Farxiga is AstraZeneca’s brand name for dapagliflozin, an SGLT2 inhibitor taken as one 5 mg or 10 mg tablet a day. Like the other drugs in its class, it is prescribed across type 2 diabetes, heart failure, and chronic kidney disease, so the same tablet turns up in three different kinds of medication list.
Of the three big brand-name diabetes tablets that Medicare negotiated prices for in 2026, Farxiga is the one whose cash market changed the most. Generic dapagliflozin arrived this year, AstraZeneca opened a $35 direct-to-patient channel, and the company’s own charity program stopped taking new Farxiga patients as a direct result. All three of those things happened in 2026, and the numbers below were verified on September 1, 2026.
What Does Farxiga Cost Without Insurance?
Brand prices come from AstraZeneca’s own direct-purchase site and the federal TrumpRx listing. Generic prices come from Cost Plus Drugs, TrumpRx’s generic page, and SingleCare’s live dapagliflozin page updated September 2, 2026. The negotiated price comes from the CMS 2026 maximum fair price fact sheet.
| Option | Low | Average | High |
|---|---|---|---|
| Generic dapagliflozin 10 mg, 30 tablets, Cost Plus Drugs | $7.43 + $5.25 shipping | $12.32 | $12.32 |
| Generic dapagliflozin 10 mg, 30 tablets, Amazon Pharmacy | $8.00 (with Prime) | $8.00 | $539.70 (without Prime) |
| Generic dapagliflozin, 30 tablets, retail counter with a coupon | $14.04 | $23.56 | $25.29 |
| Brand Farxiga, 30 tablets, AstraZeneca Direct | $35.00 | $35.00 | $35.00 |
| Brand Farxiga, 30 tablets, average retail cash | $758.49 | $758.49 | $758.49 |
| Medicare Part D negotiated price (Part D enrollees only) | $178.50 | $178.50 | $178.50 |
The sources genuinely disagree on the top end, and it is worth saying so plainly. TrumpRx cites a $377.82 reference price for 30 brand tablets, SingleCare reports a $758.49 average retail cash price for the same count, and CMS recorded a $556.00 list price for 2023. Those are three different measurements (a federal reference price, an observed pharmacy average, and a historical manufacturer list price), and the honest range across them is roughly $378 to $758. What is not in dispute is that no cash payer needs to pay any of those numbers in 2026.
The generic changed everything, unevenly
The FDA’s first-generic approval list shows a wall of dapagliflozin entries dated April 6, 2026, granted to Sandoz, Teva, Zydus, Cipla, Lupin, Alkem, Alembic, Aurobindo, Biocon, MSN, Micro Labs, Macleods, Ajanta, Inventia, and others, covering both plain Farxiga and the Xigduo XR combination.
What that has not produced yet is a single settled price. On one September 2026 snapshot, TrumpRx showed the identical generic at $8.00 through Amazon Pharmacy with a Prime membership and $539.70 without one, $12.32 through Cost Plus Drugs, and $14.04 to $25.29 at nearby retail counters with a coupon. SingleCare’s page put the spread at $16.10 to $498.72 across chains. A 60-fold spread on the same molecule is the defining feature of a freshly genericized drug, and it means the pharmacy you pick matters more here than anywhere else on this site.
Cost Plus Drugs publishes its arithmetic, which is a useful reality check on the whole category: $2.11 to manufacture 30 tablets, a $0.32 markup, $5.00 of pharmacy labor, and $5.25 of shipping.
What Drives the Price
Brand versus generic. Brand Farxiga at $35 through AstraZeneca Direct is cheap by brand standards. Generic dapagliflozin at $7 to $25 is cheaper still. Both are enormously below an unhelped retail counter.
Which channel you buy through. AstraZeneca sells brand Farxiga directly at $35 per 30 tablets, with the prescription e-prescribed to ASPN Pharmacies and either delivered or transferred to a local pharmacy for pickup. That price does not exist at a walk-in counter.
Whether your pharmacy has repriced the generic. Six months after launch, some chains are at $14 and others are still at $498 for the same bottle. This is the single most expensive assumption a cash payer can make.
