Almost nobody searching for the price of Accutane is actually shopping for Accutane. The original Roche product left the market years ago, the name came back as an Abbreviated New Drug Application marketed by JG Pharma (its FDA labeling on DailyMed was updated January 30, 2026), and what pharmacies actually dispense is generic isotretinoin under names like Absorica, Claravis, Myorisan, Amnesteem and Zenatane. CMS prices all of them as generics.
That matters for your wallet, because the capsules turn out to be cheap and the paperwork turns out not to be. Isotretinoin is dispensed only through the FDA’s iPLEDGE program, which layers registered prescribers, registered pharmacies, monthly check-ins and monthly lab work on top of the fill. This guide prices the drug and the program separately, using CMS acquisition data effective August 19, 2026 and cash prices checked September 2, 2026, so you can see where the money actually goes.
What Does Accutane Cost Without Insurance?
Isotretinoin is dosed by body weight and taken in two divided doses, so a month is usually 60 capsules rather than 30. The rows below use 60 capsules of the 30mg strength, the same quantity SingleCare prices on its isotretinoin page.
| Item | Low | Average | High |
|---|---|---|---|
| Isotretinoin, per capsule, CMS NADAC (all strengths) | $1.86 | n/a | $6.75 |
| Isotretinoin 30mg, 60 capsules, NADAC acquisition cost | $130.46 | n/a | $221.03 |
| Isotretinoin 30mg, 60 capsules, SingleCare card price | $82.31 | n/a | $82.31 |
| Isotretinoin 30mg, 60 capsules, average retail | n/a | $1,284.27 | n/a |
| Lipid panel, cash price (Quest, includes $6 physician fee) | $65.00 | $65.00 | $65.00 |
| Liver health blood test, cash price (Labcorp OnDemand) | $79.00 | $79.00 | $79.00 |
| Teledermatology Accutane program, whole course | $500 | $500 | $500 |
| Full 15-to-20-week course, drug plus visits plus labs | $1,050 | n/a | $2,050 |
The unit math is worth showing because it is the whole argument. NADAC, the CMS survey of what retail pharmacies actually pay for a drug, priced isotretinoin 30mg capsules at $2.17435 and $3.68377 per capsule (two different manufacturers) on its August 19, 2026 effective date. Sixty capsules is therefore $130.46 to $221.03 of acquisition cost. The 40mg capsule ranged from $2.18492 to $6.74721, and the 20mg from $1.86128 to $4.94159, so the per-capsule spread across the whole generic market is roughly $1.86 to $6.75.
Against that, SingleCare listed a $1,284.27 average retail price for a one-month supply and a $82.31 card price for the same quantity. Note what that means: the discount price sits below what the pharmacy paid for the bottle. Named-chain prices on discount-card sites move between visits, so treat any single pharmacy’s quote as a sample rather than a fixed price, but the average retail and card figures were stable.
What Drives the Price
The strength and the manufacturer, not the brand. Two manufacturers of the identical 40mg capsule were $2.18 and $6.75 per capsule in the same NADAC file. Nothing distinguishes them clinically. Which bottle your pharmacy stocks is a supply-chain accident that can triple your drug cost.
Your weight. Isotretinoin is dosed per kilogram in two divided doses, so a heavier patient simply swallows more capsules per month than a lighter one. Two people on the same regimen at the same pharmacy can have meaningfully different bills.
Course length. The FDA-approved labeling describes a single course of therapy as 15 to 20 weeks. That five-week spread is a full extra month of drug, labs and a check-in, which is real money at cash prices.
The iPLEDGE machinery. This is the big one, and it is covered in its own section below.
Whether you missed a pickup window. Under iPLEDGE, a prescription has to be collected inside a set window or the authorization lapses and screening has to be repeated. That is not a paperwork annoyance, it is a repeat bill.
The iPLEDGE Program Is the Expensive Part
The FDA’s iPLEDGE REMS exists to prevent fetal exposure, and it is a shared system covering every FDA-approved isotretinoin product. Prescribers, pharmacies and patients all have to be registered. Patients who can become pregnant must complete pregnancy testing before, during and after treatment. Everyone on the drug has monthly prescriber contact and monitoring bloodwork.
