Rotator cuff repair is a same-day operation that produces a bill with a startling number of moving parts. The tendon gets anchored back to the bone through a few small incisions, you go home a few hours later, and then the charges arrive from the facility, the surgeon, the anesthesia group, the implant vendor, the brace supplier, and the physical therapy clinic, each on its own schedule.
This guide prices all of it with figures verified in September 2026. The headline to hold onto: the surgeon’s fee is the same wherever the operation happens, and the facility’s fee is not even close. That one line item accounts for most of the spread between a $4,671 total and a $12,965 one.
What Does Rotator Cuff Surgery Cost Without Insurance?
The code that matters is CPT 29827, arthroscopy of the shoulder with rotator cuff repair. Several cheaper shoulder scopes share the operating room but not the code, so they are broken out here as well.

The bundled prices come from published self-pay lists rather than estimates. Surgery Center of Allentown posts $9,114.74 for CPT 29827, including facility, physician, anesthesia, and pathology fees, with implants priced separately. Surgery Center of Oklahoma posts $9,682 all-inclusive for the arthroscopic repair, covering surgeon, facility, anesthesia, and routine follow-up, plus a separate $250 consultation. Newport Orthopedic Institute posts $10,995 for an arthroscopic rotator cuff repair at its Newport Beach surgery center. ATX Orthopedics in Austin posts $11,000 for 29827, on a list it last updated in May 2026.
The Medicare figures come from the CMS Procedure Price Lookup tool and reflect 2026 national averages. They are not self-pay prices, but they are the cleanest measure of what the setting alone is worth.
The MDsave range is the widest number in the table at $4,833 to $26,826, with a $13,679 MDsave national average against an estimated national average of $25,494 for the same surgery. Sidecar Health’s cash data splits its $5,588 to $12,965 range cleanly by facility type, which is the most useful single fact in this guide.
What Drives the Price
Ambulatory surgery center versus hospital outpatient department
This is the dominant factor and the one you can actually influence. Under 2026 Medicare national averages, the surgeon is paid $976 for CPT 29827 no matter where the case is done. The facility is paid $3,695 at an ambulatory surgery center and $7,413 at a hospital outpatient department, which takes the total from $4,671 to $8,389. A Medicare patient’s own share goes from $934 to $1,677 for identical work.
Cash prices show the same split even more starkly. Sidecar Health’s state-level data puts surgery center averages at $5,588 to $7,702 and outpatient hospital averages at $9,120 to $12,965, so the hospital setting adds roughly 60 to 70 percent to the same repair. Every published all-inclusive bundle we could verify comes from a surgery center, because hospitals rarely post one.
How much of the cuff is torn, and what the surgeon actually does
Not every shoulder scope is a cuff repair. A partial-thickness tear treated with debridement, or impingement treated with a subacromial decompression, is a genuinely cheaper operation with its own code. Surgery Center of Allentown posts $3,435.74 for decompression (CPT 29826), $4,705.64 for limited debridement (29822), and $5,047.50 for extensive debridement (29823), against $9,114.74 for a full repair. Newport prices a debridement at $7,875 and a subacromial decompression at $8,160 against $10,995 for the repair.
A full-thickness tear that needs a double row of suture anchors instead of a single row does not change the CPT code, but it changes the implant bill and the operating room time. This is a clinical decision your surgeon makes based on the size and shape of the tear, not something you choose. What you can do is ask, before surgery, roughly how many anchors are planned and what each one costs, because both Surgery Center of Allentown and Surgery Center of Oklahoma exclude implants from their posted prices. Allentown asks for implant payment up front and refunds anything not used.
Open versus arthroscopic repair
Counterintuitively, the open version is the cheaper one on published lists. Surgery Center of Oklahoma posts $7,159 for an open rotator cuff repair against $9,682 arthroscopically. Newport posts $8,325 open against $10,995 arthroscopic. Arthroscopic repair is far more common and has a different recovery profile, so this is not a menu you order from, but if your surgeon has recommended an open repair the price is one of the few things about it that works in your favor.
