Clomiphene citrate is an unusual drug to price, because it has two completely separate cash markets that never meet. In one, it is the cheapest tablet in a fertility clinic’s cabinet and the real bill is the monitoring wrapped around it. In the other, it is prescribed off-label to men to raise their own testosterone, and it lives on a telehealth invoice next to injectable testosterone and hCG.

This guide prices both, plus the hCG that sits alongside clomiphene in each market. Every figure was verified on September 2, 2026. Nothing here is medical advice, and where a dosing pattern is described it is only so the arithmetic works.

What Does Clomid Cost Without Insurance?

Acquisition cost comes from CMS National Average Drug Acquisition Cost data effective August 19, 2026. Cash and discounted retail prices come from SingleCare pages updated September 2, 2026. Cycle costs come from two fertility clinics that publish their pricing.

ItemLowAverageHigh
Clomiphene citrate 50 mg, per tablet$4.65 (CMS NADAC)$5.44 (card retail)$16.20 (retail cash)
Clomiphene, typical five-day course$23 (acquisition)$27 (with a discount card)$81 (retail cash)
Clomiphene, 30 tablets$140 (acquisition)$163 (with a discount card)$486 (retail cash)
Oral ovulation drugs, per cycle (clinic estimate)$30$80$130+
hCG 10,000 unit vial$350.13 (card, Kroger)$365$384.99 (retail cash, CVS)
Ovidrel 250 mcg prefilled trigger syringe$248.30 (card)$283$318.05 (retail cash)
Cycle monitoring and bloodwork$300 (Illume, low end of clinic and lab services)$1,000$2,000 (CNY, upper clinic range)
Monitored ovulation-induction cycle, all in$1,000$1,500$2,000+

The sources genuinely diverge on the tablet, and it is worth saying so plainly. CMS puts wholesale acquisition at $4.65 a tablet, SingleCare shows a $243.04 retail cash price for 15 tablets (about $16.20 each) and an $81.54 discounted price (about $5.44 each), and Illume Fertility tells patients to budget $30 to $130 for a cycle of oral medication. Those are three different measurements of the same pill, and the honest range for a five-day course is $23 to $81.

Market one: ovulation induction in women

Here the drug is a rounding error and the cycle is the product. A monitored cycle typically includes a baseline ultrasound, one or more follicle-tracking scans, hormone bloodwork, the tablets, and often a trigger injection to time ovulation.

Illume Fertility’s published cost guide, updated April 22, 2026, prices ovulation induction with timed intercourse at $1,000 to $2,000 per cycle, with the oral medication at $30 to $130 of that. Its separate IUI page breaks the same components out differently: diagnostic testing at $50 to $300 or more per test, clinic and lab services at $300 to $1,200 or more per cycle, and the insemination procedure at $300 to $650 or more, landing at $1,200 to $2,350 per cycle all in.

CNY Fertility’s IUI cost page puts monitoring and bloodwork at $1,000 to $2,000 depending on the clinic, trigger shots at $100 to $300, medication from about $30 for a letrozole-only cycle up to roughly $2,000 for injectable gonadotropins, and the insemination itself at about $1,000 on average.

If insemination is part of the plan rather than timed intercourse, our IUI cost guide prices that pathway in full, and our IVF cost guide covers what happens if oral cycles do not work. The scans themselves are ordinary transvaginal ultrasounds, and our ultrasound cost guide is useful for checking whether a clinic’s per-scan charge is in line with what an imaging center would charge for the same study.

Market two: off-label use in men

Clomiphene is also prescribed off-label to men as an alternative to testosterone replacement, on the theory of stimulating the body’s own production rather than supplying testosterone from outside. It carries no FDA approval for that use, which is why it is almost entirely a cash market and why it turns up bundled inside men’s health telehealth subscriptions rather than billed to a plan.

The pharmacy price is identical to the fertility price, but the quantity is not. A woman is typically written a five-tablet course per cycle. A man is typically written a continuing daily or every-other-day supply, so the relevant unit is 30 tablets: about $140 at CMS acquisition cost, $163 at SingleCare’s discounted rate, or up to $486 at a retail counter with no card.

Telehealth clinics rarely sell the tablet at pharmacy prices. They fold it into a monthly membership, which is a different product with a different price. Our TRT cost guide prices those memberships at $25 to $269 a month and explains what is and is not included, and our testosterone injection cost guide covers the injectable alternative at $22 to $37 per 10 mL vial.

hCG, and why the cheap version disappeared

Human chorionic gonadotropin shows up in both markets: as the trigger injection that times ovulation in a fertility cycle, and as an adjunct some men’s clinics add to a testosterone protocol.

