Hydrochlorothiazide is very close to the cheapest prescription drug this site prices. CMS puts the acquisition cost of a 25 mg tablet at just over a penny, which means a pharmacy’s entire drug cost for a month is 35 cents. There is no coupon strategy that beats that, because there is almost nothing left to discount.

Which makes this a guide about the other costs. Being on a thiazide diuretic means periodic checks of your electrolytes and kidney function, and it means office visits to order those checks and adjust the dose. Those two things cost between five and seventeen times what the pills cost, every year, and they are what an uninsured person should actually be budgeting for. Every figure below was researched on September 4, 2026.

What Does Hydrochlorothiazide Cost Without Insurance?

Drug acquisition costs come from the CMS National Average Drug Acquisition Cost survey, effective August 19, 2026 for most strengths and August 26, 2026 for the 50 mg tablet. Lab fee schedule figures come from the CMS Clinical Laboratory Fee Schedule for CY2026 Q3. Billed-charge and allowed-amount figures are 2024 national averages from CMS’s Medicare Physician and Other Practitioners by Geography and Service dataset, last updated May 21, 2026. Retail prices come from Walmart’s program list, Drugs.com and Cost Plus Drugs.

One row in that table deserves a second look. Drugs.com lists 90 of the 25 mg tablets at $5.35, which is cheaper than Walmart’s $10 for the same 90 days. On a drug this cheap the retail programs are not automatically the winner, and it is worth pricing the 90-count both ways.

What Drives the Price

The drug is almost free, so the dispensing fee is the price. Cost Plus Drugs publishes its own itemization and on hydrochlorothiazide it is stark. Of the $5.24 it charges for 30 of the 25 mg tablets, $0.21 is the medication, $0.03 is the 15 percent markup, and $5.00 is pharmacy labor, with $5.25 standard shipping added at checkout. You are buying the act of dispensing, not the chemical.

Quantity, for exactly that reason. Because the fee is flat and the drug is not, the cost per month falls fast as the count rises. CMS acquisition cost for 1,000 of the 25 mg tablets is $11.65, and Drugs.com lists 1,000 at $16.45. If your prescriber writes 90 or 100 days at a time, you cut the number of dispensing fees you pay in a year from twelve to four.

Strength, which is not proportional. The 25 mg tablet is the cheapest per tablet at $0.01165 in CMS data. The 12.5 mg tablet costs three times as much at $0.03730 and the 12.5 mg capsule costs $0.02792, more than double the 25 mg tablet despite containing half the drug. The 50 mg tablet is $0.03098. If you are prescribed 12.5 mg, that is the most expensive per-milligram way to take this drug.

Whether it comes as a combination tablet. Most hydrochlorothiazide in the United States is not dispensed alone. It is dispensed inside lisinopril-HCTZ or losartan-HCTZ. CMS prices lisinopril-hydrochlorothiazide 20-25 mg at $0.04048 a tablet and losartan-hydrochlorothiazide 50-12.5 mg at $0.05732, and both combinations sit in Walmart’s $4 tier. Taking them as one tablet means one $4 fill instead of two, which halves the dispensing cost. Our lisinopril cost guide and losartan cost guide price the other half of each combination.

The monitoring, which is the real annual number. Thiazides move potassium, sodium and kidney function, so prescribers order metabolic panels. Priced against Medicare’s own numbers, two office visits and two comprehensive metabolic panels a year is $191.40 at allowed amounts and $675.80 at average submitted charges. That range, not the $4 fill, is what an uninsured person on hydrochlorothiazide is actually signing up for. Our guides to comprehensive metabolic panel costs and blood work without insurance cover how to shop that piece.

Cash Price vs. Insurance

On the drug itself, do not use insurance. A $4 Walmart fill or a $4.62 Drugs.com price is below almost any copay, and running a claim on a $4 drug can leave you paying more than the drug costs while adding a claim to your record for no benefit.

On the labs and the visits, the calculation flips completely. A comprehensive metabolic panel is billed at an average of $61.45 and allowed at $10.33, a gap of about six to one, and an office visit is billed at $276.45 and allowed at $119.71. Those discounts are negotiated, not posted, and you do not get them by asking politely at a hospital registration desk. If you have any coverage at all, use it for the testing even while you pay cash for the pills.

If you are fully self-pay, the equivalent move is to ask for the self-pay rate before the test is performed, in writing. Hospitals publish machine-readable cash prices, and our guide to hospital price transparency explains how to read one before you book. Run any quote through our fair price checker first.

How to Pay Less

  1. Take the Walmart program price and stop shopping the drug. Hydrochlorothiazide 12.5 mg, 25 mg and 50 mg tablets and the 12.5 mg capsule are all in the $4 per 30 days and $10 per 90 days tier of the Walmart Prescription Program list effective March 24, 2025. That is $40 for a year, no card, no coupon and no sign-up. Then stop optimizing the drug, because there is nothing left to save.

  2. But price the 90-count both ways anyway. Drugs.com lists 90 of the 25 mg tablets at $5.35, which undercuts the $10 program price for the same supply. A discount card can also beat both on a given day. Our comparison of GoodRx vs. SingleCare covers how to run more than one against the same prescription.

  3. Ask for the combination tablet if you take both drugs. If you are on lisinopril or losartan alongside hydrochlorothiazide, one combination tablet is one fill and one dispensing fee instead of two, and both combinations are in the same $4 tier. That is a straightforward halving of your annual pharmacy cost.

  4. Shop the metabolic panel separately from the clinic that orders it. Lab orders are portable. A direct-pay lab or a retail draw site will often quote a fraction of what a hospital outpatient lab bills, and the order travels with you. Ask what the cash price is for CPT 80048 or 80053 specifically, since naming the code gets you a real number instead of an estimate.

  5. Use a sliding-scale clinic for the visits. The office visit is the largest recurring line on this page at $85 to $276 apiece. A free or sliding-scale clinic can handle the blood pressure check, the refill and the lab order on an income-based fee, which collapses the biggest number in the annual budget.

  6. Negotiate the lab bill that shows up anyway. With billed-to-allowed ratios around six to one on metabolic panels, there is real room in almost every lab bill. Our guide to negotiating medical bills has the scripts, and our negotiation script tool will draft one for a specific charge.

Bottom Line

Hydrochlorothiazide costs $4 to $5.24 for 30 tablets and about $40 to $63 for a year, and no amount of coupon-hunting will improve on that meaningfully. The pills are a solved problem.

What is not solved is the rest of it. Two office visits and two metabolic panels a year run $191 at Medicare-allowed rates and $676 at average billed charges, which means the surrounding care costs five to seventeen times the drug. Put your effort there: take the $4 program fill without thinking about it, ask for the combination tablet if you are on an ACE inhibitor or an ARB, shop the metabolic panel by CPT code, and get the visit onto a sliding scale if you qualify. If a loop diuretic was prescribed instead of or alongside the thiazide, our furosemide cost guide tells a very similar story, and our doctor visit cost guide prices the visit line in more detail.