Priced at cash lab rates, that adds up quickly. Quest’s self-pay Cholesterol (Lipid) Panel was $59.00 plus a $6.00 physician service fee, and Labcorp OnDemand’s Liver Health Test was $79.00, both checked September 2, 2026. One round of both is $144. Three rounds across a course is $432, before any pregnancy testing.
The best evidence on what this costs in practice comes from a 2021 study in Pediatric Dermatology, archived in NIH’s PMC, that tracked 173 patients at a children’s hospital from 2012 to 2019. Excluding the drug entirely, female patients incurred $502.53 to $768.40 in visits and testing per course, and the higher figure belonged to patients who had missed at least one prescription window. Total billed treatment costs averaged $3,246.53 for males and $2,736.91 for females. Those are billed charges from an academic center rather than cash prices, so a careful self-pay patient today should land well under them, but they show where the money sits.
There is live movement here. FDA approved modifications to iPLEDGE on February 9, 2026 intended to reduce burden, including allowing at-home pregnancy testing during and after treatment when a prescriber permits it, and dropping the 30-day prescription window for patients who cannot become pregnant. Implementation was pushed from August 8, 2026 to November 15, 2026. Whether any of that reaches your bill depends on your prescriber’s process, and none of it changes the requirement for pre-treatment testing in a medical setting. Eligibility and contraception questions are entirely between you and your prescriber; what we are pricing here is the visit and lab overhead the program creates.
Cash Price vs. Insurance
Isotretinoin is one of the few expensive-sounding drugs where a cash payer is not badly disadvantaged on the medication itself. The molecule is generic, NADAC puts acquisition at roughly $2 to $7 a capsule, and a free discount card was quoting $82.31 a month. A commercially insured patient with a $50 copay is not saving much over that.
Where insurance genuinely helps is the wraparound. Monthly office visits and repeat lab panels are where the uninsured bill diverges, because a covered patient pays a copay per visit while you pay the practice’s self-pay rate. That is also the piece you have the most room to shop, since visit and lab pricing vary far more than drug pricing does.
If you do have coverage, ask the pharmacy to run the claim and the discount card separately and take whichever is lower on each fill. You cannot stack them. If a plan denies the drug, a formulary exception is worth filing here in a way it is not for a $10 generic, because the retail number behind a denial can be four figures.
How to Pay Less
-
Price the fill with a discount card before you pick it up. The gap between $82.31 and a $1,284.27 average retail price is the single largest lever on this page, and pulling it is free. Compare both major networks first using our guide to GoodRx vs. SingleCare, because the winner changes by chain and zip code.
-
Ask which manufacturer your pharmacy stocks. NADAC shows a three-fold spread between generic isotretinoin manufacturers at the same strength. If your pharmacy carries the expensive one and your card price reflects it, a different pharmacy may carry the cheap one.
-
Buy your labs as cash lab tests instead of through a clinic draw. Quest’s lipid panel was $65 all-in and Labcorp OnDemand’s liver panel $79 as of September 2, 2026. Clinic-ordered versions of the same panels are frequently billed at multiples of that. Our blood work cost guide covers how to price a panel before you sit down.
-
Compare a flat-fee teledermatology program against per-visit dermatology. DermOnDemand publishes a $500 consultation fee for its Accutane program (a yearly service), $200 for other chronic conditions and $95 for a one-time acute visit. A flat program fee can beat four or five separately billed office visits, or it can lose to them. Price your local option using our dermatologist cost guide before you commit.
-
Do not miss a pickup window. The published research says this is the most expensive avoidable mistake in the whole process, adding hundreds of dollars in repeat screening and extending the course.
-
Use income-based pricing if you qualify. Federally qualified health centers and sliding-scale clinics can prescribe and monitor at a fraction of a private practice’s self-pay rate. See free clinics and sliding-scale care. If a bill has already landed, our medical bill negotiation guide and the negotiation script tool are the next stop.
Bottom Line
Budget $82 to $221 a month for the drug and $1,050 to $2,050 for a complete cash-pay course as of September 2026. If a pharmacy quotes you four figures for a month of isotretinoin, you are looking at an uncouponed retail price, not a real one. Check that number against our fair price checker before you pay it.
If you are pricing acne treatment more broadly, the topical route is a different order of magnitude and is covered in our tretinoin cost guide. For inflammatory skin conditions treated with biologics rather than retinoids, see our cost guides for Dupixent, Skyrizi, Tremfya, Rinvoq and Otezla.