Current cash prices are high and remarkably flat. SingleCare’s chorionic gonadotropin page updated September 2, 2026 shows 10,000 unit vials at $353.13 at Kroger and Harris Teeter, $371.96 at Walgreens, $375.35 at Publix and $384.99 at CVS, with discount-card prices only about $3 below each. Ovidrel, the recombinant 250 mcg prefilled syringe, was $318.05 retail and $248.30 with a card.

The reason discounts barely move those numbers is regulatory. On March 23, 2020, a provision of the Biologics Price Competition and Innovation Act converted approved applications for a set of protein products into biologics licenses. FDA’s notice to compounders names human chorionic gonadotropin as one of four bulk drug substances affected, and states that transitioning biological products are not eligible for the compounding exemptions in sections 503A and 503B. In plain terms, the inexpensive compounded hCG that used to circulate through men’s health and fertility clinics is no longer legally available from compounding pharmacies, and what remains is the FDA-approved product at FDA-approved product pricing.

What Drives the Price

Whether you are buying a drug or a cycle. The tablet is $23 to $81 for a course. The monitored cycle around it is $1,000 to $2,000. Almost everyone who searches for a Clomid price is really trying to budget for the second one.

How much monitoring your protocol calls for. A cycle with a baseline scan and one follicle check costs a fraction of one with four scans and serial bloodwork. Clinics differ enormously here, and it is a fair question to ask before you start.

Whether a trigger shot is used. That is a $248 to $385 line item that many people do not know is coming.

Quantity dispensed. Five tablets versus thirty tablets is the difference between an $81 fill and a $486 fill at the same counter, at the same per-tablet price.

Whether a discount card is run. SingleCare’s own figures put a 15-tablet fill at $243.04 without a card and $81.54 with one. That is a 66% swing on a generic.

Cash Price vs. Insurance

Clomiphene is the rare drug where having insurance may not help much, because coverage tends to split the drug from the service.

Many plans cover clomiphene as an ordinary generic tablet, since it is cheap and long off patent, while excluding infertility treatment as a benefit category. The practical result is that the pill is paid and the ultrasounds, bloodwork and clinic fees are not, which is exactly backwards from what the patient needs. Off-label prescribing to men is usually denied outright, since it sits outside the approved indication.

That makes this a category where you should get the self-pay quote in writing before you assume anything. Fertility clinics generally have published self-pay rates and discounted packages precisely because so much of their volume is cash, and many will price a monitoring-only cycle differently from a full treatment cycle. Our fair price checker is a quick way to test a quote against the ranges on this page.

If you are already holding a bill from a clinic or hospital lab that you did not expect, our guide to negotiating medical bills applies here as much as anywhere, and cash-pay fertility practices are often more flexible than hospital systems.

How to Pay Less

  1. Price the tablet separately from the cycle. Ask the clinic for a prescription you can fill anywhere rather than accepting an in-house dispensing price. At $4.65 in acquisition cost per tablet, there is no reason a five-day course should cost triple digits.

  2. Run a discount card on the fill. The gap between the retail counter price and the card price on clomiphene was $161 on a single 15-tablet fill in September 2026. Our comparison of GoodRx vs. SingleCare covers checking both before you pay.

  3. Ask what the monitoring package costs before the cycle starts, not during it. Get the number of scans, the number of blood draws, and the price of each in writing. This is where the $1,000 to $2,000 lives, and it is the part clinics are most willing to adjust.

  4. Ask whether a trigger shot is actually part of your protocol. If it is, price both the 10,000 unit hCG vial and the Ovidrel prefilled syringe, because the gap between them ran to $137 in September 2026 and clinics do not always quote the cheaper one.

  5. Use an outside lab for the bloodwork where the clinic allows it. Hormone panels ordered directly are usually far cheaper than the same markers billed through a specialty practice. Our blood work cost guide covers direct-order pricing.

  6. Check whether a lower-cost clinic will handle the monitoring. A free or sliding-scale clinic will not run a fertility cycle, but it can often handle the baseline labs and the general medical workup that a specialty practice would bill at specialty rates.

Bottom Line

Budget $23 to $81 for a five-day course of clomiphene and $140 to $486 for 30 tablets. Budget $1,000 to $2,000 for a monitored ovulation-induction cycle, because the scans and the bloodwork are the actual product. Budget $248 to $385 if a trigger shot is used, and do not count on finding a compounded version, because the March 2020 biologics transition took that option off the table.
