Multiplied by 60 or 120 months, and multiplied again by whether you fill 30 tablets at a time or 90, it turns into a number that varies by roughly $2,000 between the best and worst way of buying exactly the same drug.

There are two other traps on this page. Brand Nolvadex no longer exists as a purchasable product, so any price you see attached to that name is stale. And Soltamox, the oral solution version, is a genuinely expensive brand product hiding inside a drug class where everything else is pennies. Every figure below was researched on September 3, 2026.

What Does Tamoxifen Cost Without Insurance?

The low column is Cost Plus Drugs or the CMS National Average Drug Acquisition Cost, the federal survey of what pharmacies actually pay, at $0.28326 per generic 20 mg tablet effective August 19, 2026. The average column is 2024 Medicare Part D average spending per dosage unit ($0.3907972). The high column is the Drugs.com cash retail price for a 30-count. Dosing and duration come from the FDA-approved prescribing information, which specifies 20 mg daily for five to ten years in the adjuvant setting and 20 mg daily for five years for DCIS and for risk reduction.

One vintage note. The generic tamoxifen NADAC rows are effective August 19, 2026, but the SOLTAMOX 20 MG/10 ML SOLN row in the same dataset is a May 21, 2025 vintage, more than a year older. It is the most recent published figure for that product, and it lands within 4% of both the Medicare Part D per-milliliter average and the Drugs.com retail price, so the three agree, but they are not the same week and this guide does not pretend otherwise.

The Soltamox rows are the widest spread on this page. At the same 20 mg daily dose, the solution costs $1,369.44 for 30 days against $20.48 for the tablets, roughly 67 times as much for the same drug in a different container. Medicare Part D covered 428 Soltamox claims for 81 beneficiaries in 2024 at an average of $1,227.31 per claim, a small sample worth reading as directional rather than precise.

For scale, Part D spent $10.22 million on generic tamoxifen citrate across 383,294 claims for 100,297 beneficiaries in 2024, at $26.68 per prescription claim. That per-claim figure covers fills of 30, 60 or 90 days, so it is not a monthly price.

What Drives the Price

Five marketed generics and no marketed brand tablet. FDA’s Drugs@FDA record lists 14 tamoxifen citrate applications. Five generic tablet applications carry an active prescription marketing status (Actavis, Mylan, Dr. Reddy’s, Zydus and Eugia), seven generic applications are discontinued, and AstraZeneca’s Nolvadex is discontinued at both strengths. Soltamox is the only brand product still marketed. A generic-only tablet market is why the tablet price is measured in cents.

Fill length is worth more than any coupon. Drugs.com prices the 20 mg tablet at $0.68 each in a 30-count and $0.51 each in a 90-count. Over 1,826 tablets that gap is about $316, and over 3,652 tablets it is about $633. Nothing else on this page moves the number that much.

The formulation, not the molecule. Soltamox exists because some patients cannot swallow tablets. It is a legitimate product with a legitimate use, and it is also the single largest price decision in this drug class. If tablets are an option, the solution is a $1,300-a-month choice.

Strength arbitrage is real but small. Two 10 mg tablets cost $0.26632 in acquisition cost against $0.28326 for one 20 mg tablet, so the twice-daily route is marginally cheaper per day at the pharmacy’s cost. At retail the ordering can flip depending on package size. Either way it is a few dollars a month, not a strategy.

The monitoring, not the pill. Five to ten years of endocrine therapy comes with ongoing imaging and specialist visits, and those are where a self-pay budget actually gets spent. See mammogram costs and, if hereditary risk is part of the picture, genetic testing costs.

Cash Price vs. Insurance

Tamoxifen is a clear case where the cash price can beat the copay. A $20 or $25 generic-tier copay is more than the $8.45 Cost Plus price and more than a $15.28-per-month equivalent 90-day retail fill. If you have a deductible you have not met, running the claim can cost more than not running it. Ask for the cash price at the counter before you hand over a card.

The comparison changes for Soltamox. At $1,369.44 a month, this is a drug where coverage matters enormously and where a denial is worth appealing. Mayne Pharma is the manufacturer of record, and if the solution is medically necessary rather than a preference, that is the case a prior authorization needs to make.

For Medicare beneficiaries, the 2024 Part D average of $26.68 per tamoxifen claim sits above the cash price of a 30-day retail fill and well above a per-month 90-day fill. That is not a reason to skip coverage, which exists for the rest of cancer care, but it is a reason to price this specific prescription both ways every year.

One more coverage note that catches people off guard: tamoxifen is also prescribed for risk reduction in women who have never had cancer, and some plans treat that indication differently from adjuvant treatment. At these prices, paying cash is usually faster than fighting it.

How to Pay Less

Ask for a 90-day prescription on day one. This is the highest-value single action on a five-to-ten-year drug. Drugs.com prices 90 tablets of the 20 mg at $45.85 against $20.48 for 30, which is $0.51 per tablet instead of $0.68. Over ten years that is about $633.

Use Cost Plus Drugs as the benchmark, then check locally. Cost Plus lists tamoxifen citrate 20 mg at $8.45 for 30 tablets against a $76.80 retail comparison on its own page, plus $5 shipping. Three fills at once beats a 90-count retail fill here, unlike some other generics where it does not. Compare a discount card at your local pharmacy too; our guide to GoodRx vs. SingleCare covers how those actually work at the register.

Do not count on the Walmart list. Tamoxifen appears nowhere in the Walmart Prescription Program PDF at any of its four tiers, and the program has no oncology category. Verified twice, September 3, 2026.

Ask whether tablets are an option before accepting the solution. If a prescription arrives written for Soltamox and swallowing tablets is not actually a problem, that one question is worth roughly $1,349 a month. This is a conversation with the prescriber, not a decision to make at the pharmacy counter.

Go through a safety-net pharmacy if you have no coverage at all. Federally qualified health centers buy at 340B prices and frequently pass the discount along on a sliding scale, which matters much more for the oncology visits and scans than for a $10 prescription. Our guide to free and sliding-scale clinics explains how to find one.

Put the negotiating energy on the big bills. There is nothing to negotiate on a $20 prescription and a great deal to negotiate on hospital and infusion charges. Start with how to negotiate medical bills, and check any quoted price against national data with our fair price tool.

The Bottom Line

Generic tamoxifen tablets are an $8.45 to $20.48 monthly purchase, a $514 to $1,247 five-year purchase and a $1,029 to $2,493 ten-year purchase. The spread inside those ranges is almost entirely about fill size and which pharmacy you use, not about the drug.

The two things that will actually blow up a tamoxifen budget are Soltamox at $1,369.44 a month and a decade of 30-day fills at retail. For the aromatase inhibitor most postmenopausal patients take instead, see letrozole costs. If treatment moves beyond endocrine therapy, chemotherapy costs and Ibrance costs cover what comes next.
