Myrbetriq (mirabegron) is a beta-3 adrenergic agonist for overactive bladder, and for most of its life it was one of the more expensive maintenance prescriptions an American could be handed. Medicare Part D alone spent $2.297 billion on it across 762,957 beneficiaries in 2024, at an average of $3,010.74 per beneficiary for the year, according to the CMS Medicare Part D Spending by Drug dataset.

Generic mirabegron arrived recently, and that is the single fact that changes what you pay. But the discount is smaller than a normal generic launch delivers, and where you fill the prescription matters more than whether the label says Myrbetriq or mirabegron. This guide uses figures read on September 3, 2026 from CMS NADAC (August 19, 2026 vintage), the Drugs.com cash price guide, Cost Plus Drugs product pages and SingleCare.

What Does Myrbetriq Cost Without Insurance?

The table below prices a standard 30-day supply. Pharmacy acquisition cost from CMS NADAC is the floor, not a retail price, and is included so you can see how much of the counter price is markup.

Product (cash, no insurance)LowAverageHigh
Generic mirabegron ER 50 mg, 30 tablets$189.28 (SingleCare card price)$240.58 (Cost Plus Drugs)$375.12 (Drugs.com price guide, high end)
Generic mirabegron ER 25 mg, 30 tablets$230.49 (Cost Plus Drugs)$281.98 (CMS NADAC acquisition, $9.39921 per tablet)$375.12 (Drugs.com price guide, high end)
Brand Myrbetriq ER 25 or 50 mg, 30 tablets$440.96 (CMS NADAC acquisition, $14.69882 per tablet)$457.52 (Drugs.com price guide)$624.63 (SingleCare stated retail)
Generic mirabegron ER, 90 tablets$1,051.75 (Drugs.com price guide)$1,109.77 (Drugs.com price guide, high end)$1,356.77 for 90 brand tablets (Drugs.com)
Mirabegron 8 mg/mL granules for suspension, 100 mL$182.35 (generic, Drugs.com)$220.69 (CMS NADAC acquisition for brand, $2.20689 per mL)$233.54 (brand Myrbetriq Granules, Drugs.com)

Two numbers there are worth sitting with. The generic at a transparent-pricing pharmacy ($240.58) is roughly half the brand price at a typical retail counter ($457.52), which is a real saving. But the same generic at that same retail counter is $355.78 to $375.12, which is only a modest discount off the brand.

The 25 mg and 50 mg strengths are priced identically at retail on the Drugs.com guide, so a dose increase does not cost more. At Cost Plus the 50 mg is $10.09 a month dearer than the 25 mg.

What Drives the Price

The generic did not cut the price the way generics usually do

Generic mirabegron extended-release first appears in the CMS NADAC file with an effective date of December 17, 2025, at $9.66406 per 50 mg tablet. Eight months later, effective August 19, 2026, it is $9.59601. That is a 0.7 percent decline over most of a year.

Compare that to brand Myrbetriq at $14.69882 per tablet on the same August 19, 2026 file. The generic sits about 35 percent below the brand at wholesale, where a mature multi-source generic normally lands 80 to 90 percent below. Prices should keep falling as more manufacturers ship, but as of September 2026 they have not.

Which pharmacy you hand the prescription to

This is the biggest swing factor and it has nothing to do with the drug. The identical 30 generic 50 mg tablets are $240.58 at Cost Plus Drugs, $355.78 to $375.12 on the Drugs.com retail price guide, and $189.28 with a SingleCare card according to SingleCare’s own page. That is a spread of roughly 2x on one prescription.

Discount-card prices are also the least stable figures in this whole picture. Card networks renegotiate per-pharmacy rates constantly, so treat the $189.28 as evidence that a low number exists in the market, not as a price you are guaranteed at any particular counter.

Tablets versus granules for children

Myrbetriq Granules is a separate product, not just a liquid version of the tablet. Per the FDA label published on DailyMed, the granules are indicated for neurogenic detrusor overactivity in pediatric patients aged 3 years and older, and the label states explicitly that the tablets and granules are not substitutable on a milligram-per-milligram basis.

A 100 mL bottle of brand Myrbetriq Granules is $233.54 on Drugs.com, with a CMS NADAC acquisition cost of $220.69. A generic from Ascend Laboratories entered the market on June 15, 2026 under ANDA 219323 and Drugs.com prices it at $182.35 per 100 mL bottle. That generic is too new to have a NADAC row yet, so the $182.35 rests on a single published source.

