“ADHD testing” is not one product, and that is the single biggest reason quotes range from under $200 to over $5,000. At one end is a diagnostic interview: a clinician takes your history, has you and someone who knows you complete standardized rating scales, rules out other explanations, and reaches a conclusion. At the other end is a psychological testing battery, several hours of standardized tests administered, scored, interpreted, and written up in a long report.
Both can end in an ADHD diagnosis. Only one of them costs four figures. This guide covers what each path actually costs cash in 2026, what the free and low-cost options are, and how to tell which product you are being quoted before you put down a deposit.
What Does ADHD Testing Cost Without Insurance?
The figures below are published cash or self-pay prices read in September 2026, plus national Medicare claims data as a reality check on what providers put on the bill.

Sources: CMS 2024 Medicare Physician and Other Practitioners public use file (released May 2026), Circle Medical, ADHD Online, Washington State University Psychology Clinic, The ADHD Center for Success, and Grey Matter Psychological Assessment, all read September 2026. Regional variation is large and the top of that table is a high-end practice, not a national average.
The Medicare data is useful because it shows what providers nationally charge rather than what they collect. For CPT 90791, the psychiatric diagnostic evaluation that anchors the cheap path, the average submitted charge in 2024 was $293.32 in an office setting and $309.47 in a facility, against average Medicare allowed amounts of $144.73 and $141.35. For CPT 90792, the same evaluation performed with medical services by a prescriber, the average submitted charge was $383.44 in an office against $171.35 allowed. That gap between billed and allowed is the negotiating room a cash payer is working with.
Telehealth platforms have collapsed the interview path into a flat published price. ADHD Online lists a $199 mental health diagnostic evaluation (or three installments of $73, a $219 total including a $20 admin fee), then $199 for an initial medical treatment appointment and $185 for follow-ups. Circle Medical publishes self-pay pricing of $179 for the first appointment and $120 per follow-up, and notes that reaching an ADHD treatment decision typically takes two to three appointments, putting the self-pay total near $419. Both pages were read on September 3, 2026, and these companies change prices often.
What Drives the Price
Which product you are actually buying. A diagnostic interview is one billable encounter. A testing battery is billed in time units under the psychological testing codes: 96130 for the first hour of test evaluation and interpretation, 96131 for each additional hour, and 96136 and 96137 for each 30 minutes of test administration and scoring. In the 2024 Medicare data, providers submitted an average of $288.14 for 96130, $168.61 for each additional 96131 hour, and roughly $125 to $128 per 30-minute administration unit. Six to ten of those units is how a quote reaches $2,000.
Who signs the report. Doctoral-level psychologists price above nurse practitioners and primary care physicians, and neuropsychologists price above general clinical psychologists. If the real referral question is a broader cognitive problem rather than attention specifically, you are shopping for a different and more expensive service. Our guide to neuropsychological testing cost covers that one, which is billed per hour under the 96132 and 96133 codes and priced accordingly.
Whether a written report is included. The report is often the most expensive hour in the process, and it is the thing schools, employers, and testing agencies actually want. The ADHD Center for Success publishes a $1,450 adults-only evaluation with a 5 to 6 page summary and no follow-up session, against $1,650 for the comprehensive version with a feedback session. That $200 difference is essentially the longer report plus the debrief.
How many questions you are asking at once. Adding executive functioning or sensory processing raised that same practice’s price from $1,650 to $1,795, and to $1,950 with both. Batteries that also assess autism or a learning disorder cost far more: Grey Matter Psychological Assessment publishes $5,875 to $7,250 for a combined ADHD and autism evaluation and $5,880 to $8,725 for an educational evaluation covering learning disorders, each involving 4 to 6 hours of testing plus a 30 to 40 page report.
Where you live and who owns the practice. Metro-area practices price above small-city practices for the same battery. Because psychological testing usually carries no facility fee, essentially the entire spread is the clinician’s hourly rate and the number of hours they budget.
Cash Price vs. Insurance
ADHD evaluation is one of the services where paying cash is often the rational move, for three reasons.
First, plans routinely require prior authorization for psychological testing and deny it when the referral question is “rule out ADHD” without documented functional impairment. A denial after the fact leaves you owing the full billed charge, which the Medicare data shows is roughly twice the negotiated rate.
Second, many testing psychologists are out of network by default. If you are going to pay the full fee either way, paying it as a disclosed flat rate beats paying it as a surprise. Ask for a superbill so you can submit for out-of-network reimbursement yourself.
Third, the interview path is priced for cash already. The telehealth platforms above quote flat self-pay numbers on their public pricing pages, and Circle Medical notes that insured patients averaged about $35 per appointment out of pocket over the prior six months, which is not far below its $120 cash follow-up price once a deductible enters the picture.
How to Pay Less
Start with the interview, not the battery. The CDC notes there is no single test for ADHD, and that primary care providers, psychologists, and psychiatrists can all make the diagnosis from a medical exam, rating scales, and information gathered across settings. A $179 to $293 diagnostic interview answers the question for a large share of people. Buy the four-figure battery when the interview does not settle it, not before. Our psychiatrist cost guide covers what the prescriber visit itself runs.
Use the school system if the patient is a child. Under 34 CFR 300.301, a public agency must conduct a full initial evaluation before providing special education services, and must complete it within 60 days of receiving parental consent unless the state sets its own timeframe. There is no charge to parents. Put the request in writing and date it, because the clock starts at consent, and the regulation lists only narrow exceptions (a parent repeatedly failing to produce the child, or a mid-evaluation transfer between districts).
Call the nearest university psychology department. Training clinics run supervised assessments at a fraction of private-practice rates. Washington State University’s Psychology Clinic charges a $50 intake plus $500, $600, or $700 for an ADHD assessment depending on household income, on a fee schedule effective May 2025, with the balance due in full on the first day of testing. Expect a waitlist.
Try a federally funded health center or community mental health center. These charge on an income-based sliding scale and cannot turn you away for inability to pay. Our guide to free clinics and sliding-scale care covers how to find one and what to bring to document income.
Negotiate the flat fee, and pin down what it includes. Testing practices set their own prices, and many will discount for payment in full up front or split a flat fee across the intake, testing day, and feedback session. Ask specifically whether the quote covers scoring, the written report, and the feedback appointment, because those are the line items that reappear later. The tactics in how to negotiate medical bills work the same way on a testing quote as on a hospital bill, and you have far more leverage before the service than after it.
Price the treatment, not just the test. The evaluation is a one-time cost. If a stimulant follows, that is a recurring one, and generic prices vary enormously between pharmacies. See our Adderall cost without insurance guide, and run any quoted evaluation price through the fair price checker before you accept it.
What You Actually Need for a Prescription
For most people the answer is a licensed prescriber who has done a diagnostic evaluation, not a stack of test scores. Psychiatrists, psychiatric nurse practitioners, and many primary care physicians can evaluate and prescribe, which is why telehealth platforms can price a complete path at $199 to $419. Controlled-substance rules and state licensing determine who can prescribe where, so confirm that a platform actually prescribes in your state before you pay.
Where a full battery genuinely earns its cost is documentation. Extended time on standardized exams, workplace accommodation requests, an individualized education program, and disability paperwork usually require formal testing with scores and a written report. If nobody is going to read a report, you may be paying for a document you do not need.
Bottom line: get the quote in writing, confirm which CPT codes it covers, and ask whether the clinic will do the interview first and add testing only if it turns out to be necessary. That one question is worth more than any coupon.
