Propranolol is one of the oldest beta blockers still in everyday use, and it reaches the pharmacy counter for three completely different reasons. Some people take it every day for high blood pressure. Some take it every day to prevent migraines. And a large and growing group takes it occasionally, before a presentation, an audition or a flight, for situational performance anxiety, usually after an online visit rather than an office one.

All three groups get quoted very different prices, and the reason is almost never the molecule. It is which form the prescription says, how many tablets a real month takes, and whether the label reads propranolol or Inderal. This guide prices all of it, including the online visit that many cash payers use to get the prescription. Every figure below was researched on September 4, 2026.

What Does Propranolol Cost Without Insurance?

Acquisition costs come from the CMS National Average Drug Acquisition Cost survey, effective August 19, 2026, which is what pharmacies actually pay wholesalers. Retail cash prices come from Drugs.com and Cost Plus Drugs, both read September 4, 2026. Brand marketing status comes from Drugs@FDA.

One number in that table needs a caveat before you budget from it. A 30-count of the immediate-release tablet is usually not a month. Propranolol IR is short-acting and is typically dosed two to four times a day, so a genuine 30-day supply is often 60 or 90 tablets. Drugs.com prices 60 of the 20 mg at $9.89 and 90 at $12.71, and 90 of the 40 mg at $16.08. The extended-release capsule is once daily, so 30 capsules really is 30 days.

What Drives the Price

Immediate release versus extended release. These are the same drug in two delivery systems and they are priced about three times apart. CMS puts the 80 mg IR tablet at $0.15955 and the 80 mg ER capsule at $0.19325, which is close. At retail the gap widens sharply: Drugs.com sells the 80 mg tablet at about $0.25 each and the 80 mg ER capsule at $0.52 each. If you are paying cash and your prescriber is flexible on form, that difference is worth a conversation.

Brand versus generic, and this one is extreme. Inderal LA 80 mg is $2,204.56 for 30 capsules on Drugs.com. The generic ER capsule at the same strength works out to about $15.72 for 30. That is a spread of roughly 140 to 1 for a molecule that has been off patent for decades. CMS acquisition data confirms the brand side independently: Inderal XL 80 mg is $68.78504 per capsule effective August 19, 2026, against $0.19325 for the generic.

Which brands still exist. Drugs@FDA lists the original Inderal immediate-release tablets (NDA 016418, Wyeth) and the Inderal oral suspension as Discontinued as of September 2026. What remains marketed are ANI Pharmaceuticals’ extended-release capsules: Inderal LA (NDA 018553) and InnoPran XL (NDA 021438). So if a prescription says Inderal, it is almost certainly the expensive capsule, and there is no branded tablet to fall back on.

Strength, which does not scale the way you would expect. The 10 mg tablet costs pharmacies $0.04750 and the 80 mg costs $0.15955, so you get eight times the drug for a bit over three times the price. If your prescriber writes a higher strength and you take a fraction of a tablet where that is appropriate for your prescription, the per-dose cost falls. That is a question for the prescriber, not something to do on your own.

Where you fill it, and the flat dispensing fee. Cost Plus Drugs publishes its own math, and on propranolol it is revealing. A 30-count of the 20 mg tablet is $5.93, made up of $0.81 of manufacturing cost, $0.12 of 15 percent markup, and $5.00 of pharmacy labor. Add $5.25 standard shipping at checkout. The drug is 14 percent of the bill. That flat labor fee is why Cost Plus is a poor deal on a 30-count and a good one on a 90-count.

Propranolol is not on the Walmart $4 list. A coordinate-level read of the Walmart Prescription Program drug list effective March 24, 2025 turns up no propranolol entry in any tier. Atenolol sits in the $4/$10 tier and metoprolol and amlodipine sit in the $9/$24 tier, but propranolol is simply absent. The retail-program shortcut that works for most cardiac generics does not work here.

Cash Price vs. Insurance

On generic propranolol, paying cash is usually the better move. A $7 fill is below most copays, and there is no prior authorization to clear and nothing to count against a deductible.

On the branded capsules the calculation inverts completely, and the right answer is usually neither. A $2,200 cash price for Inderal LA is not a price anyone should pay when the generic is $16. If a prescription arrives written for Inderal LA or InnoPran XL, ask the pharmacy whether it can be filled with generic propranolol ER before you pay anything. In most states a pharmacist can substitute an equivalent generic unless the prescriber has specifically blocked it.

The one genuinely insurance-shaped cost here is the visit. Getting a first prescription requires a prescriber, and a self-pay office visit is typically the largest single line in year one. Our doctor visit cost guide has the current self-pay ranges, and you can sanity-check any quote you are given with our fair price checker.

How to Pay Less

  1. Price a 90-count, not a 30-count. Because dispensing fees are flat, quantity is the single biggest lever on a drug this cheap. Drugs.com lists 90 of the 20 mg tablets at $12.71 against $7.07 for 30, so tripling the supply costs less than doubling the price. Ask your prescriber to write 90 days if the prescription is a maintenance one.

  2. Check more than one discount card on the exact strength. Card prices on propranolol move around by strength and by chain, and the card that wins on the 20 mg tablet often loses on the ER capsule. Our comparison of GoodRx vs. SingleCare covers how to run both against the same prescription before you hand it over.

  3. Refuse the brand. This is the largest possible saving on this page. Confirm with the pharmacy that your prescription can be filled as generic propranolol ER rather than Inderal LA or InnoPran XL. The difference is over $2,100 on a single fill.

  4. For occasional situational use, price the telehealth bundle, not the pharmacy. If you need a small number of tablets for a specific event rather than a daily prescription, the all-in telehealth offers can beat a separate visit plus a separate fill. RedBox Rx lists propranolol for performance anxiety at $20 for 20 tablets with no membership fee and free standard shipping. Nurx publishes a $59 consultation fee, $25 a month for the medication and a recurring $69 monthly medication management fee, which is a very different structure for the same drug. Read what recurs before you sign up.

  5. Use a sliding-scale clinic for the prescribing visit. If you need ongoing blood pressure or migraine management rather than a one-off, the visit is the recurring cost and a free or sliding-scale clinic can handle the visit and the refills on an income-based fee.

  6. Dispute anything that arrives looking like a brand price. If a pharmacy or a mail plan bills you hundreds of dollars for propranolol, that is a substitution or coding problem, not a market price. Our guide to negotiating medical bills has the scripts for getting a charge like that corrected rather than paid.

Bottom Line

Generic propranolol is a cheap drug and the cash market treats it that way, as long as the prescription says propranolol. Budget about $4.32 to $8.19 for 30 immediate-release tablets, $13.57 to $21.24 for 30 extended-release capsules, and remember that the IR tablet is usually dosed more than once a day so a real month may be 60 or 90 tablets.

The one number that can go badly wrong is the brand. Inderal LA and InnoPran XL are still marketed and still priced above $2,100 for 30 capsules, which is roughly 140 times the generic. Everything else on this page is a rounding error next to getting that substitution right. If a beta blocker was chosen for blood pressure rather than migraine or anxiety, our metoprolol cost guide and carvedilol cost guide price the two most common alternatives, and if propranolol was prescribed for migraine prevention, our sumatriptan cost guide covers the rescue side of the same budget.
