Heart valve replacement is one of the few big operations where the cheaper version is also the newer one. Transcatheter aortic valve replacement (TAVR) threads a valve up through the femoral artery and usually sends the patient home in a day or two. Open surgical replacement (SAVR) opens the chest, runs the heart-lung machine, and books a week of hospital time. The device costs more in the first case and the hospital costs more in the second.
That is the whole economics of valve pricing, and it explains why the numbers you find online swing from $42,000 to over half a million dollars. This guide covers 2026 cash prices for TAVR, open aortic valve replacement, and mitral repair or replacement, what the mechanical versus tissue valve decision does to your long-run costs, and where the negotiating room actually sits. Figures are 2024 Medicare claims released in 2026 and cash prices verified in September 2026.
What Does Heart Valve Replacement Cost Without Insurance?
There are three price layers here and they are not interchangeable. Prepaid cash marketplaces show what a self-pay patient pays when the price is fixed in advance. Hospital price-transparency files show posted cash rates. Medicare inpatient claims show what hospitals submit, which is the number an uninsured patient gets billed off.

Sources: MDsave’s prepaid national listings for cardiac valve repair or replacement ($42,613 to $65,450, average $55,881, against an estimated non-discounted national average of $108,068), for the version bundled with cardiac catheterization ($53,698 to $105,765, average $76,951), and for TAVR ($97,864 against an estimated national average of $99,284); Turquoise Health’s average cash price across hospital price-transparency files; 2024 Medicare inpatient claims for MS-DRG 212, 216 through 221, and 266 and 267, released April 2026; 2024 Medicare physician claims for CPT 33361, 33405, 33426 and 33430, released May 2026.
Notice that MDsave’s prepaid valve surgery average of $55,881 and the average submitted charge of $398,710 for MS-DRG 219 describe operations on the same organ. One is a price. The other is an opening position.
What TAVR actually costs versus open surgery
In 2024 Medicare claims, TAVR was the far more common procedure: 65,402 admissions across MS-DRG 266 and 267, against 26,254 for open valve surgery without catheterization in MS-DRG 219 through 221. TAVR was also the cheaper admission. Average total payments were $57,046 (with a major complication) and $44,064 (without), while open surgery ran $82,051, $54,391 and $44,816 across its three severity tiers.
That gap is not the device. The transcatheter valve is the more expensive piece of hardware. It is the hospital stay, the intensive care time, and the operating room, all of which are shorter or absent with a catheter-based approach.
What Drives the Price
The facility charge, which is nearly everything
In 2024 Medicare physician claims, surgeons submitted an average charge of $4,510 for TAVR (CPT 33361) and $7,328 for open aortic valve replacement (CPT 33405), with Medicare allowing $731 and $1,255. Set that against an average hospital submitted charge of $221,241 to $398,710 for the same admissions. The professional fee is roughly 2% of the bill. Everything you can meaningfully argue about sits on the hospital’s side of the statement, the same way it does on /costs/heart-bypass-surgery-cost/ and /costs/implantable-defibrillator-cost/.
Which valve, and whether more than one is involved
Mitral work bills higher than aortic on the professional side: $8,589 average submitted charge for mitral valve replacement (CPT 33430) and $7,295 for mitral repair with a prosthetic ring (CPT 33426), against $7,328 for the aortic equivalent. The bigger jump is doing two valves at once. MS-DRG 212, concomitant aortic and mitral valve procedures, carried an average submitted charge of $568,203 in 2024, the highest figure in this guide.
Whether a catheterization is bundled in
Adding a cardiac catheterization to the same admission moves the case into MS-DRG 216 through 218, where average submitted charges were $492,416, $331,660 and $319,239. On the cash side the same pattern shows up as MDsave’s split between $55,881 without the catheterization and $76,951 with it. If you have already had a diagnostic /costs/angiogram-cost/, ask whether it needs repeating inside the surgical admission.
Complications and length of stay
The severity tiers tell the story cleanly. For open valve surgery in 2024, average submitted charges were $232,005 without a complication or comorbidity, $276,025 with one, and $398,710 with a major complication. A single complicated night is worth more than any discount you will negotiate, which is also why /costs/hospital-stay-cost-per-day/ and /costs/anesthesia-cost/ are worth reading before you sign anything.