There is no manufacturer help

Astellas has effectively exited the affordability side of this product. Read on September 3, 2026, myrbetriq.com has no cost page, no savings card and no copay program anywhere in its navigation, and the “Medicine Savings” link in its own footer points to an empty page anchor. Myrbetriq is also missing from the product list on Astellas Pharma Support Solutions, which supports Cresemba, Xtandi, Padcev, Xospata, Izervay, Veozah and Vyloy.

Drugs.com independently reports “No manufacturer promotions could be found” and “No Patient Assistance Programs were found” for both Myrbetriq and generic mirabegron. If you have been searching for a Myrbetriq coupon and coming up empty, that is why.

It repeats every month

Overactive bladder is a chronic condition and mirabegron is a daily maintenance drug, so this is not a one-time expense. At the Drugs.com retail figure, a year of generic mirabegron is roughly $4,270 and a year of brand Myrbetriq is roughly $5,490. At the Cost Plus price the same year of generic is about $2,887 plus shipping. The annual gap between the best and worst channel is larger than most people’s deductible.

Cash Price vs. Insurance

For a drug in this price band, insurance usually wins if you have it, because a formulary generic copay is almost always below $230. The cash-versus-insurance question is really a question for people who are uninsured, in a high-deductible plan before the deductible is met, or on a plan that excludes the drug.

If you are pre-deductible, run the math both ways. Paying the cash or discount-card price is typically cheaper in the moment, but that spending usually does not count toward your deductible or out-of-pocket maximum, which matters if you expect other large medical bills this year. If you are on a plan you will not exhaust, the lower number simply wins.

Ask the pharmacist for the cash price before the claim goes through and compare it to your copay. Pharmacists are permitted to tell you which is cheaper. Our comparison of GoodRx vs. SingleCare walks through how the discount-card channel prices drugs like this one.

How to Pay Less

1. Ask the prescriber to write for the generic, then price it at a transparent pharmacy. Cost Plus Drugs publishes its full math: mirabegron ER 50 mg for 30 tablets is $204.85 in drug cost, $30.73 in 15 percent markup and $5.00 in pharmacy labor for $240.58, with $5.25 standard shipping added at checkout. Delivered, that is $245.83, against $457.52 for the brand at a retail counter.

2. Price at least three pharmacies with a free discount card in hand. The 2x spread on identical tablets is the whole opportunity here. Run the drug through more than one card, and use the fair price checker to see what a reasonable cash number looks like before you commit.

3. Ask about a 90-day fill. Drugs.com prices 90 generic tablets at $1,051.75 to $1,109.77, which works out to about $11.69 per tablet versus $11.86 for the 30-count. The per-tablet saving is small, but three months in one transaction means one dispensing fee instead of three and fewer trips.

4. Compare the whole drug class on price, not just this molecule. Older overactive bladder generics such as oxybutynin and solifenacin (Vesicare) are far cheaper per month than mirabegron, and Gemtesa, the other beta-3 agonist, is more expensive because it has no generic. Which drug is medically right for you is your prescriber’s call, but the price differences are large enough to be worth raising at the appointment.

5. Do not overlook a federally funded health center. Many community health centers run in-house pharmacies at 340B pricing and charge on a sliding scale based on income. Our guide to free and sliding-scale clinics covers how to find one and what the income tiers look like.

6. Deal with the visit bills separately. The urology workup that produced this prescription, whether that was urodynamic testing or a cystoscopy, can easily cost more than a year of the drug. If one of those bills has already landed, our walkthrough on negotiating medical bills covers what to ask for and how to ask for it in writing.

The Bottom Line

If you are paying cash, generic mirabegron at a transparent-pricing or discount-card channel is the number to aim for: roughly $190 to $245 for a 30-day supply. If you are quoted more than about $375 for 30 generic tablets, another pharmacy in your area is charging materially less for the same bottle.

Brand-name Myrbetriq has no cost justification for most cash payers now that the generic exists, and Astellas offers no savings card or patient assistance to soften the difference. The two situations where the brand still comes up are a prescriber who wants a specific product and the pediatric granules, where the generic launched only in June 2026 and is not yet widely stocked. For the other beta-3 agonist, see our guide to Gemtesa costs.