Anesthesia and the interscalene nerve block
Shoulder surgery almost always involves a regional nerve block alongside general anesthesia, and at a hospital that is a separate bill from a separate group. This is the most common surprise charge after rotator cuff surgery. Every surgery center bundle in this guide states that the anesthesiologist’s professional fee is inside the posted number: Surgery Center of Oklahoma, Surgery Center of Allentown, Newport Orthopedic Institute, and ATX Orthopedics all say so explicitly. Our anesthesia cost guide covers how anesthesia is billed when it is not bundled.
The sling, the imaging, and the rehab
The extras are not trivial on a shoulder. A post-operative abduction sling with a pillow is typically worn for four to six weeks; DME-Direct’s published retail prices run from $32.99 for a basic immobilizer to $230.99 for a brand-name abduction sling, with most post-op models between $128.99 and $233. Newport Orthopedic Institute posts $900 for a self-pay MRI without contrast and $355 for an initial shoulder evaluation. And rotator cuff rehab is long: MDsave prices a single physical therapy visit at $42 to $179 nationally, and a full post-op course is measured in dozens of sessions. Our MRI cost and physical therapy cost guides cover shopping those separately.
Cash Price vs. Insurance
Rotator cuff repair is a strong candidate for paying cash. If you carry a $7,000 or $9,000 deductible, a $9,115 all-inclusive surgery center price is often less than what a hospital collects from you on the way through that deductible, and it comes without coinsurance, without a separate anesthesia bill, and without an implant charge arriving a month later.
The catch is the usual one: money paid as cash normally does not count toward your deductible or your out-of-pocket maximum. If you expect other significant claims in the same plan year, run the math on both paths before committing. Some practices will break out the surgeon fee separately so you can submit for out-of-network reimbursement yourself.
If you are uninsured or self-pay, the federal No Surprises Act entitles you to a written Good Faith Estimate before a scheduled procedure. Ask for it in writing and ask specifically whether the facility, the surgeon, the anesthesiologist, the surgical assistant, and the implants are all inside the number. Our guide to hospital price transparency explains how to pull a hospital’s own posted cash rate for CPT 29827 before you call anyone.
How to Pay Less
Ask where your case is scheduled, then ask whether the surgery center is an option. Medicare’s 2026 facility fee goes from $3,695 at an ASC to $7,413 at a hospital outpatient department for the same code, and Sidecar’s cash data shows the same gap in the real world. Many shoulder surgeons operate at both.
Get bundled quotes from three surgery centers. The published prices here span Pennsylvania, Oklahoma, California, and Texas and land between $9,115 and $11,000, which tells you a fair all-inclusive price is roughly $9,000 to $11,000. Anything meaningfully above that deserves a challenge. Run your quote through our fair price checker first.
Get the implant question answered in writing. Suture anchors are the most common line item to fall outside a bundle. Ask how many are planned, what each costs, and whether unused anchors are refunded. A repair quoted at $9,115 with anchors excluded is not cheaper than one quoted at $10,995 with everything included.
Buy the sling yourself. A brace billed through the hospital as durable medical equipment routinely costs several times the retail price of the same product. At $32.99 to $230.99 published retail, this is a small win, but it is a five-minute one.
Ask for the self-pay discount before surgery, not after. Hospitals nearly always have a posted uninsured discount that is applied on request rather than automatically. If the bill has already arrived, our medical bill negotiation guide and the negotiation script tool give you the language, and requesting an itemized hospital bill is the step that makes negotiation possible.
Check charity care before assuming you earn too much. Nonprofit hospitals are required to maintain financial assistance policies, and the income thresholds are often well above the federal poverty line. Our hospital charity care guide and the charity care checker walk through finding and applying to a specific hospital’s program.
The Bottom Line
Budget $9,115 to $11,000 for a bundled arthroscopic rotator cuff repair at an ambulatory surgery center as of September 2026, and expect a hospital outpatient department to land closer to Sidecar Health’s $9,120 to $12,965 range before implants. Add roughly $900 for the MRI, $355 for the initial evaluation, up to $231 for a sling, and a long course of physical therapy at $42 to $179 a visit.
The number worth memorizing is the facility gap. Medicare’s 2026 figures put the facility fee for CPT 29827 at $3,695 in a surgery center and $7,413 in a hospital outpatient department, with an identical $976 surgeon fee on both sides. If you are paying out of pocket, where the operation happens matters more than anything else you control. The same logic drives our ACL surgery, shoulder replacement, and meniscus surgery guides.