Prime membership, oddly. Amazon Pharmacy’s price on TrumpRx was $8.00 with Prime and $539.70 without it. That is a membership gate, not a drug price.
Strength and quantity. Farxiga and its generic come in 5 mg and 10 mg, in 30, 60, and 90 counts. Cost Plus lists the 5 mg generic at $7.07 for 30 against a $618.90 retail comparison, so the two strengths price similarly.
Cash Price vs. Insurance
CMS negotiated a maximum fair price of $178.50 for a 30-day supply of Farxiga, effective January 1, 2026, down 68% from the $556.00 list price CMS recorded for 2023. Farxiga accounted for $4.3 billion in Part D gross covered drug costs in 2023, with 994,000 Part D enrollees using it.
Here is the unusual part: for Farxiga, the negotiated Medicare price is no longer the best price on the board. A Part D enrollee paying $178.50 is paying more than a cash customer buying brand Farxiga direct from AstraZeneca at $35, and vastly more than a cash customer filling generic dapagliflozin at $8 to $25. That does not mean Medicare enrollees should abandon their coverage, because Part D dollars count toward the $2,100 out-of-pocket cap that switches on catastrophic coverage and cash dollars do not. It does mean this is one of the rare drugs where checking the cash price against your copay is genuinely worth doing every year.
The same logic applies to commercial insurance. If your plan puts Farxiga on a tier with a copay above $35, the direct price beats it outright, and the generic beats it by an order of magnitude. AZ&Me is closing for this drug
AstraZeneca’s patient assistance program, AZ&Me, stopped accepting new Farxiga and Xigduo XR patients on May 1, 2026. Patients already enrolled can keep receiving the medicine until December 31, 2026, with a final refill request due by December 15, 2026, and AstraZeneca states flatly that there are no exceptions or extensions. The company’s stated reason is that broad generic availability arriving in April made the charity program unnecessary, and it now directs Farxiga patients to AstraZeneca Direct.
If you have been relying on AZ&Me for this prescription, that is a hard deadline worth putting on a calendar now rather than discovering at a pharmacy counter in January.
How to Pay Less
-
Price the generic before anything else. At $7.43 plus shipping through Cost Plus Drugs or $8.00 at Amazon Pharmacy with Prime, generic dapagliflozin is one of the cheapest chronic-disease medications in the country right now. Your prescriber has to write it that way, so ask before the prescription is sent.
-
Check TrumpRx for the current published floor. The federal site at trumprx.gov carries both the brand Farxiga page and a generic dapagliflozin page that compares home-delivery and local pickup prices side by side, and it flags when a cheaper generic equivalent exists.
-
Use AstraZeneca Direct if you want the brand. $35 per 30 tablets, with benefits verified for you and delivery or local pickup. There is no income test to use a direct-purchase price.
-
Run a discount card at the counter if you fill locally. The gap between the card price and the no-card price on this molecule ran into the hundreds of dollars in September 2026. Our comparison of GoodRx vs. SingleCare covers how to check both on the same fill.
-
If you are already on AZ&Me, plan for December 31, 2026. Request your last refill by December 15 and line up a generic source before the program closes.
-
Do not let the office visit eat the savings. A free or sliding-scale clinic can handle prescribing and follow-up at little or no cost, and a routine cash-pay doctor visit typically runs $100 to $300 otherwise.
Bottom Line
Budget $7 to $25 a month for generic dapagliflozin and $35 a month for brand Farxiga bought directly from AstraZeneca. Budget $758 only if you walk into a pharmacy with no card, no coupon, and no questions, which in 2026 is an avoidable mistake rather than a market price.
The contrast with the rest of this shelf is instructive. Jardiance is the same drug class with no U.S. generic, and it still costs $277 to $768 a month in cash. Januvia sits in between, with a generic that launched in mid-2026 but has not repriced everywhere. If Farxiga was prescribed for heart failure rather than diabetes, our Entresto cost guide covers the drug most often filled alongside it.