Geography
State variation is extreme. For MS-DRG 267 (TAVR without a major complication) in 2024 Medicare data, average submitted charges ran from $73,891 in Maryland to $382,326 in Nevada, with California at $302,584. For MS-DRG 219 the spread was $141,174 in Maryland to $705,611 in California. A 2024 analysis of hospital price-transparency disclosures found TAVR price ranges within individual hospitals as wide as $119,043, and only 311 of 640 ranked hospitals disclosed a price for even one relevant code.
Mechanical versus tissue valve
The implant charge is similar either way. The difference is what happens afterward. A mechanical valve commits you to warfarin and regular INR testing indefinitely, which is a recurring cost stream rather than a one-time charge.
The drug is not the problem. Warfarin’s national average acquisition cost was about $0.09 per tablet in NADAC data effective August 19 2026, roughly $33 a year, and /costs/warfarin-cost/ covers the retail side. The monitoring is the cost. In 2024 Medicare claims, the average submitted charge was $27.41 for an INR blood test (CPT 85610) and $37.75 for the anticoagulant management service (CPT 93793). Monthly checks at those charge levels come to about $780 a year, or roughly $16,000 over 20 years. Home INR testing bills higher still: the average submitted charge for the test materials code (G0249) was $339.79, with Medicare allowing $87.72.
A tissue valve avoids that stream but carries its own long-run possibility of a second procedure. Which valve is right is a clinical decision, not a shopping one. The point here is only that the sticker price of the operation is not the whole cost of the choice.
Cash Price vs. Insurance
Valve replacement is an inpatient operation at a program that must meet federal volume requirements, so the self-pay playbook is narrower than it is for outpatient procedures. Medicare’s national coverage determination for TAVR, finalized June 21 2019, requires an established program to perform at least 50 aortic valve replacements a year including 20 TAVRs, or 100 over two years including 40 TAVRs, plus 300 percutaneous coronary interventions annually and a formal heart team of two cardiac surgeons and an interventional cardiologist. You cannot move this procedure to a cheaper surgery center.
What you can do is get the price fixed before admission. MDsave’s prepaid bundles at $42,613 to $105,765 are roughly half the estimated non-discounted national average of $108,068 for the same service, and the bundle is a single price rather than a stack of separate bills arriving for three months. Turquoise Health’s cross-hospital average posted cash rate of $97,879 sits well below every Medicare billed-charge figure in this guide, which tells you the chargemaster number is not a real price.
If you do have coverage, use it. Even a high-deductible plan caps your exposure on a six-figure admission in a way no cash discount will.
How to Pay Less
Pull the hospital’s posted cash rate before you are admitted. Every hospital must publish machine-readable standard charges including a discounted cash price. /save/hospital-price-transparency/ shows how to find and read those files, and /tools/fair-price/ lets you check a quote against the benchmarks above.
Ask for a prepaid bundle in writing. A bundled price folds the facility, surgeon, and anesthesia into one number. Compare it against the MDsave averages in the table rather than against the hospital’s own estimate.
Apply for charity care before the surgery, not after. Nonprofit hospitals must maintain a written financial assistance policy, and on a $200,000 admission that is worth more than any discount you can talk your way into. /save/hospital-charity-care/ covers eligibility and /tools/charity-care-check/ helps you locate a specific hospital’s policy. Signing a payment plan first can disqualify you.
Get the itemized bill and read the implant line. /save/itemized-hospital-bill/ walks through requesting line-level detail. On valve cases the device, the perfusion time, and the intensive care days are where duplicate and inflated charges cluster.
Negotiate against what hospitals actually collect. In 2024 Medicare data, hospitals collected average total payments of $44,064 on TAVR admissions they billed at $221,241, about 20%. /save/how-to-negotiate-medical-bills/ and /tools/negotiation-script/ give you the language for opening at a number you can defend.
Price the follow-on care too. If a mechanical valve is in play, budget the INR monitoring stream described above, and factor in /costs/echocardiogram-cost/ for the surveillance imaging that follows any valve procedure.
Bottom Line
Heart valve replacement is a $42,613 to $105,765 procedure at cash prices as of September 2026, and a $221,241 to $568,203 admission at billed-charge levels. TAVR is the cheaper admission despite the more expensive valve, because the hospital time is shorter.
The gap between those two sets of numbers is the entire opportunity. Hospitals collect around 20% of what they bill on these cases, so a six-figure statement is a starting position rather than a debt. Fix the price before admission if you can, apply for financial assistance early if you cannot, and remember that with a mechanical valve the operation is the beginning of the cost rather than the end of it.
